INTRODUCTION: The human foot plays an important role in the biomechanical function of the lower limb. Plantar pressure distribution (PPD) can be a useful way to gauge functional knee stability. The objective of the study was to determine whether PPD can be used as an objective criterion for return to sports post anterior cruciate ligament (ACL) reconstruction (ACLR).
METHODS: Over a period of 18 months, 54 symptomatic, MRI-confirmed, isolated ACL tear patients were enrolled in the study. Plantar pressure distribution was documented preoperatively and at 3, 4, 5, and 6 months postoperatively.

CASE: A 55-year-old man presented with a history of forefoot pain and swelling. Radiographs revealed a mass with internal calcifications and osseous erosion of the fifth metatarsophalangeal bone. The mass was isointense to muscle on T1-weighted magnetic resonance imaging (MRI) and hyperintense on T2-weighted MRI. A biopsy was performed, and intraoperatively, the lesion appeared as chalky white material, which under polarized light microscopy was composed of weakly positively birefringent rhomboid crystals, leading to a diagnosis of tophaceous pseudogout.
CONCLUSION: Tophaceus pseudogout shoul

Eccrine poromas are solitary nodules that predilect the soles of the foot. Rarely, malignancies may arise within these tumors. When this occurs, the growth is classified as a malignant eccrine poroma, or porocarcinoma. A case of a 39-year-old woman who originally presented with an incidental plantar skin lesion that was later diagnosed as a porocarcinoma is reported. The patient underwent local excision of the lesion and a sentinel node biopsy. At 18 months postoperative there were no signs of recurrence. This case has been presented to demonstrate that skin lesions with minimal abnormalities

Forefoot pain can have single or multiple etiologies, and frequently pain is attributed solely to a forefoot nerve entrapment. It is well known that forefoot nerve entrapments, such as Morton's, can be falsely assumed to be the cause of forefoot pain when in fact other factors, such as plantar plate disturbances, are the true cause. Frequently, the cause of the patient's forefoot pain starts as a forefoot nerve entrapment, but then, as a result of treatment with a corticosteroid injection, other pathologies manifest, such as plantar plate rupture. The development of high-resolution, high-frequ

The accepted hallmarks of care for plantar forefoot ulceration are meticulous wound care, nutrition, management of infection, and non-weight bearing of the ulcerative area. Tendo-Achilles lengthening is crucial in healing these ulcerations when it is determined that the Achilles tendon is one of the main biomechanical stresses that led to the ulceration. The Silfverskiold test helps determine whether a percutaneous lengthening or gastrocnemius recession is called for. A gastrocnemius recession is the safer operation because it does not carry the postoperative risk of overlengthening or rupture

This paper presents the design and testing of an arched foot with several biomimetic features, including five individual MTP (toe) joints, four individual midfoot joints, and plantar fascia. The creation of a triple-arched foot represents a step further in bio-inspired design compared to other published designs. The arched structure creates flexibility that is similar to human feet with a vertical deflection of up to 12 mm. The individual toe joints enable abduction-adduction in the forefoot and therefore a natural pronation motion. Adult female bone data was obtained and converted into a CAD

BACKGROUND: Forefoot Varus is characterized by inversion of the metatarsal heads relative to the calcaneal bisector. It is present in 83.67 % of cases and contributes to overpronation and related foot/knee/hip pathologies. Despite multiple assessment methods, their reliability remains unclear. This systematic review evaluates the most reliable measurement technique.
METHODS: This systematic review and meta-analysis selected studies from several databases: PubMed, Scopus, Cochrane Library, Web of Science, and PEDro. The search strategies included keywords such as "forefoot", "varus forefoot",

Skin injuries remain a persistent problem for users of lower-limb prostheses despite sustained progress in prosthesis design. One factor limiting the prevention of skin injuries is that skin on the residual limb is not suited to bear the mechanical loads of ambulation. One part of the body that is suited to this task is the sole of the foot. Here, we propose a novel strategy to actively augment skin's tolerance to load, increasing its resistance to mechanically induced injuries. We hypothesise that the load tolerance of skin can be augmented by autologous transplantation of plantar fibroblasts

The gastrocnemius is the main muscle of the posterior compartment of the leg. As a biarticular muscle it has specific biomechanical propertiess. This article discusses these properties combining the major biomechanical topics of anatomy, dynamics, kinetics, and electromyography. This muscle is remarkable in that it has very low energy consumption and very high mechanical efficacy. In addition to the biomechanical features, the consequences of its tightness are discussed. The dysfunction also appears in all the biomechanical topics and clarifies the reasons of the location of symptoms in the mi

This study used a within-subjects correlational design to determine the relationship between forefoot varus angle and standing rearfoot angle among individuals with above-average forefoot varus angles. Clinicians have long theorized that forefoot position is related to the standing rearfoot angle and that both may be related to pronation during gait. More recently, data suggest that the association between static forefoot and rearfoot position may be limited to those individuals with forefoot varus angles larger or smaller than average. However, little data are available on the association bet

BACKGROUND: The aim of this study was to evaluate plantar pressure changes after second metatarsal neck osteotomy using the Weil technique.
METHODS: Six below-knee cadaver specimens were used. Each specimen was held in a custom-built apparatus and loaded to 500 N for a period of 3 seconds. Using a computerized Musgrave pedobarograph, pressure measurements were made before and after osteotomy in both neutral and 45-degree heel rise positions. All osteotomies were made at an angle of approximately 20 degrees relative to the long axis of the metatarsal shaft. The metatarsal heads were displaced

Pain, plantar to the metatarsophalangeal joints, is a common location for the presentation of pain in the forefoot. In the absence of fractures or specific inflammatory conditions common causes of pain in this area include: (1) nonspecific metatarsalgia, (2) plantar fat pad atrophy, and (3) interdigital (intermetatarsal) neuroma. The aggravation and possibly the etiology of these forms of forefoot pain may be related to acute or chronic extrinsic pressure to the forefoot. The role of nonweightbearing, compression of the metatarsal heads, weightbearing, and toe-stance (on the metatarsal heads)

Background: Congenital talipes equinovarus (CTEV) or clubfoot is a common pediatric congenital foot deformity that occurs 1 in 1,000 live births. Clubfoot is characterized by four types of foot deformities: hindfoot equinus; midfoot cavus; forefoot adductus; and hindfoot varus. A structured assessment method for clubfoot is essential for quantifying the initial severity of clubfoot deformity and recording the progress of clubfoot intervention. Aim: This study aims to develop a three-dimensional (3D) assessment method to evaluate the initial severity of the clubfoot and monitor the structural c

Despite cross-slope surfaces being a regular feature of our environment, little is known about segmental adaptations required to maintain both balance and forward locomotion. The purpose of this study was to determine kinematic adaptations of the foot segments in relation to transverse (cross-sloped) walking surfaces. Ten young adult males walked barefoot along an inclinable walkway (level, 0° and cross-slope, 10°). Kinematic adaptations of hindfoot with respect to tibia (HF/TB), forefoot with respect to hindfoot (FF/HF), and hallux with respect to forefoot (HX/FF) in level walking (LW), incli

Understanding the relationship between footwear features and their potential influence on running performance can inform the ongoing innovation of running footwear, aimed at pushing the limits of humans. A notable shoe feature is hollow structures, where an empty space is created in the midsole. Presently, the potential biomechanical effect of the hollow structures on running performance remains unknown. We investigated the role of hollow structures through quantifying the magnitude and timing of foot and footwear work. Sixteen male rearfoot runners participated in an overground running study

OBJECTIVE: To estimate the heritability of 3 common disorders affecting the forefoot, i.e., hallux valgus, lesser toe deformities, and plantar forefoot soft tissue atrophy, in white adult men and women.
METHODS: Between 2002 and 2008, a trained examiner used a validated foot examination to document the presence of hallux valgus, lesser toe deformities, and plantar soft tissue atrophy in 2,446 adults from the Framingham Foot Study. Among these, 1,370 participants with an available pedigree structure were included. Heritability was estimated using pedigree structures by the Sequential Oligogeni

INTRODUCTION: Lawn mower injuries represent a rare cause for severe limb threatening wounds of the lower extremity in children. Most patients can be treated with simple wound treatment involving debridement and VAC therapy,. Major amputation injuries requiring prolonged treatment and follow-up surgeries are relatively rare. In such cases, a multidisciplinary approach is often required. As part of the surgical management, the biomechanics of gait must be considered by the orthopedic team to ensure that a physiological gait pattern is maintained in these typically young patients.
