Relation between functional mobility and dynapenia in institutionalized frail elderly.

OBJECTIVE: To investigate the relation between functional mobility and dynapenia in institutionalized frail elderly. METHODS: A descriptive, correlational study involving 26 institutionalized elderly men and women, mean age 82.3±6 years. The instruments employed were the Mini Mental State Examination, the Geriatric Depression Scale, the International Physical Activity Questionnaire, the Timed Up and Go test, a handgrip dynamometer and a portable dynamometer for large muscle groups (shoulder, elbow and hip flexors, knee extensors and ankle dorsiflexors). RESULTS: Significant negative correlat

Missing links in oral health care for frail elderly people.

A national interdisciplinary strategy is needed to address the comprehensive oral health care needs of frail elderly people residing in long-term care facilities. Reasonable care within the social and personal context of frailty encompasses active prevention of disease augmented by necessary restorative treatment, provided with sensitivity to a person"s propensity to seek care. Typically, dental emergencies are managed quite well in longterm care facilities, either by treating the resident on site or by transporting the resident to a local clinician. In addition, facility administrators are us

Comorbidity and social factors predicted hospitalization in frail elderly patients.

OBJECTIVE: Studies on factors predicting the hospital admission of geriatric patients have reported different findings. The present study was undertaken to examine the rate of hospitalization among a large sample of frail elderly people living in the community and to identify the most important clinical and patient-centered factors associated with the hospital admission. STUDY DESIGN AND SETTING: This is an observational cohort study. All patients (n = 1,291) in six Italian home health care agencies were assessed by a trained staff who collected data on the Minimum Data Set for Home Care (MDS

Recovery of surgery in the elderly (ROSE) program: The efficacy of a multi-modal prehabilitation program implemented in frail and pre-frail elderly undergoing major abdominal surgery.

BACKGROUND: Major abdominal surgery is associated with a high rate of post-operative complications with increased risk of adverse surgical outcomes due to the presence of frailty. This study aims to evaluate the effectiveness of the multimodal Recovery of Surgery in the Elderly (ROSE) prehabilitation program with supervised exercise in mitigating postoperative functional decline when compared to standard care. METHOD: The ROSE program enrolled ambulant patients who were 65 years and above, had a Clinical Frailty Scale score of 4 or more and were planned for major abdominal surgery. Participat

Anticoagulant Regimen for Nonvalvular Atrial Fibrillation in the Elderly and Frail Population.

Anticoagulant therapy is the cornerstone of stroke prevention in atrial fibrillation (AF), yet its management in elderly and frail patients remains particularly challenging. This review aims to summarize current evidence, guideline recommendations, and future perspectives regarding the optimal anticoagulant strategy in this vulnerable population. A narrative review of pivotal randomized clinical trials and major real-world registries was conducted, focusing on outcomes of direct oral anticoagulants (DOACs) versus vitamin K antagonists (VKAs) in patients aged ≥75 years or meeting frailty criter

Platypnea-Orthodeoxia Syndrome in a Frail Elderly Patient: It's Not Always Pneumonia.

Platypnea-orthodeoxia syndrome (POS) is a rare condition characterized by positional dyspnea and arterial desaturation that worsens in the upright position and improves when supine. It is most commonly associated with intracardiac right-to-left shunting, often through a patent foramen ovale (PFO), and is frequently under-recognized. We report the case of an 84-year-old frail woman admitted with acute confusion and severe hypoxemia, initially treated for presumed community-acquired pneumonia. Despite high oxygen requirements, inflammatory markers were minimally raised, and imaging did not suppo

The 2023 WSES guidelines on the management of trauma in elderly and frail patients.

BACKGROUND: The trauma mortality rate is higher in the elderly compared with younger patients. Ageing is associated with physiological changes in multiple systems and correlated with frailty. Frailty is a risk factor for mortality in elderly trauma patients. We aim to provide evidence-based guidelines for the management of geriatric trauma patients to improve it and reduce futile procedures. METHODS: Six working groups of expert acute care and trauma surgeons reviewed extensively the literature according to the topic and the PICO question assigned. Statements and recommendations were assessed

Management of autoimmune factor XIII deficiency in a frail, elderly patient.

Autoimmune factor XIII/13 deficiency (aFXIII deficiency) is a rare hemorrhagic disorder, for which typical guideline-directed treatment is aggressive immunosuppressive therapy. Approximately 20% of patients are over 80 years old; however, and optimum management of such patients has not reached consensus. Our elderly patient had massive intramuscular hematoma, and aFXIII deficiency was diagnosed. The patient opted against aggressive immunosuppressive therapy, so he was managed with conservative treatment only. Thorough survey of other correctable causes of bleeding and anemia is also required i

Pain in the Frail or Elderly Patient: Does Tapentadol Have a Role?

Persistent pain affects the elderly disproportionally, occurring in 50% of elderly community-dwelling patients and 80% of aged care residents. The management of pain in the elderly and frail patient is complicated because of the risks posed by changes in pharmacokinetics and pharmacodynamics, polypharmacy, and drug-disease interactions. Trials evaluating the efficacy of analgesics have often excluded elderly patients and universally excluded frail patients; therefore, the true efficacy and side-effect profiles in these population groups are largely unknown, especially for long-term use. A step

Refeeding syndrome in the frail elderly population: prevention, diagnosis and management.

Aging is linked to physiological and pathophysiological changes. In this context, elderly patients often are frail, which strongly correlates with negative health outcomes and disability. Elderly patients are often malnourished, which again is an independent risk factor for both frailty and adverse clinical outcomes. Malnutrition and resulting frailty can be prevented by adequate nutritional interventions. Yet, use of nutritional therapy can also have negative consequences, including a potentially life-threatening metabolic alteration called refeeding syndrome (RFS) in high-risk patients. RFS

Weekly carboplatin in a frail elderly woman with advanced peritoneal carcinoma.

INTRODUCTION: Standard chemotherapy for ovarian, peritoneal and fallopian tube carcinoma has been a combination of carboplatin (AUC = 6) and paclitaxel 175 mg/m2 every 3 weeks. For frail and elderly patients who may not tolerate the toxicity associated with combination chemotherapy there is the option of omitting paclitaxel and giving carboplatin (AUC = 6) as a single agent every 3 weeks. The toxicity may be reduced further, without decreasing effectiveness, by giving single-agent carboplatin at a reduced dose (AUC = 2) every week. CASE REPORT: A frail 79-year-old woman with advanced peritone

Abuse and neglect of the frail elderly at home: an exploration of theoretical relationships.

Using an ex post facto, correlational descriptive design, a purposive sample of 74 elderly individuals who were identified as either having a 'good relationship' or an 'abusive/neglectful relationship' with their related caregivers were interviewed in their own homes. The purpose of the study was to test two hypotheses which predicted the nature of differences between the two groups and four which predicted the magnitude and direction of theoretically deduced causal relationships. The data showed that demographic characteristics and certain theoretical variable such as anger, hostility and str

Coned hemipelvis reconstruction for osteoporotic acetabular fractures in frail elderly patients.

AIMS: Complex displaced osteoporotic acetabular fractures in the elderly are associated with high levels of morbidity and mortality. Surgical options include either open reduction and internal fixation alone, or combined with total hip arthroplasty (THA). There remains a cohort of severely comorbid patients who are deemed unfit for extensive surgical reconstruction and are treated conservatively. We describe the results of a coned hemipelvis reconstruction and THA inserted via a posterior approach to the hip as the primary treatment for this severely high-risk cohort. METHODS: We have prospec

Hospitalized frail elderly patients - atrial fibrillation, anticoagulation and 12 months' outcomes.

BACKGROUND AND OBJECTIVE: Multiple chronic conditions and recurring acute illness are frequent among elderly people. One such condition is atrial fibrillation (AF), which increases the risk of stroke up to fivefold. The aim of this study was to investigate the prevalence of AF among hospitalized frail elderly patients, their use of anticoagulation and their 12-month outcomes. PATIENTS AND METHODS: This was a clinical observational study of acutely hospitalized frail patients over the age of 75 years. The CHA2DS2-VASc Score was used to evaluate ischemic stroke risk in patients with AF. Clinica

[Institutional care for frail elderly from Weimar to Bonn (1924-1961)].

The article deals with the development of institutionalized care and its overall discourse concerning the elderly sick people in Germany from 1924 to 1961. This period of time embraces an ambivalent process of modernization that falls short of any unilinear success story. Neither politics nor the medical sciences had the impact to make the nursing homes catch up with the advanced hospitals. They became low-grade institutions within the national welfare system. These homes evolved from poor-law houses with no specialized care whatsoever. The chronically ill and infirm old people emerged during

Emphysematous Cystitis With Extraperitoneal Bladder Perforation in a Frail Elderly Patient on Long-Term Corticosteroid Therapy Without Diabetes: A Hidden Geriatric Threat in a Frail Elderly Patient.

Emphysematous cystitis is a rare but serious urinary tract infection characterized by gas formation within and around the bladder wall, affecting elderly immunocompromised patients. The case report describes an 82-year-old frail gentleman admitted to a geriatric ward with nonspecific symptoms, including malaise, later diagnosed with emphysematous cystitis complicated by bladder perforation. The case is unusual due to the absence of classic urinary symptoms, chronically elevated C-reactive protein (CRP) with emphysematous cystitis presenting insidiously in a frail elderly patient on long-term h

Non-invasive ventilation for frail elderly patients with acute respiratory failure.

We describe 3 elderly patients with acute respiratory failure who received respiratory assistance with nasal bi-level positive airway pressure (BiPAP) on the ward. In these patients with poor prognostic factors, non-invasive positive pressure ventilation was preferred as a reasonable alternative to invasive ventilation; indeed, their admission to the intensive care unit for mechanical support was considered inappropriate. Despite the small number of patients and limited experience with the technique, BiPAP is discussed here as a possible treatment for severe respiratory failure when endotrache
