Stress fractures of the lateral metatarsal bones in metatarsus adductus foot deformity: a previously unrecognized association.

OBJECTIVE: To document a distinctive pattern of stress fractures in the lateral metatarsal bones of patients with metatarsus adductus foot deformity. DESIGN AND PATIENTS: Conventional radiographs and available medical records were reviewed in 11 patients (6 women, 5 men; ages 25-61 years) with stress fractures of the lateral (fourth or fifth) metatarsal bones and metatarsus adductus. Evaluation included the number and location of fracture(s), forefoot adduction angle, and qualitative assessment of bone mineral density. Conditions that might predispose patients to metatarsal fractures, includi

Tumors and pseudotumors of foot and ankle: Bone lesions.

Bone tumors and tumor-like lesions (pseudotumors) are not uncommonly encountered as asymptomatic findings on imaging, or as symptomatic lesions clinically. Radiographic imaging is the first diagnostic tool for their management strategy, since the symptoms are commonly non-specific, such as pain, swelling, and redness. Image findings must be analyzed with attention to the specific features such as lesion location, margination, zone of transition, mineralization, size and multifocality, soft tissue component and/or loco-regional extent. Cross-sectional imaging including CT and MRI serve as compl

[Bone and soft-tissue tumors of the foot and ankle].

BACKGROUND: This study reports the epidemiology of bone and soft-tissue tumors of the foot and ankle, presents therapy strategies, and evaluates mid-term clinicofunctional outcome after surgery for malignant tumors. METHODS: Two hundred four patients with tumors of the foot and ankle were analyzed (163 benign and 41 malignant). Epidemiology and surgical therapy are reported. RESULTS: The most frequent tumors were exostosis, bone cyst, and osteoid osteoma for benign tumors and metastases and chondrosarcoma and Ewing's sarcoma for malignant tumors. In more than 90% of the benign tumors, local

Accessory ossicles and sesamoid bones of the ankle and foot: imaging findings, clinical significance and differential diagnosis.

Accessory ossicles and sesamoid bones are frequent findings in routine radiographs of the ankle and foot. They are commonly considered fortuitous and unrelated to the patient's complaint; however, they may eventually cause painful syndromes or degenerative changes in response to overuse and trauma. They may also suffer or simulate fractures. Our aim was to review, illustrate and discuss the imaging findings of some of the more frequent accessory ossicles and sesamoid bones of the ankle and foot region, with particular emphasis on those that may be of clinical significance or simulate fractures

[Elimination of pathologic torsion of the crural bones during treatment of various severe foot deformities in children].

The author has studied the mechanism of the torsion development of shin bones in normality. The disturbance of the mechanism as an extensive external torsion of shin bones is established in 127 patients with neglected congenital talipes and as an internal twist in critical forms of plano-valgoid deformity of feet in 16 patients. The significance of the two kinds of the torsion pathology of shin bones in the treatment of such feet deformities is substantiated. An original author's method of treatment with an original apparatus is described.

A Case of Mangled Foot With Midfoot Bone Defect.

INTRODUCTION: The decision between amputation and reconstruction for mangled feet is often challenging. The foot must be functional for reconstruction to be considered successful. In this report, we present a case of a mangled foot with a metatarsal bone defect. CASE DESCRIPTION: A man in his 50s presented to the hospital after a heavy load fell on his left foot at work. He was diagnosed with an open dislocation fracture of the Lisfranc joint (Gustilo IIIC), a comminuted fracture of the navicular bone, and a defect of the intermediate cuneiform bone. TREATMENT COURSE: A foot ring-type extern

Aseptic (avascular) bone necrosis in the foot and ankle.

Aseptic necrosis may be defined as a group of diseases that have bone necrosis as a common denominator. They usually appear in the epiphyses and in the carpal and tarsal bones. They generally appear during a growth period and principally at those skeletal points subjected to particular stress. In Müller-Weiss disease in the advanced stages, talonavicular-cuneiform arthrodesis, with or without back foot correction, is the best surgical option. In Freiberg-Kohler disease, treatment can be conservative and we can maintain the head of the metatarsal by performing a joint debridement of the metatarso

Autologous Bone Graft in Foot and Ankle Surgery.

Bone graft is a common adjunct procedure in orthopedic surgery used for fusions, fracture repair, and the reconstruction of skeletal defects in the foot and ankle. Autologous graft, or autograft, involves the transport of bone from a donor site to another location in the same patient. It is considered by many to be the gold standard of bone grafting, as it is provides all biologic factors required for functional graft. Further, autograft is 100% histocompatible with no risk of disease transmission.

Synthetic bone grafting in foot and ankle surgery.

Synthetic bone graft materials have an established role as osteoconductive materials. The basic function is providing a matrix to support the attachment of bone-forming cells for subsequent bone formation, but these materials in various forms can be used for other functions. They can be used as a vehicle for local antibiotic delivery and in injectable form they can be used in a minimally invasive fashion to fill voids and strengthen purchase of screws in osteoporotic bones. They can provide prolonged structural support, which is important for early weight bearing in the lower extremity. These

MR imaging of bone lesions of the ankle and foot.

MR imaging is frequently used to evaluate patients with ankle and foot pain, with or without a relationship to trauma or other known clinical outcomes. Abnormal signal intensity of the bone marrow is the common denominator of all entities described in this article, including bone contusion, fractures, osteonecrosis, transient osteoporosis, and reflex sympathetic dystrophy. Specific MR imaging features in association with the available clinical and radiographic findings can be helpful not only to supply an accurate diagnosis but also to provide valuable information for staging and proper manage

Epidermoid cysts of the foot: with or without bone involvement.

Epidermoid cysts should be considered in the differential diagnosis of soft tissue lesions of the extremities. Although more common in the hand than the foot, they share certain gross characteristics with other pathologies, yet maintain specific inherent traits. Dr. Wu presents one example of a post-traumatic entity, and briefly reviews pertinent literature on this subject.

The diabetic foot. Soft tissue and bone infection.

Diabetic patients, as a consequence of various neurologic, vascular, and metabolic perturbations, are at high risk for developing infections of the soft tissue and bones of the feet. The microbial etiology of soft tissue in infections is best determined by cultures of a tissue curetting or aspiration, rather than a swab. Aerobic gram-positive cocci are the major pathogens in diabetic foot infections; these may be the sole isolate(s) in acute uncomplicated infections, but they are usually accompanied by aerobic gram-negative bacilli or anaerobes in chronic or previously treated infections. Compensatory hypertrophy of bone following surgery on the foot.

Compensatory hypertrophy of bone developed in 5 patients following surgery on the foot. The typical finding of diffuse cortical thickening of the entire ray is thought to represent a static compensatory response which occurs when the usual dynamic response of altered weight-bearing is inadequate or overridden.

The Rotational Positioning of the Bones in the Medial Column of the Foot: A Weightbearing CT Analysis.

BACKGROUND: Malrotation of medial column bones of the foot has been advocated as an important factor in foot conditions such as hallux valgus and progressive collapsing foot deformity. Although stated as a deformity component, variances of normality in the general population are not completely understood. This study intended to describe the rotational profile of all medial column bones using weightbearing computed tomography (WBCT) images in a cohort of patients with different foot and ankle problems. METHODS: In this retrospective study, 110 feet of 95 consecutive patients that received a WB

Development of a Radiographic Union Score for Determining Osteotomy Union Rates in Long Bones of the Foot.

Determining the status of bone healing after osteotomy can be challenging and has implications ranging from clinical decision-making to standardization of research outcomes without the use of computed tomography. To date, no method has been validated for determining osseous healing of an osteotomy site of the long bones of the foot. The purpose of the present study was to develop a radiographic union scoring system that would enhance the diagnostic healing assessment. We adapted existing orthopedic scales that had been validated for healing in the leg for application in the long bones of the f
