Many of the advantages of operative fixation of pelvic and lower extremity fractures have been assumed rather than proved. This review illustrates how for some injuries, particularly for pelvic fractures, hip fractures in the senescent patient, distal femoral fractures, and tibial plateau fractures, operative treatment can increase the fracture disease. Because of its consistently high frequency of serious complications, operative treatment of pelvic fractures should still be considered experimental surgery. Despite impressive developments in diagnostic imaging, the axiom that we treat patient

The term "early infection" should only be used for infections of the soft tissues. Involvement of the major fragments or the medullary cavity indicates either delayed diagnosis or insufficient surgical revision. In addition to already postulated rules of treatment, two further conclusions are being drawn: 1. occasionally it is necessary to apply conservative methods of immobilisation. 2. if careful observation is guaranteed larger fragments may be left in place, as long as they are still in contact with one of the major fragments. At least partial revascularisation is possible.

From a series of 190 consecutive fractures of the shaft of the femur treated by closed Küntscher nailing, 32 of the more severely comminuted have been studied to determine whether treatment by closed nailing was justified. Radiological criteria were used to divide them into 19 who were moderately comminuted and 13 grossly so. Twenty-four patients were less than 21 years of age and 19 patients had 40 significant associated injuries. Fifteen fractures were nailed on the day of injury, and complications were few. Nineteen patients had a supplementary cast-brace and all those who had been employed

STUDY DESIGN: This retrospective study assessed the long-term outcomes of closed treatment (CT) using intermaxillary fixation (IMF) for bilateral subcondylar mandibular fractures. It included 30 patients treated with IMF for 6 weeks (study group) and 29 controls without temporomandibular joint (TMJ) disorders. Clinical assessments included mandibular function, pain, facial symmetry, and occlusion, while radiographic evaluations measured condylar deviation and vertical height reduction (VHR). Fracture severity and patient age at trauma were analyzed as influencing factors.
METHODS: The study g

INTRODUCTION: Open reduction and internal fixation (ORIF) is considered the standard care for displaced tibial tubercle fractures, but closed reduction and internal fixation (CRIF) can also be successful. Our aim was to compare outcomes between ORIF and CRIF for tibial tubercle fractures.
MATERIALS AND METHODS: Children younger than 18 years presenting for a tibial tubercle fracture at a single institution. The main outcomes were operative details (blood loss, tourniquet time, operative time) and complications. Follow-up of at least one month was required.
RESULTS: 98 fractures from 95 patie

Vascular compromised fractures typically result in a high rate of healing complications, such as avascular necrosis, nonunion, delayed union, and arthritis, which severely affect a patient's function and quality of life. The purpose of this review was to identify and describe the epidemiology and available treatment options for the most well-known vascular compromised closed fractures. The injuries discussed in detail in this review were scaphoid, lunate, femoral neck, and talar fractures. Current evidence suggests that optimal treatment for vascular compromised fractures is dependent on the d

Tibial shaft fractures are common injuries in emergency departments (EDs). Although many of these fractures require surgery, nearly all are stabilized in the ED with a long leg splint or bivalved cast. Long leg splinting is often challenging for a single health care provider. Further, even with assistance or previously described techniques for fracture reduction and stabilization, fracture angulation may occur, potentially leading to pain for the patient, fracture displacement, or further soft tissue injury. The authors propose a method for splinting tibial fractures that avoids fracture angul

The aim of this study was to evaluate the efficacy of closed reduction and the effects of the time of intervention on young patients' satisfaction. Patients older than 16 years and those with additional maxillofacial fractures were excluded. All patients were treated by closed reduction and external fixation. The patients and parents were asked about their satisfaction in a survey 6 months' postoperatively. They were divided into groups according to the time of intervention, and the results. Twenty-four patients, age range 4-16 years, with isolated nasal fractures were included in the study. T

PURPOSE: The purpose of the study was to review the literature regarding the evolution of current thoughts on management of mandibular condylar fractures (MCFs) in children.
METHODS: An electronic search in PubMed was undertaken in May 2012. The titles and abstracts from these results were read to identify studies within the selection criteria. Eligibility criteria included studies published in English or German from the last 20 years (from 1992 onwards) reporting clinical series of MCFs in children and adolescents until the age of 18.
RESULTS: The search strategy initially identified 542 st

BACKGROUND: The proximal phalanx (PP) of the fingers is fractured more frequently than the middle or even distal phalanges. The problems of malunion, stiffness and sometimes loss of skin or other soft tissues associated with PP fracture increases the disability. The optimum treatment depends on fracture location, fracture geometry and fracture stability. The objective of the study was to analyse the treatment outcome in a series of closed proximal phalangeal fractures of the hand.
MATERIALS AND METHODS: Eighty-four proximal phalangeal fractures in 68 patients were enrolled from 2007 to 2009.

PURPOSE OF THE STUDY Segmental tibial fractures are commonly believed to be more difficult to manage, requiring surgical treatment. Our experience with forty-eight segmental tibial fractures suggests that these fractures, if closed and with shortening of an acceptable initial degree and a corrected deformity, may be successfully treated with functional braces. MATERIAL AND METHODS Forty-eight closed segmental fractures of the tibia that had initial shortening .12 mm and angular deformity manually corrected to .7 degrees were stabilized in an above-the-knee cast for a median time of 33 days and

We have studied 178 consecutive cases of 203 patients admitted to this unit between 1999 and 2003 with diagnosis of peripheral nerve palsies associated with closed fractures and dislocations in the upper and lower limbs. Fifty-one nerves were explored, 25 of which were found partly or completely transected. Indications for surgery were high-energy transfer injury, severe neuropathic pain, failure to progress and a static Tinel's sign. Spontaneous recovery was good in only 25 of the 51 non-operated cases of high-energy transfer injury to the circumflex, radial and common peroneal nerves. Good s

Fractures of the tibial shaft are the most common long bone fractures. Operative treatment of isolated closed tibial shaft fractures frequently is delayed in favor of treatment of life threatening injuries. A retrospective chart review of 200 tibial fractures was performed. These injuries were managed by two surgeons at a Level 1 trauma center between 1989 and 1996. Strict inclusion criteria identified 54 patients with an isolated closed tibial fracture. Postoperative hospital stay and complication rates were recorded. At a mean followup of 3.6 years, a quality of life questionnaire was admini

PURPOSE: This study examined the changes in the position of the fractured condylar process immediately before and immediately after application of arch bars and at 6 weeks after surgery in a group of patients who underwent closed treatment.
PATIENTS AND METHODS: Sixty-five patients over the age of 16 years underwent closed treatment of unilateral mandibular condylar process fractures. Coronal and sagittal displacement of the condylar process was examined using Towne's and panoramic radiographs before treatment, immediately after placement of arch bars, and at 6 weeks. The change in position o

Twenty-three subacute scaphoid fractures were retrospectively reviewed to determine the efficacy of nonoperative treatment. All of the patients sought medical attention between 4 weeks and 6 months after injury, and their fractures were classified according to location and stability. Nineteen fractures were observed to radiographic union or until closed treatment was abandoned; four patients were lost to followup. Nine of 10 stable subacute middle-third scaphoid fractures healed with cast immobilization in an average of 19 weeks (range, 11 to 38), and these were compared with a randomly select

We treated two children with the unusual complication of ulnar nerve palsy after closed both-bone forearm fractures. Both patients developed an ulnar claw-hand deformity within 7 weeks of injury that resolved spontaneously by 20 weeks postinjury with nonoperative treatment. No patient showed any signs or symptoms of an ischemic compartment syndrome. Both nerve injuries were identified immediately at the time of fracture by a careful neurologic examination. This avoids confusion with a postreduction nerve entrapment injury or ischemic injury after a localized compartment syndrome, which may hav

The authors demonstrate a group of 27 patients subjected to intramedullary osteosynthesis of the forearm by means of Kirschner wires, using closed surgery. They prefer the described method in patients suffering from multiple injuries who have devastated soft parts of the forearm and in unstable fractures in children. The authors draw attention to the fact that they have achieved very good results by the simple surgical method they described.

Twenty-eight patients ranging from 8 to 13 years old were treated at Childrens Hospital of Alabama for diaphyseal femur fractures. We studied two groups: 14 patients (Group I) were treated by conventional 90-90 balanced skeletal traction with late spica cast application; 14 patients (Group II) with multiple associated injuries or hyperkinetic problems were treated with closed retrograde Ender nailing. Groups I and II compared closely with regard to age, sex, fracture type and location, and mechanism of injury. The alignment, leg length, length of hospitalization, and length of therapy were ana
