Complications associated with open calcaneus fractures.

BACKGROUND: Literature on open calcaneus fractures is limited and inconsistent. This study's purpose was to report complications such as osteomyelitis, amputations, and soft tissue infections in open calcaneus fractures that were treated at a Level I Trauma Center. METHODS: From January 1995 through December 2007, 1,157 calcaneus fractures were identified with 127 fractures being open (11.0%). Average followup time was 9.1 (range, 2 to 53) months. All open fractures were treated by a similar protocol of intravenous (i.v.) antibiotics, emergent irrigation and debridement (I&D), initial fractur

Open calcaneal fractures: assessment and management.

Open calcaneal fractures are obscure fractures. Typically, these injuries result in a high-energy, polytraumatized patient population. Initial management consists of irrigation and debridement. Secondary forms of management continue to evolve. High-energy fracture subtypes and severe soft tissue injuries portend an unsatisfactory outcome, despite appropriate management. Standards for the type and timing of treatment have not been established.

Open fractures of the distal radius.

A review of 18 patients treated at our institution for open distal radius fractures was performed. The patients were followed up for an average of 32 months (range, 12 months to 5.3 years). Wounds were classified according to Gustilo and Anderson with 9 type I (50%), 3 type II (17%), and 6 type III (34%) (3 IIIa, 2 IIIb, 1 IIIc) injuries. Seven patients underwent a total of 2 to 4 procedures to treat their injuries, and 3 patients required 5 or more procedures. Eleven (61%) patients had postoperative complications. Patients were evaluated with the Disabilities of the Arm, Shoulder, and Hand qu

Open treatment of acute calcaneal fractures.

Displaced intraarticular fractures of the calcaneus require operative intervention to restore the anatomy of the bone, which in turn is the requirement for recovery of subtalar joint mobility. Surgery through a lateral incision, without opening the sheath of the peroneal tendons, and the use of lag screws and a "Y-plate," must restore not only the respective positions of the "three poles" of the calcaneus but also the respective orientations of the articular surfaces. Stable internal fixation allows early active and passive mobilization of the subtalar and ankle joints. If this is not achieved

[Open fractures of the elbow joint].

Both in open and in closed elbow fractures, anatomical joint reconstruction and rigid internal fixation, which permit early initiation of motion, are essential for good joint function. However, in open distal intraarticular humerus fractures this is more difficult to achieve, owing to the high rate of comminuted types C 2 and C 3 fractures. In addition, the extensive soft tissue lesions often mean that physiotherapy cannot be started until later than would otherwise be desirable. This retrospective study shows that the functional results in 60 patients operated on for correction of open distal

Compartment syndrome in open tibial fractures.

A retrospective review of the cases of 180 patients who had 198 acute open fractures of the tibial shaft and were admitted to a multiple-trauma referral center over a three-year period revealed an incidence of accompanying compartment syndrome of 9.1 per cent (eighteen fractures in sixteen patients). Each of the eighteen compartment syndromes was documented by measurements of intracompartmental pressure that were obtained by the saline-injection technique, and all were treated by four-compartment fasciotomy. The incidence of compartment syndrome was found to be directly proportional to the deg

[Open diaphyseal tibial fractures in children].

In a collective study of 22 surgical clinics 138 open diaphyseal tibial fractures in children from 0 to 14 years were examined. The incidence of open fractures, in relation to all tibial shaft fractures comes up to 6.9%, one third (2.3%) are second degree and third degree open fractures. 80% of these fractures are caused by traffic accidents. In 33% there exist multiple fragments. In first degree open fractures conservative treatment generally is practiced, while in second and third degree open fractures osteosynthesis often is performed.

Management of open fractures: A narrative review.

Open fractures are an emergency where the principal aim of the treatment is to maximise the restoration of limb function while preventing the dreaded consequences of infection and non-union. The decision-making process for open injuries is influenced by a variety of criteria, such as patient age, injury features, systemic response, activity level, comorbidities, and functional requirements. A collaborative orthoplastic approach to treating these injuries is essential for minimizing complications and need to be considered as a single specialty in early and long-term management. It has been show

Open lower limb fractures in the elderly.

OBJECTIVE: The aim of this study was to assess the outcomes of open lower limb fractures in patients aged 65 and over. The primary outcome was mortality at 30 days, and the secondary outcome was reoperation. METHODS: This study retrospectively identified patients aged 65 and over, presenting with an open tibia or ankle fracture, over a two-year period. Patient demographics, injury characteristics and surgical interventions were documented. The 30-day and one-year mortality was recorded, as well as any complications encountered. RESULTS: There were 88 patients included in the study, with a me

The Management of Pediatric Open Forearm Fractures.

Open pediatric forearm fractures are common injuries that present to emergency departments across the United States. A total of 32% to 80% of all open pediatric fractures involve the forearm. Standard treatment for these injuries includes prompt intravenous antibiotic administration, tetanus prophylaxis, and usually bedside irrigation as a temporizing measure. Gustilo and Anderson type 2 and 3 open pediatric forearm fractures are generally managed with formal irrigation and debridement and fracture stabilization in the operating room. Management of Gustilo and Anderson type 1 open pediatric fo

Are open mandibular fractures still an emergency?

INTRODUCTION: Early surgical management is often advocated for fractures of the tooth-bearing portion of the mandible. A 6-hour delay has been mentioned for the fixation of these fractures. Our aim was to bring this paradigm into question. METHODS: All patients referred to our department from September 2012 to May 2014 for fractures of the tooth-bearing portion of the mandible were retrospectively included. For each patient, age, gender, aetiology of the fracture, and characteristics of the fractures were recorded. Tobacco and/or alcohol addictions, diabetes and mandibular dental condition we

Open fractures of the foot and ankle.

Open fractures of the lower extremity cover a wide gamut of injuries ranging from the mangled, pulseless leg necessitating amputation to the more innocuous pinhole open wounds associated with simple fracture patterns. Prompt diagnosis and appropriate care can make a dramatic difference in decreasing complication rates and improving ultimate outcomes. Principles of management of open fractures have been created with the main goal of decreasing infection rates, while providing for stabilization of the bone and soft tissue injury.

Evidence-based treatment of open ankle fractures.

Fractures of the ankle are fairly common injuries. Open ankle fractures are much less common and associated with severe injuries to surrounding tissues. We have performed a systematic review of the literature concerning the clinical results and complication rates in the treatment of open ankle fractures. We conducted a search limited to the following databases: Pubmed/Medline, Cochrane Database of Systematic Reviews, Cochrane Clinical Trial Register and Embase. These were searched from 1968 to April 2010 to identify studies relating to the treatment of open ankle fractures. Fifteen articles co
