A Pragmatic Randomized Controlled Trial of Financial Incentives in Case Management for Homeless Adults With Mental Illness.

OBJECTIVE: Financial incentives can facilitate behavior change and service engagement in health care settings, but research on their use with adults experiencing homelessness is limited. This study examined the effectiveness of financial incentives in improving service engagement and health outcomes among homeless adults with mental illness in Toronto. METHODS: The authors of this randomized controlled trial recruited 176 participants receiving brief multidisciplinary case management services for homeless adults with mental illness after hospital discharge. In a 1:1 randomization design, 87 p

Cost-effectiveness of Financial Incentives for Patients and Physicians to Manage Low-Density Lipoprotein Cholesterol Levels.

IMPORTANCE: Financial incentives shared between physicians and patients were shown to significantly reduce low-density lipoprotein cholesterol (LDL-C) levels in a randomized clinical trial, but it is not known whether these health benefits are worth the added incentive and utilization costs required to achieve them. OBJECTIVE: To evaluate the long-term cost-effectiveness of financial incentives on LDL-C level control. DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation, a previously validated microsimulation computer model was parameterized using individual-level data from the ran

Financial impact of intravenous iron treatments on the management of anaemia inpatients: a 1 year observational study.

Background Intravenous (IV) iron preparations bypass the difficulties (malabsorption and side effects) associated with oral iron for the treatment of iron deficiency anaemia (IDA). Ferric carboxymaltose (FCM) can be administered as a single infusion over short periods of time but is more expensive than iron sucrose (IS) when the patients are hospitalized. Objectives To evaluate the appropriateness of FCM prescriptions and to establish the economic impact of this management (including disease coding) compared to the use of IV IS. Setting This study was conducted for inpatients in all department

The holistic management of consequences of cancer treatment by a gastrointestinal and nutrition team: a financially viable approach to an enormous problem?

There is no national NHS tariff to fund services for patients experiencing long-term bowel and nutritional problems after cancer treatment. In this paper, we report the clinical characteristics and outcomes of patients referred to our service and the estimated cost of a completed episode of care. Patient characteristics, symptom severity, investigations, diagnoses, number of clinic visits and referrals elsewhere were recorded in a prospective cohort study. During 2013-14, 325 patients completed assessment and treatment. The majority of original cancer diagnoses were urological (43%) and gynaec

Unveiling the Financial Burden of Systemic Lupus Erythematosus Management in Saudi Arabia: Insights from a Single-Center Study.

BACKGROUND: Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder that impacts multiple organs. In Saudi Arabia, the prevalence of SLE is about 19 per 100,000 people, primarily affecting women at a 9:1 ratio. This study aimed to estimate the direct medical costs of managing SLE in Saudi Arabia. METHODS: Data were collected from electronic medical records at a university-affiliated tertiary care center using a micro-costing approach over a 12-month period, encompassing the costs for laboratory tests, imaging, medications, and outpatient visits. A Generalized Linear Model (GLM) wa

Financial constraints on genetic counseling and further risk-management decisions among U.S. women at elevated breast cancer risk.

Clinical guidelines recommend that women at high risk of breast cancer should consider various risk-management options, which remain widely underutilized. We conducted semi-structured, qualitative interviews with 50 high-risk women to understand how financial constraints affect use of genetic counseling, genetic testing, and further risk-management decisions. Inductive analyses revealed three categories of health-related financial constraint: (a) lack of insurance, (b) underinsurance, and (c) other financial constraints (e.g., medical debt, raising children, managing comorbidities). Various br

The impact of digitalization policies on teaching behaviors in university financial management programs: a teacher-student dual-path model.

INTRODUCTION: Against the backdrop of educational digitalization and the urgent need for the "data-intelligence" transformation of Financial Management programs, this study investigates the mechanisms through which digitalization policies affect teaching and learning behaviors. To overcome the limitations of previous research, which often focused solely on either the teacher or student perspective, this study innovatively develops a "Teacher-Student Dual-Path Model." METHODS: Adopting a non-matched independent sampling design, we collected survey data from 253 faculty members and 280 students

A double-edged sword? Exploring the heterogeneous effects of industrialization, energy efficiency, and disaggregated financial development on environmental management.

This study investigates the impact of industrialization, technological innovation, financial sector development, and energy efficiency on the ecological footprint within emerging and growth-leading economies (EAGLE) countries (i.e., Brazil, China, India, Indonesia, Mexico, Russia, and Turkey) over the period 1990-2022. Recognizing the interconnectedness of these economies, the analysis accounts for cross-sectional dependence and heterogeneity by employing advanced second-generation panel econometric techniques, particularly the Method of Moments Quantile Regression (MMQR). The empirical result

Combining the benefits of decision science and financial analysis in public health management: a county-specific budgeting and planning model.

State public health agencies are charged with providing and overseeing the management of basic public health services on a population-wide basis. These activities have a re-emphasized focus as a result of the events of September 11, 2001, the subsequent anthrax events, and the continuing importance placed on bioterrorism preparedness, West Nile virus, and emerging infectious diseases (eg, monkeypox, SARS). This has added to the tension that exists in budgeting and planning, given the diverse constituencies that are served in each state. State health agencies must be prepared to allocate finite

Financial analysis of East Coast fever control strategies in traditionally managed Sanga cattle in Central Province of Zambia.

Five different East Coast fever (ECF)-control strategies (involving ECF immunisation by the infection-and-treatment method) were tested in groups of traditionally managed Sanga cattle in the Central Province of Zambia over a period of 2.5 years. Two groups were under intensive tick control (weekly spraying with acaricide)--one group immunised and the other non-immunised. Two groups were under no tick control--one group immunised and the other non-immunised. The fifth group was under seasonal tick control (18 sprays/year) and was immunised against ECF. The input and output data were used to con

Household financial management behavior and catastrophic health expenditure risk in rural China: the moderating role of health insurance coverage.

INTRODUCTION: Catastrophic health expenditure (CHE) represents a major cause of poverty among rural households in China. Although health insurance coverage in rural China exceeds 95%, CHE incidence remains high, and existing research has predominantly focused on external protection mechanisms such as health insurance, with insufficient attention to household financial management capacity. METHODS: Drawing on data from 17,156 rural households in the 2019 China Household Finance Survey, this study constructed a financial management behavior index encompassing four dimensions: saving behavior, a

Financial Burden, Insurance Type, and Medication Adherence Among Community-Managed Patients with Hyperthyroidism: A Cross-Sectional Study from Nanning, China.

BACKGROUND: Hyperthyroidism necessitates prolonged pharmacotherapy; however, adherence is frequently inadequate in community settings. Although knowledge gaps are frequently cited, the impact of socioeconomic factors and health system fragmentation - particularly in middle-income countries undergoing health reforms - remains less understood. The present study aimed to investigate the prevalence of non-adherence and its association with socioeconomic status, out-of-pocket expenditure, and health insurance schemes among community-dwelling patients with hyperthyroidism in urban China, with a spec
