Treatment of malunited fractures of the anterior tibial spine.

Avulsion fractures of the anterior tibial spine are uncommon injuries, and most often seen in childhood. They can be associated with other intraarticular lesions and have, if adequately treated, a good long-term prognosis. In adults these fractures can be complicated by loss of knee extension because of the displacement of the bony fragment. This is most often as a result of non- or malunion of the displaced fragment. We review two cases of malunited fractures of the anterior tibial spine. Both patients reported persistent knee pain and loss of knee extension, and had had an anterior knee laxi

[Reconstructive surgery of malunited joint fractures of the hand].

Malunion of fractures, joint fractures and pseudoarthroses leads to therapy-resistant pain during function and loading, especially in the wrist, with a risk of early arthrosis. In spite of the extraordinary adaptability of the hand, malunion in the axis and rotation can lead, for example, to cross-finger malfunction. Because of the relatively thin layer of soft tissue, secondary operations to correct the axis, rotation and the length of the phalanx are possible. Each reoperation on the hand diminishes the blood flow, and the anatomical structures change with cicatrices. Thus, the risk of damag

[Surgical correction of malunited fractures of the forearm in children].

AIM: In our goal-oriented society the demands for a forearm with full function are increasing. Functional deficits are not acceptable even if they persist only for a limited period of time. Therefore a rising number of surgical corrections of malunited forearm fractures in the pediatric patient has been performed during the last years. However literature about indication, technique and outcome in these patients is rare. For that reason we report our experience about surgical correction of malunited fractures of the forearm in 14 children. METHODS: Fourteen patients with an average age of 13 y

Malunited Colles' fracture. Analysis of stress distribution.

To try to explain disability in the wrist after malunited Colles' fractures, an experiment was designed to evaluate pressure distribution through the wrist joint with varying degrees of angulation of the distal radius. The results of computer simulation analysis of stress concentration using finite element methods were compared with those of pressure distribution studies on the radial articular surface of cadaveric radii in various positions using pressure-sensitive film. Stress existed in volar regions of the radio-lunate joint under normal pressure in the neutral position, but shifted to dor

Management of malunited fractures of the metacarpal and phalangeal shafts.

Malunions of the tubular bones of the hand should be carefully studied to understand the original deforming forces at the time of injury. Once a three-dimensional concept of the deformity is embraced, a careful plan for osteotomy can be developed and executed. Surgical approach must afford adequate access with the least possible injury to soft tissues. Technique of osteotomy must be tailored to the configuration, location, and biomechanical requirements for proper realignment of the malunited fracture. Important principles in the management of metacarpal and phalangeal malunions are: 1. Rotato

Using a cervical spine cage to reconstruct malunited fibular fractures.

PURPOSE: Anatomical reduction and fixation of unstable ankle fractures is necessary to prevent post-traumatic arthritis. Malunion of the distal fibula in unstable ankle fractures can lead to late degenerative changes of the ankle. Late reconstruction of the ankle can improve its function and postpone the need for ankle fusion or replacement. METHODS: We discuss three patients who presented with fibular malunion. All developed medial gutter opening, syndesmotic widening, and lateral shift and/or talar tilt. Surgery involved an anteromedial approach to clean the medial gutter, an anterolateral

Midcarpal instability caused by malunited fractures of the distal radius.

Thirteen patients with malunited fractures of the distal radius developed symptoms of pain and instability of the midcarpal joint. In six cases, a recurrent voluntary midcarpal subluxation was also present during ulnar deviation. These symptoms were first noticed several weeks, and at times several months, after all immobilization for the treatment of the original fracture had been discontinued. We believe that the loss of the normal palmar tilt of the distal articular surface of the radius prepositions the carpus in a dorsal collapse alignment, which enables this instability to develop. [The malunited distal radius fracture - early or late correction?].

UNLABELLED: BACKGROUND/GOAL: Despite the fact that corrective osteotomy for malunited distal radius fractures is widely accepted, the optimal timing is still a matter of controversy. The presented study compares the clinical and radiological results of early vs. late corrective osteotomy at the distal radius to evaluate the influence of timing. PATIENTS AND METHODS: We prospectively studied 34 consecutive patients with extraarticular malaligned fractures of the distal end of the radius who underwent corrective osteotomy. Early correction was performed in 14 patients (10 -women and 4 men with

Corrective osteotomy of malunited fractures of the distal end of the radius.

Thirty-nine patients with painful impaired forearm rotation and reduced grip strength after malunited fracture of the distal radius were treated with corrective osteotomy of the distal radius, bone grafting and internal fixation and followed for an average of 1.5 years. All patients stated that they were improved by the procedure. At follow-up 36 patients were graded as excellent or good, implying no pain, moderate limitation of motion and grip strength not less than 70% of the uninjured hand. Two patients were graded as fair and one as poor. It is concluded that surgical correction of the def

The use of the Zickel device for a malunited subtrochanteric femur fracture.

Surgical treatment of adult subtrochanteric femur fractures by internal fixation, using the Jewett nail, has been associated with a high degree of major complications. Jewett nail failures in these fractures can produce functionally disabling varus deformities. The Zickel device is an effective means of providing internal fixation following subtrochanteric osteotomy. A case is presented in which the Zickel device was effectively used.

Early correction of malunited supracondylar humerus fractures in young children.

INTRODUCTION: Supracondylar fracture malunion usually results in cubitus varus deformity. The long-standing unsightly deformity causes psychological stress to the child and his/her parents in addition to biomechanical disturbances and functional disorders in the affected elbow. The optimal timing of corrective osteotomy is not well-established in the literature. The present study aimed to report the results of early correction of cubitus varus deformity using a simple technique and to focus on the timing of early correction after supracondylar fracture malunion. PATIENTS AND METHODS: Thirty c

Arthroscopic resection arthroplasty for malunited intra-articular distal radius fractures.

PURPOSE: Cartilage damage of the carpals is a contraindication for corrective osteotomy of the malunited intra-articular distal radius fracture and typically is treated in the symptomatic patient with a salvage procedure. Here, we present our experience and early results with arthroscopic resection arthroplasty of the radiocarpal joint. METHODS: We treated 10 patients (age, 17-68 y; average, 53 y) who had intra-articular malunion of the distal radius with mirror erosion on the carpals. The original fracture occurred 4 to 36 months (average, 9 mo) before our intervention. We performed arthrosc

Multiple Reconstructive Osteotomy Treating Malunited Calcaneal Fractures Without Subtalar Joint Fusion.

OBJECTIVE: Various surgical techniques have been reported in treating calcaneal malunions over the decades, while the operations on single plane were accompanied by respective limitations. The purpose of this study is to evaluate the efficacy of a novel multiple reconstructive osteotomy for treating malunited calcaneal fractures without subtalar joint fusion. METHODS: From March 12, 2010, to August 17, 2017, 10 patients (10 feet) with malunited calcaneal fractures were treated with multiple reconstructive osteotomy with subtalar joint-preserving operations. All patients were treated with a co

Volar Radioscapholunate Arthrodesis and Distal Scaphoidectomy After Malunited Distal Radius Fractures.

PURPOSE: The purpose of this study was to assess range of motion (ROM), pain, and incidence of radiographic degenerative joint disease (DJD) after volar radioscapholunate (RSL) arthrodesis and distal scaphoidectomy (DSE) following malunited distal radius fractures (DRF). METHODS: Fourteen patients with malunited DRF and DJD limited to the radiocarpal joint underwent RSL arthrodesis and DSE between 2006 and 2014. These were retrospectively analyzed both clinically and radiologically. Eleven patients with a mean follow-up of 63 months (range, 30-97 months) were included in the final analysis be

Surgical management of mandibular malposition after malunited condylar fractures in adults.

Fourteen patients who had occlusal disturbance and mandibular malposition following malunited condylar fractures are presented, to demonstrate the variety of surgical corrective procedures used. In order to achieve an anatomically correct mandibular position, a subcondylar osteotomy was performed in 2 patients, and in 11 patients a uni- or bilateral sagittal split osteotomy was carried out. In 1 patient, a Le Fort I osteotomy was the method of choice. In all patients, it was possible to correct the occlusal and mandibular imbalance, as well as to facilitate normal mandibular movement.
