Fluid shifts are main drivers for microgravity simulation-induced immune-physiological changes: findings from the VIVALDI studies.

Microgravity strongly affects human physiology during spaceflight. Biological sex has not yet been sufficiently considered as a variable for spaceflight deconditioning. The VivalDI studies investigated physiological systems affected by 5-days dry immersion (DI) in females and males, with a focus on immune changes in this report. In both sexes proportions of peripheral granulocytes and NK cells were elevated during DI and T-cell numbers were reduced. Leukocyte activation and cytokine levels were moderately affected. Females showed a higher Torque-Teno-virus shedding at the end of DI. Noradrenal

Polycystic Ovary Syndrome: Unraveling the Minor Shifts in Fatty Acid Composition of Follicular Fluid Phospholipids and Triglycerides.

The effect of polycystic ovary syndrome (PCOS) on the fatty acid (FA) content of follicular fluid (FF) is not fully understood. The present study aimed to determine whether the FA composition of FF phospholipids (PLs) and triglycerides (TGs) undergoes alterations in women with PCOS. A total of 40 subjects, including 20 PCOS patients and 20 controls, were enrolled. Thin-layer chromatography followed by gas chromatography was carried out to isolate FF lipid fractions and measure relative concentrations of their FAs, respectively. Percentages of individual FAs in FF PLs and TGs did not statistica

[Factors influencing severity variability in obstructive sleep apnea and the role of fluid shift].

Objective:The variability of the apnea-hypopnea index(AHI) measured in the first and second halves of the night is significant in patients with obstructive sleep apnea hypopnea syndrome(OSAHS). This variation may be related to fluid redistribution caused by the supine position during sleep. Methods:Eighty-nine adult subjects were enrolled. Circumferences(neck, chest, waist, and calf) were measured before sleep onset and upon awakening. Polysomnography(PSG) was performed, and the night was divided into two halves based on the midpoint of total sleep time to calculate AHI for each half. The corr

Midline Shift Induced by the Drainage of Cerebrospinal Fluid in Three Patients With External Decompression.

It is not rare that progressive hydrocephalus worsens clinical conditions in a patient with external decompression and drainage or shunt surgery is required. However, spinal drainage or shunt surgeries potentially carry a risk of causing paradoxical herniation in a patient with decompressive craniectomy, particularly in a comatose case with wide craniectomy. Careful and strict observations are necessary for such patients. In our three comatose cases with craniectomy, paradoxical herniation occurred due to excessive drainage after 5-7 days of shunt surgery and lumbar drainage, although the drai

Role of nocturnal rostral fluid shift in the pathogenesis of obstructive and central sleep apnoea.

Obstructive sleep apnoea (OSA) is common in the general population and increases the risk of motor vehicle accidents due to hypersomnolence from sleep disruption, and risk of cardiovascular diseases owing to repetitive hypoxia, sympathetic nervous system activation, and systemic inflammation. In contrast, central sleep apnoea (CSA) is rare in the general population. Although their pathogenesis is multifactorial, the prevalence of both OSA and CSA is increased in patients with fluid retaining states, especially heart failure, where they are associated with increased mortality risk. This observa

A Shift in Glycerolipid Metabolism Defines the Follicular Fluid of IVF Patients with Unexplained Infertility.

Follicular fluid (FF) constitutes the microenvironment of the developing oocyte. We recently characterized its lipid composition and found lipid signatures of positive pregnancy outcome after in vitro fertilization (IVF). In the current study, we aimed to test the hypothesis that unexplained female infertility is related to lipid metabolism, given the lipid signature of positive-outcome IVF patients we previously found. Assuming that FF samples from IVF patients with male factor infertility can represent a non-hindered metabolic microenvironment, we compared them to FF taken from women with un

ALM Fluid Therapy Shifts Sympathetic Hyperactivity to Parasympathetic Dominance in the Rat Model of Non-Compressible Hemorrhagic Shock.

Excessive sympathetic outflow following trauma can lead to cardiac dysfunction, inflammation, coagulopathy, and poor outcomes. We previously reported that buprenorphine analgesia decreased survival after hemorrhagic trauma. Our aim is to examine the underlying mechanisms of mortality in a non-compressible hemorrhage rat model resuscitated with saline or adenosine, lidocaine, magnesium (ALM). Anesthetized adult male Sprague-Dawley rats were randomly assigned to Saline control group or ALM therapy group (both n = 10). Hemorrhage was induced by 50% liver resection. After 15 min, 0.7 mL/kg 3% NaCl

Obstructive Sleep Apnea Severity and Overnight Body Fluid Shift before and after Hemodialysis.

BACKGROUND AND OBJECTIVES: Obstructive sleep apnea is associated with significantly increased cardiovascular morbidity and mortality. Fluid overload may promote obstructive sleep apnea in patients with ESRD through an overnight fluid shift from the legs to the neck soft tissues. Body fluid shift and severity of obstructive sleep apnea before and after hemodialysis were compared in patients with ESRD. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Seventeen patients with hemodialysis and moderate to severe obstructive sleep apnea were included. Polysomnographies were performed the night before

Physical exercise related improvement in obstructive sleep apnea. Look for the rostral fluid shift.

Obstructive sleep apnea (OSA) is a common and underdiagnosed medical disorder. OSA is associated with the symptoms of excessive daytime sleepiness (EDS). These patients typically follow a sedentary lifestyle, and sedentary behavior is related to impaired fluid dynamics in the lower body, particularly the legs. In a supine position this fluid can move towards the neck, with a subsequent increase in upper airway (UA) resistance and UA collapse. Several studies have shown that rostral fluid shift worsens OSA; however, whether physical activity can influence this has not been tested. Physical acti

Fluid shift by lower body positive pressure increases pharyngeal resistance in healthy subjects.

INTRODUCTION: Fluid displacement into nuchal and peripharyngeal soft tissues while recumbent may contribute to narrowing and increased airflow resistance of the pharynx (Rph), and predispose to pharyngeal collapse in patients at risk for obstructive sleep apnea. OBJECTIVES: To determine whether displacement of fluid from the lower body to the neck will increase both neck circumference and Rph in healthy subjects. METHODS: In 11 healthy, nonobese subjects, studied while awake and supine, leg fluid volume, neck circumference, and Rph were measured at baseline. Subjects were then randomized to

Transepithelial fluid shift generated by osmolarity gradients in unstimulated perfused rat submandibular glands.

The effects of osmotic gradients on transepithelial water movements were examined in unstimulated perfused submandibular glands of the rat. Osmotic gradients were applied transepithelially by adding sucrose to or removing it from the perfusate. An infusion of hypotonic perfusate shifted fluid from the interstitium to the lumen (luminal fluid shift) transiently, whereas an infusion of hypertonic perfusate shifted fluid from the lumen to the interstitium (interstitial fluid shift) transiently. The amount of fluid shifted from lumen to interstitium increased as the luminal fluid osmolarity was ra

Fluid shift across the placenta: II. Fetomaternal transfer of horseradish peroxidase in the guinea pig.

The fetal side of the isolated guinea-pig chorio-allantoic placenta has been perfused with a fluid containing horseradish peroxidase. Electron micrographs show that protein is transferred across the fetal capillary endothelium via micropinocytosis as well as by transport through intercellular clefts. Under normal hydrostatic pressure no pinocytosis or other signs of fetomaternal protein transfer across the trophoblast have been observed. Instead, macrophages ingest the proteins that have reached the interstitial space between fetal endothelium and trophoblast. When the fetal venous pressure is

Effects of rostral fluid shift on small airway narrowing in asthma before and after bronchodilation.

PURPOSE: The recumbent posture during sleep increases small airway narrowing and nocturnal asthma despite optimal asthma treatment. Fluid accumulation in the chest can narrow the airway lumen, contributing to small airway narrowing. We hypothesized that in people with asthma, treatment with a short-acting bronchodilator cannot fully prevent airway narrowing attributable to rostral fluid shift. METHODS: Using a cross-over design, participants laid supine for 30 minutes and received lower body positive pressure (LBPP) to reproduce overnight fluid shift out of the legs. They either inhaled salbut

Postural respiratory-related cortical activation and rostral fluid shift in awake healthy humans.

NEW FINDINGS: What is the central question of this study? Moving to supine induces upper airway modifications and a fluid shift to the neck, which represent inspiratory load that predisposes to upper airway collapse. Is there cortical participation in the response to the load induced by transition to a supine posture in awake healthy subjects? What is the main finding and its importance? Moving to supine induces transient cortical activation in awake healthy subjects, with greater fluid shift, supporting possible cortical participation in the response to upper airway load induced by transition

Fluid shift from intravascular compartment during fetal red blood cell transfusion.

OBJECTIVES: Intrauterine transfusion imposes a considerable burden on the fetal circulation by increasing volume and pressure, and a fluid shift from the fetal circulation occurs even during the procedure. The aim of this study was to quantify the intraprocedural fluid shift and to investigate the effect of procedural and fetal characteristics on this fluid shift. METHODS: In 95 alloimmunized pregnancies, we calculated fluid shift at the first intrauterine transfusion by determining initial and final blood volumes. We evaluated the association of the fluid shift with the speed and volume of t

Effects of fluid management on edema volume and midline shift in a rat model of ischemic stroke.

BACKGROUND AND PURPOSE: The purpose of this study was to investigate the effects of fluid management on brain water content (BW) and midline shift (MLS) after a focal cerebral ischemic insult. METHODS: A suture model was used to induce focal cerebral ischemia for 90 minutes (n=44). The rats were randomly assigned to 3 groups 2. 5 hours after reperfusion: dehydration (n=24), control (n=8), or hydration (n=12). BW was obtained with the wet-dry weight method 24 hours after middle cerebral artery (MCA) occlusion. In addition, MRI were obtained (n=31) 24 hours after the onset of ischemia so that t

Cerebrospinal fluid metabolomics implicate bioenergetic adaptation as a neural mechanism regulating shifts in cognitive states of HIV-infected patients.

OBJECTIVES: To identify prognostic surrogate markers for change in cognitive states of HIV-infected patients. DESIGN: Longitudinal cerebrospinal fluid (CSF) samples were collected from 98 HIV-infected patients identified by temporal change in cognitive states classified as normal, stably impaired, improving and worsening. METHODS: The metabolic composition of CSF was analysed using H nuclear magnetic resonance (H NMR) spectroscopy that focused on energy metabolites. Metabolic biomarkers for cognitive states were identified using multivariate partial least squares regression modelling of the

Daily use of countermeasures to prevent headward fluid shifts throughout 30 days of strict head-down tilt bed rest.

Strict 6° head-down tilt bed rest (HDTBR) induces a chronic headward fluid shift similar to weightlessness that may underlie the development of spaceflight-associated neuro-ocular syndrome. We tested the efficacy of two headward fluid shift countermeasures, using either 25 mmHg lower body negative pressure (LBNP, 2 × 3 h daily, n = 12) or use of venoconstrictive thigh cuffs (VTC) following 1 h of moderate aerobic cycle exercise (Ex + CUFF, 6 h daily for 6 of 7 days, n = 12) across 30 days of HDTBR. The study also included a seated posture group (Upright, 2 × 3 h daily, n = 11), which was condu
