Safety and tolerability of fluoroquinolones.

Since their introduction in the mid-1980s, the fluoroquinolones have been administered to more than 100 million patients. Generally, adverse effects reported in association with the fluoroquinolones have been those that could have been predicted by previous experience with non-fluorinated derivatives and by animal toxicity studies. Examples of such adverse events are CNS-related toxicity, upper gastrointestinal tract reactions and phototoxicity. Some adverse experiences in animals, notably cartilage toxicity, have been of minimal clinical importance. This should lead to a re-evaluation of the

Fluoroquinolones: old drugs, putative new toxicities.

INTRODUCTION: Fluoroquinolone (FQ) antibiotics were approved in 1986 for treatment of urinary tract infections, sinusitis, and bronchitis. Numerous putative FQ-associated adverse events have been recently reported. AREAS COVERED: We review international regulatory agency experience with these FQ-associated toxicities. A 2015 FDA Advisory Committee meeting led regulatory agencies in Canada, Australia, the European Union, New Zealand, and Japan between 2017 and 2021 to evaluate FQ-associated long-term disability and aortic aneurysm/dissections. Regulatory agency guidance in the United States in

Chemical tuning for potential antitumor fluoroquinolones.

Phototoxic effects of 6,8 dihalogenated quinolones confers to this type of molecules a potential property as photochemotherapeutic agents. Two photodehalogenation processes seem to be involved in the remarkable photoinduced cellular damage. In this context, a new 6,8 dihalogenated quinolone 1 (1-methyl-6,8-difluoro-4-oxo-7-aminodimethyl-1,4-dihydroquinoline-3-carboxylic acid) was synthetized looking for improving the phototoxic properties of fluoroquinolones (FQ) and to determine the role of the photodegradation pathways in the FQ phototoxicity. With this purpose, fluorescence emissions, laser

Appropriate use of fluoroquinolones in children.

With the increasing resistance to antibiotics among common bacterial pathogens, challenges associated with the use of fluoroquinolones (FQs) in paediatrics have emerged. The majority of FQs have favourable pharmacokinetic properties, although these properties can differ in children compared with adults. Moreover, all FQs have broad antimicrobial activity both against Gram-positive and Gram-negative bacteria. However, only some FQs for which adequate studies are available have been approved for use in children in a limited number of clinical situations owing to the supposed risk of development

The use of fluoroquinolones in children.

PURPOSE OF REVIEW: The purpose of this review is to better understand the spectrum of disease in torticollis, which is the third most common pediatric orthopaedic diagnosis in childhood. Besides the benign muscular tightness of the sternocleidomastoid muscle leading to the classic head position, the differential diagnosis of the wry neck include sequelae to inflammatory, ocular, neurologic or orthopedic diseases. Patients present with a stiff and tilted neck, and therefore require a thorough and systematic work-up, including a complete physical and neurologic examination and cervical spine rad

Fluoroquinolones in the Treatment of Meningitis.

The continuous increase of resistant pathogens causing meningitis has limited the efficacy of standard therapeutic regimens. Due to their excellent activity in vitro and their good penetration into the cerebrospinal fluid (CSF), fluoroquinolones appear promising for the treatment of meningitis caused by gram-negative microorganisms, ie, Neisseria meningitidis and nosocomial gram-negative bacilli. The newer fluoroquinolones (moxifloxacin, gemifloxacin, gatifloxacin, and garenoxacin) have excellent activity against gram-positive microorganisms. Studies in animal models and limited clinical data

Topical fluoroquinolones for eye and ear.

Topical fluoroquinolones are now available for use in the eye and ear. Their broad spectrum of activity includes the common eye and ear pathogens Staphylococcus aureus and Pseudomonas aeruginosa. For the treatment of acute otitis externa, these agents are as effective as previously available otic preparations. For the treatment of otitis media with tympanic membrane perforation, topical fluoroquinolones are effective and safe. These preparations are approved for use in children, and lack of ototoxicity permits prolonged administration when necessary. Topical fluoroquinolones are not appropriat

Trends in bacterial resistance to fluoroquinolones.

The emergence of resistance to fluoroquinolones in virtually all species of bacteria was recognized soon after the introduction of these compounds for clinical use more than 10 years ago. Various resistance mechanisms, often interdependent, may explain different levels of resistance. Epidemiological factors, local antibiotic policies, patients' characteristics, origin of the strains, and geographic location are among the factors contributing to highly variable resistance rates. During the last several years, resistance to fluoroquinolones has remained very high among methicillin-resistant Stap

The activity of the methylpiperazinyl fluoroquinolone CG 5501: a comparison with other fluoroquinolones.

The in-vitro activity of CG 5501 against a wide range of recent clinical isolates was compared with that of three fluoroquinolones. CG 5501 inhibited 90% of the species of the family Enterobacteriaceae at 0.5 mg/L or less, exceptions being Enterobacter spp. (MIC90 2 mg/L) and Serratia spp. (MIC90 4 mg/L). Ninety per cent of Pseudomonas aeruginosa, Stenotrophomonas maltophilia and Acinetobacter spp. were inhibited by 16, 4 and 1 mg/L respectively. CG 5501 had high activity against Gram-positive cocci, 90% of staphylococci being inhibited at 2 mg/L. Methicillin-resistant Staphylococcus aureus st

Fluoroquinolones and Biofilm: A Narrative Review.

Background: Biofilm-associated infections frequently span multiple body sites and represent a significant clinical challenge, often requiring a multidisciplinary approach involving surgery and antimicrobial therapy. These infections are commonly healthcare-associated and frequently related to internal or external medical devices. The formation of biofilms complicates treatment, as they create environments that are difficult for most antimicrobial agents to penetrate. Fluoroquinolones play a critical role in the eradication of biofilm-related infections. Numerous studies have investigated the s

Oral fluoroquinolones and risk of fibromyalgia.

The Food and Drug Administration (FDA) has recently included a black box warning on fibromyalgia-like symptoms with fluoroquinolones (FQs) but no large epidemiologic study is to date available. We undertook a case-control study using a random sample of 9 053 240 subjects obtained from the PharMetrics Plus health claims database in the United States. Cases received at least two fibromyalgia diagnoses coded by a rheumatologist matched to ten randomly selected controls. After identifying 5148 cases of fibromyalgia and 51 480 controls, the adjusted rate ratio (RR) of fibromyalgia for use of any fl

[Adverse effects and interactions of fluoroquinolones].

Floroquinolones are derivatives of nalidixic acid that act as a large-spectrum antibiotics. Adverse effects and interactions of the particular fluoroquinolone depend on its chemical structure and, often, on predisposing factors in patients, including the age, hidden or previous neurological diseases, metabolic disturbances, and allergies. The adverse effects do not significantly differ among different fluoroquinolones. They can be considered as mild, moderate and severe, and their incidence is irrespective of the gender. They can occur even after the short-term administration of these antibiot

[Corneal precipitation of fluoroquinolones with magnesium].

Infrared spectrophotometric analysis (FTIR) was performed on a crystalline deposit developed in a corneal ulcer by an old woman who received ciprofloxacine ophthalmic drops. We collected the data of the literature on the subject. After in vitro crystallization experiments, we conclude that ciprofloxacin and norfloxacin corneal precipitates occur at physiological lachrymal pH with magnesium.

Fluoroquinolones: interaction profile during enteral absorption.

Fluoroquinolones are used worldwide in the treatment of severe infections. These drugs, however, can interact with other agents. This paper is a review of drug interactions with different quinolone derivatives at the absorption phase; the review deals mainly with the prototype quinolones, ciprofloxacin and ofloxacin, and also with some of the newer agents. The concomitant agents considered are food, H2-receptor antagonists, anticholinergic drugs and metallic cation-containing compounds. Food (standard breakfast), H2-receptor antagonists and anticholinergic drugs had no major effect on the bioa

Adverse reactions and interactions of fluoroquinolones.

Recent research into fluoroquinolone antibacterials has led to the discovery of a number of compounds with greatly improved potency, spectrum, pharmacokinetics and clinical efficacy. In general their safety and tolerance mirrors these attributes. Like previous naphthyridines, cinnolines, and fluoroquinolones, these agents may cause gastrointestinal, central nervous system, and cutaneous reactions, but these are usually mild and self-limiting, affect 5-10% of patients only and rarely require withdrawal from therapy. Extremely rarely, more serious CNS effects including fits and psychosis may acc
