The evolving landscape of doctor and nurse mobility in the OECD since 2000 with a focus on Canada.

BACKGROUND: OECD health systems face rising demand for health workers, increasingly relying on international recruitment of doctors and nurses. Growing dependence raises "brain drain" concerns, particularly when recruits originate from WHO Health Workforce Support and Safeguards List countries. OBJECTIVE: This study examines how mobility to OECD countries has evolved over two decades, focusing on Canada, and considers implications for destination and origin countries. METHODS: Using harmonised data on foreign-born and foreign-trained doctors and nurses, we analyse stocks and annual inflows a

Role of Social Determinants of Health in Risk Prediction Models for ST-Segment-Elevation Myocardial Infarction Death: A Registry-Based Study.

BACKGROUND: Social determinants of health (SDOH) are increasingly recognized as contributors to disparities in cardiovascular outcomes. However, their incremental value in risk prediction models for ST-segment-elevation myocardial infarction (STEMI) death remains uncertain. METHODS: We analyzed hospital admissions for STEMI between 2018 and 2020 using the American Heart Association's GWTG-CAD (Get With The Guidelines-Coronary Artery Disease) registry. Logistic regression models were constructed: a clinical model including demographic, clinical, and medical history variables, and a full SDOH m

Otolaryngology Simulation Curriculum Development and Evaluation for Medical Education in Rwanda.

OBJECTIVE: This study aimed to assess the feasibility and acceptability of a new low-cost otolaryngology simulation training curriculum for medical students in Rwanda. Given the limited access to hands-on training and equipment in low-middle-income countries, building confidence in performing basic otolaryngology skills is vital for all medical students, especially where all graduates initially serve in primary care before specializing. STUDY DESIGN: Preintervention and postintervention assessments of simulation training. SETTING: Conducted at the University of Global Health Equity in Rwanda

Fighting Prejudice and Absorbing Refugees From Nazism: The National Committee for the Resettlement of Foreign Physicians, 1939-1945.

In the 1930s and 1940s, the medical profession reacted with hostility and erected formidable barriers to refugee physicians from Nazi-dominated Europe who sought to practice medicine in the United States. Yet, refugee physicians ultimately succeeded, with 77% of them working as doctors by 1945 and 98.6% by 1947. Although physician skills are readily transferable, and the United States had a genuine need for doctors after World War II drew 55 000 physicians into the military, refugee physicians' success can be attributed to the courageous physician leaders who lobbied on their behalf and the cr

Medical Student Performance Evaluations for International Medical Graduates: Variability in Content and Standardization.

OBJECTIVE: The Medical Student Performance Evaluation (MSPE) could be a standard objective measure to evaluate international medical graduates (IMGs) in the residency application. The purpose of this study was to evaluate whether international MSPEs are compliant with best-practice guidelines and whether the content allows for meaningful comparisons with other candidates. DESIGN: This was a retrospective cohort study of applications submitted in 2023 from 117 international medical schools to the obstetrics & gynecology (OB/GYN) residency program at a single institution. One representative MSP

Emotional problems of residents in psychiatry.

The authors used a questionnaire technique to determine the magnitude of the problem of emotional illness and poor performance during psychiatric residency, the procedures that are used to screen for or help disturbed residents, and characteristics that differentiate residents who are at risk. The data indicated that residents who have problems that lead to termination are rarely free of emotional disturbance. The general belief that women, foreign medical graduates, and minority group members are at higher risk for problems during residency was not supported; however, younger residents and tr

Performance of Alternative Pathway International Medical Graduates in U.S. Neuroradiology Fellowships: Program Director Perspectives.

RATIONALE AND OBJECTIVES: By 2033, the U.S. may face a shortage of up to 139,000 physicians, including radiologists. Many international medical graduates (IMGs) pursue the American Board of Radiology (ABR) Alternate Pathway, four years of U.S.-based training, research, or faculty experience, to achieve board eligibility. This study evaluated the performance of neuroradiology fellows in the ABR Alternate Pathway compared to U.S. DR residency graduates. MATERIALS AND METHODS: Data were obtained from the ABR and neuroradiology fellowship program directors via a survey distributed in January-Dece

Passing MRCP (UK) PACES: a cross-sectional study examining the performance of doctors by sex and country.

BACKGROUND: There is much discussion about the sex differences that exist in medical education. Research from the United Kingdom (UK) and United States has found female doctors earn less, and are less likely to be senior authors on academic papers, but female doctors are also less likely to be sanctioned, and have been found to perform better academically and clinically. It is also known that international medical graduates tend to perform more poorly academically compared to home-trained graduates in the UK, US, and Canada. It is uncertain whether the magnitude and direction of sex difference

Perceived clinical challenges when treating patients from different cultures: A study among psychiatry trainees in Norway.

The growing number of migrant patients in western countries calls for better cross-cultural competence among health providers. As workplaces, hospitals have become increasingly multicultural, and many doctors are themselves of foreign origin, including psychiatrists. The aims of this study were to explore what clinical challenges International Medical Graduates (IMGs) and native-born Norwegian doctors training in psychiatry perceived when treating patients from other cultures, and what factors might be associated with such cross-cultural challenges. We developed a six-item inventory of perceiv

What do physicians know about normal pressure hydrocephalus and when did they know it? A survey of 284 physicians.

Normal pressure hydrocephalus (NPH) is a relatively new neurologic disorder first described by Salamon Hakim of Bogotá, Colombia, in 1965. NPH is characterized by three symptoms - impaired gait, incontinence and dementia - and an anatomic abnormality, i.e., enlargement of the cerebral ventricles, which can be seen on computerized tomographic or magnetic resonance imaging. Surprisingly, the intracranial pressure is normal. The first author of this article, a Yale Medical School faculty member, developed NPH over the decade from 1992 to 2002, during which it was erroneously diagnosed as cerebral

Medical Student Enrolment in Canadian Universities.

Statistical returns from the 12 Canadian medical schools revealed in 1962, for the fourth consecutive year, a larger first-year enrolment (946, 970, 1006 and 1057). This is attributed to an increase in qualified applicants and expansion in size of the first-year class in the face of a physician shortage. The proportion of women graduates increased from 5.2% in 1958 to 10.1% in 1962 (U.S.A., 5.6%). The academic calibre of entering students showed little change over five years, 1957-61. Recent impressions indicate an upward swing. Loss from withdrawals and dismissals was chiefly in first year (9

Unveiling the Invisible: Challenges Faced by Arab Women International Medical Graduates in U.S. Academia.

PURPOSE: The Arab experience is understudied because until 2024 Arabs were categorized as White on the U.S. Census, leading to diminished documentation of their personal experiences. There is also little understanding of the Arab experience and its intersectionality with gender, being an international medical graduate (IMG), and working in academia. The authors studied the experience of Arab women IMGs working in the U.S. academic system. METHOD: This qualitative study used interpretative phenomenological analysis to analyze in-depth interview data from 20 first-generation U.S. immigrant Arab

The memorable life of Hosmer A. Johnson, early midwestern laryngologist.

Hosmer A. Johnson, one of the early laryngologists in the Midwest, graduated in 1852 from Rush Medical College, and was the first intern in Chicago. With four other faculty members he left Rush in 1859 to establish what subsequently became the Northwestern University Medical School. He taught many subjects and was extremely active in local and national societies. An interest in diseases of the chest led him into laryngology and he became a charter member of the American Laryngological Association, where he reported on congenital tumors of the larynx, laryngeal paralysis, and cancer of the lary

What motivates doctors to leave the UK NHS for a "life in the sun" in New Zealand; and, once there, why don't they stay?

BACKGROUND: At 44%, New Zealand has the highest proportion of international medical graduates (IMGs) in its workforce amongst OECD member countries. Around half of New Zealand's IMGs come from the UK NHS, yet only around 50% stay longer than 1 year post-registration with significant costs to the New Zealand health care system. Why these doctors go to New Zealand and do not stay for long is an important question. METHODS: UK-trained doctors who had gained registration with the Medical Council of New Zealand and currently practising in New Zealand were surveyed (n = 1357) on the motivation for
