[Cerebellar ataxic gait].

In this review, we have mainly discussed the cerebellar ataxic gait. The cerebellum can be divided into 3 phylogenically different lobes: the archicerebellum, paleocerebellum, and neocerebellum. The main components of the cerebellar circuit are 2 types of neurons, i.e., the Purkinje cells and granule cells and 3 types of fibers, i.e., mossy fibers, climbing fibers (cerebellar afferent fibers), and parallel fibers (axons of granule cells) Theoretically, cerebellar ataxia is considered to be caused by any lesions that develop within this circuit. Before diagnosing any symptoms as ataxia, we shou

The distribution of real-world gait speed and its relation to gait bout length in people with and without gait impairment.

Real-world walking has been suggested to consist of two distinct constructs: slower walking ensembled in short walking bouts, and faster walking during longer walking bouts. Here, we evaluated the presence of a bimodal gait speed distribution reflecting these constructs in people with and without gait impairment. Remote walking data were analyzed from four cohorts: healthy adults (n = 16), chronic stroke (n = 20), end-stage knee osteoarthritis (n = 21), and incomplete spinal cord injury (n = 16). Participants wore a sensor on each foot and one on the lower back for three days. Gaussian mixture

Changes in Gait and Texting Ability During Progressively Difficult Gait Tasks.

To investigate the effects of a cell phone texting task on an individual's ability to perform three ambulation-based tasks, each with different and progressively more difficult demands. 36 participants (24 male/12 female, average age 23.4) performed: a Timed Up & Go (TUG), stair ambulation (STAIR), and tandem gait (TAN). Participants completed each gait-based task under four conditions: as a practice, while holding their cellular device (baseline), while texting a message, and while reading a message. Statistically significant differences were found within the following variables: 1) mean time

Understanding Gait, Its Neurophysiologic Control and Systematic Localisation of Gait Abnormalities.

Gait analysis is a crucial diagnostic tool in neurology and rehabilitation, reflecting the intricate coordination between the central and peripheral nervous systems, musculoskeletal structures, and sensory feedback mechanisms. This article integrates the neurophysiological basis of gait, emphasising the role of central pattern generators in the spinal cord and the mesencephalic locomotor region in regulating initiation, speed, and adaptability (1). Gait disorders result from disruptions in various neural structures, including the cerebellum, basal ganglia, motor and sensory pathways, and the p

Gait Variability to Phenotype Common Orthopedic Gait Impairments Using Wearable Sensors.

Mobility impairments are a common symptom of age-related degenerative diseases. Gait features can discriminate those with mobility disorders from healthy individuals, yet phenotyping specific pathologies remains challenging. This study aims to identify if gait parameters derived from two foot-mounted inertial measurement units (IMU) during the 6 min walk test (6MWT) can phenotype mobility impairment from different pathologies (Lumbar spinal stenosis (LSS)-neurogenic diseases, and knee osteoarthritis (KOA)-structural joint disease). Bilateral foot-mounted IMU data during the 6MWT were collected

[Gait disorders in Parkinson disease. Gait freezing and falls: therapeutic management].

GAIT ARRESTS: They affect the evolution of the disease. This freezing phenomenon which induces falls sometimes constitutes an initial sign. Like the gait initiation failure, freezing can be controlled by sensory stimulation, notably visual inputs, but also by more sustained attention. FALLS ARE MAINLY CONNECTED WITH BOTH POSTURAL INSTABILITY AND RIGIDITY: They are poorly influenced by dopaminergic therapies. The progressive decrease of step width represents a main factor in their occurrence. PRECOCITY OF GAIT DISORDERS IS UNUSUAL IN PARKINSON'S DISEASE: Other parkinsonian syndromes such as pro

Factors Influencing Gait Performance: Comfortable Linear Gait and Complex Gait in School-Aged Children in a Dual-Task Model.

The aim of this study was to determine the effect of cognitive interference by using the Dual-Task (DT) paradigm on gait parameters according to sex, and age. Additionally, we aim to explore the relationship between Dual-Task-Cost (DTC), physical fitness, cognitive functioning, and weight status in schoolchildren. One hundred schoolchildren participated in this study (age = 8.83 ± 1.82 years). They were randomly assigned to Comfortable Linear Gait (CLG: gait in a straight path) or Complex Gait (CG: gait over obstacles) with and without interference. For CLG, boys and girls showed a reduction i

Reduced physical activity in multiple sclerosis: The role of gait parameters, gait variability parameters and self-reported measure of gait.

BACKGROUND: It is important to understand the gait disorders that lead to physical inactivity in patients with multiple sclerosis (PwMS). The objective of this study was to evaluate the relationship between physical activity (PA) and gait in PwMS in a holistic manner, employing objective, subjective and patient perspective methods. METHODS: The study cohort comprised 30 PwMS. PA was evaluated using a SenseWear Armband and the International Physical Activity Questionnaire. The gait was evaluated using the 10 Meter Walk Test, the Timed Up and Go Test, the GAITRite Analysis System, and the 12-it

Distinct roles of the human cuneiform and pedunculopontine nuclei in gait and freezing of gait.

Freezing of gait (FOG) in Parkinson's disease (PD) is a major cause of disability, often resistant to dopaminergic therapy and deep brain stimulation (DBS). Its underlying neural mechanisms remain unclear. The mesencephalic locomotor region (MLR) is a critical hub for gait control and a therapeutic target for DBS, but the respective functional roles of its two main nuclei, i.e. the cuneiform nucleus (CuN) and the pedunculopontine nucleus (PPN), remain unclear in human gait. Here, we combined local field potential recordings from both the CuN and PPN with detailed biomechanical analyses of gait

Effect of Discrepant Gait Assistance on the Motion and Stability of Unplanned Gait Termination.

For the development and widespread adoption of wearable robots, ensuring the safety of the wearers is critical. The gait instability risks specific to wearable gait-assist robots arise from the motion interference of the robot. Such motion interference occurs due to sudden changes in the wearer's gait. This study focuses on unplanned gait termination, an abrupt stopping motion during walking. The primary objective of this study was to clarify the effects of discrepant assistance provided continuously during gait termination, in contrast to the wearers' intention. In the experiments, unplanned

Estimability study on the age of toddlers' gait development based on gait parameters.

In the first few years of toddlers' locomotion, various gait parameters improve gradually and dynamically with gait development. Therefore, in this study, we hypothesized that the age of gait development, or the level of gait development with age as its indicator, can be estimated from several gait parameters related to gait development, and investigated its estimability. In total, 97 healthy toddlers aged about 1-3 years participated in the study. All five selected gait parameters showed a moderate or higher correlation with age, but the duration with a large change and the strength of the as

The sequence effect and gait festination in Parkinson disease: contributors to freezing of gait?

Festination and freezing of gait (FOG) are poorly understood gait disorders that cause disability and falls in people with Parkinson disease (PD). In PD, basal ganglia malfunction leads to motor set deficits (hypokinesia), while altered motor cue production leads to a sequence effect, whereby movements becomes progressively smaller as in festination. We suggest both factors may contribute to FOG. Disturbance of set maintenance by the basal ganglia in PD has previously been examined in gait, but limited systematic evaluation of the sequence effect exists. In this study, we investigated the step

Alprazolam Reduces Freezing of Gait (FOG) and Improves FOG-Related Gait Deficiencies.

BACKGROUND: Freezing of gait (FOG) is an intractable motor symptom in Parkinson's disease (PD) that increases fall risk and impairs the quality of life. FOG has been associated with anxiety, with experimental support for the notion that anxiety itself provokes FOG. We investigated the effect of acute anxiety reduction via alprazolam on FOG in PD. METHODS: In ten patients with PD, FOG, and normal cognition, we administered 0.25 mg alprazolam in one session and placebo in another, in counterbalanced order. At each session, on separate days, patients walked on a pressure-sensitive walkway. Using

A Proposal for Complex Gait Evaluation Using Dual-Task Gait Termination Time.

CONTEXT: Gait termination time (GTT) has been used to predict falls in older adults but has not been explored in the sport rehabilitation setting. The incorporation of a concurrent cognitive task as a complex measure of gait in this clinical population could lead to better health-related outcomes. OBJECTIVE: To compare the effect of planned and unplanned gait termination with and without a concurrent cognitive task on reaction time (RT), gait velocity, and GTT. DESIGN: Cross-sectional. SETTING: Laboratory. PARTICIPANTS: Twenty young adults (females 60.0%, age 20.1 [0.9] y, height 169.5 [8.

Gait-Synchronized Rhythmic Brain Stimulation Improves Poststroke Gait Disturbance: A Pilot Study.

Background and Purpose- Gait disturbance is one of serious impairments lowering activity of daily life in poststroke patients. The patients often show reduced hip and knee joint flexion and ankle dorsiflexion of the lower limbs during the swing phase of gait, which is partly controlled by the primary motor cortex (M1). In the present study, we investigated whether gait-synchronized rhythmic brain stimulation targeting swing phase-related M1 activity can improve gait function in poststroke patients. Methods- Eleven poststroke patients in the chronic phase participated in this single-blind cross

Knee hyperextension gait abnormalities in unstable knees. Recognition and preoperative gait retraining.

Five patients with symptomatic knee hyperextension thrusting patterns due to posterolateral ligament complex injury underwent gait analysis before and after a gait retraining program. Patients were trained to avoid knee hyperextension by 1) walking with their knees slightly flexed throughout stance, 2) maintaining ankle dorsiflexion in early stance, and 3) maintaining an erect trunk-hip attitude during stance. Kinematic and kinetic measurements were obtained using automated gait analysis. Four of the five patients significantly reduced hyperextension at the knee and abnormal motion patterns at
