Superior mesenteric artery syndrome or Wilkie syndrome is due to the compression of the third duodenum between the superior mesenteric artery and the aorta. It causes acute or chronic upper bowel occlusion. Abdominal CT scan facilitates the diagnosis. Severe malnutrition is its main etiological factor. Medical treatment can be based on aspiration of gastric contents and parenteral nutrition. If this fails, surgery is necessary. We here report the case of a 46-year-old patient, with a history of smoking, presenting with profuse postprandial bile and food vomiting. He had had weight loss of 7% o

Gastric artery aneurysm is a rare and lethal condition, and is caused by inflammatory or degenerative vasculopathies. We describe herein the clinical course of a patient with a ruptured gastric artery aneurysm associated with microscopic polyangiitis. Absence of vasculitic changes in the aneurysm resected and negative results of autoantibodies interfered with our diagnostic process. We should have adopted an interventional radiology and initiated steroid therapy promptly to rescue the patient.

Pseudoaneurysms due to chronic pancreatitis can be a source of major gastrointestinal (GI) hemorrhage. Computed tomography (CT) is the primary diagnostic imaging modality for pancreatic pseudocysts associated with GI bleeding. Pseudoaneurysms and associated GI bleeding can be diagnosed and embolized with transcatheter techniques once the arterial anatomy is defined. CT is a useful modality for follow-up examination of the pseudocyst; the findings must be correlated with other procedures performed on these patients. On follow-up studies, contrast medium retained in the pseudocyst after emboliza

Pancreatoduodenectomy (PD) after esophagectomy with gastric conduit reconstruction is technically challenging. Preserving the blood supply of the gastric conduit is crucial in performing PD after esophagectomy. We report the case of a 66-year-old man who underwent gastroduodenal artery-preserving PD after esophagectomy with gastric conduit reconstruction for intraductal papillary mucinous neoplasm. The patient developed pseudoaneurysm rupture postoperatively, but was successfully treated with interventional radiology. Precise assessment is important in developing a surgical strategy depending

OBJECTIVE: To investigate the imaging findings and hepatic artery variations encountered in patients with aberrant left gastric vein (ALGV).
METHODS: A retrospective database search between January 2014 and November 2018 was carried for ALGV. The course and types (1-3) of ALGV, the presence of associated liver lesions, and coexistence of hepatic artery variations were reviewed on CT images.
RESULTS: A total of 32 patients (22 men, 68.7%) with a mean age of 52.5 years (range 22-76 years) were found to have ALGV. The prevalence of ALGV was 0.073%. The most frequent type of ALGV was type 1 (n =

The use of the stomach as an esophageal substitute after esophagectomy is the most commonly accepted as the standard. The colon and supercharged pedicled jejunum are acceptable options for esophageal reconstruction when the stomach is unavailable. We describe a case of esophageal cancer with a history of right hemicolectomy scheduled McKeown esophagogastrectomy. During celiac detection, the gastroepiploic artery had been resected, and the jejunum had a relatively short mesentery. In such a situation, we used an unconventional gastric conduit with the right gastric artery as the sole blood supp…

Epigastric pain is a common symptom in the emergency department. Most epigastric pain is not fatal, except in acute myocardial infarction, aortic dissection, and abdominal aneurysm rupture. Epigastric pain can also be the only symptom of isolated spontaneous gastric artery dissection. We report the case of a 51-year-old woman diagnosed with neurofibromatosis who initially presented with only epigastric pain. She was initially misdiagnosed with gastritis, treated conservatively, and discharged. Two hours later, she returned to the emergency department with persistent epigastric pain and rebound…

A 78-year-old man presented with abdominal pain, high International Normalised Ratio and rapidly falling haemoglobin. He had a background of Marfan syndrome and was on warfarin following mechanical aortic valve replacement. Abdominal CT imaging showed haemoperitoneum with a leaking aneurysm of the left gastric artery measuring 10 mm in diameter. The decision whether to reverse his anticoagulation provided a difficult clinical scenario given the risk of thrombosis associated with the mechanical aortic valve. The patient went on to have a successful percutaneous embolisation of the aneurysm usin…

We report the case of a 61-year-old man with pancreatectomy with arterial resection and reconstruction and extended lymph node dissection because of locally advanced pancreatic cancer. Surgery revealed the tumor invading the root part of the superior mesenteric artery, so we cut off and reconstructed the artery root through end-to-end anastomosis of the left gastric artery and superior mesenteric artery. The left gastric artery is a reasonable choice to reconstruct the superior mesenteric artery, and to the best of our knowledge, only a few reports describe locally advanced pancreatic cancer a…

BACKGROUND: Variations in hepatic arterial anatomy are clinically significant in upper abdominal surgery. A left hepatic artery (LHA) arising from the left gastric artery (LGA) is a recognized variant occurring in 6-21% of cases, but a large-caliber, dominant LHA is rare and has important implications during gastrectomy.
CASE PRESENTATION: A 58-year-old man of Azerbaijani ethnicity with gastric cancer, after four cycles of chemotherapy, underwent gastrectomy with D2 + lymph node dissection. During mobilization of the gastrohepatic ligament, a large aberrant LHA was discovered intraoperatively…

BACKGROUND: A complete replacement left hepatic artery (LHA) solely originating from the left gastric artery (LGA), with no supply from the hepatic artery proper, is exceptionally rare. This variant places entire left lobe perfusion on the LGA. Literature review confirms no prior reports of such an isolated LHA replacement pattern in surgical/radiological publications. Unrecognized, this anatomy carries significant intraoperative injury risk during hepatobiliary/ upper gastrointestinal surgery.

We experienced a rare case of the gastric cancer occurred after the coronary artery bypass grafting (CABG) using the right gastroepiploic artery (RGEA). On April, 1990, a 74-year-old woman underwent CABG using RGEA for angina pectoris due to 99% stenosis of the right coronary artery. Anorexia and vomiting appeared from December 1992. Examination of the stomach by an oral procedure led to a diagnosis of the pyloric stenosis due to the gastric cancer. The coronary arterial lesion was progressive, and 90% stenosis of LAD (#6, #7) was noted. On February 1993, the re-CABG and the gastrectomy were p

PURPOSE: The left gastric artery (LGA) is commonly severed when the gastric tube is made for esophageal reconstruction. Sacrifice of the LGA can cause liver ischemic necrosis in patients with an aberrant left hepatic artery (ALHA) arising from the LGA. We experienced a case of life-threatening hepatic abscess after severing the ALHA. Therefore, the purpose of this study is to evaluate clinical outcomes of severing the ALHA.
METHODS: We retrospectively enrolled 176 consecutive patients who underwent esophagectomy with gastric tube reconstruction. They were classified into the ALHA (N = 16, 9.1…

Because the stomach in situ has few distinctive surface features and changes shape dramatically with food intake, we have used micro-CT imaging combined with two distinct contrast agents to (1) characterize the pattern of arteries, potential landmarks, on the stomach wall and (2) evaluate how meal-related shape changes affect the size of the different regions. Images generated with a contrast agent injected directly into the heart during perfusion enabled a thorough look at the organizational features of the stomach angioarchitecture. The stomach receives its blood supply primarily from two pa

Nontraumatic abdominal apoplexy, which is the occurrence of hemorrhage into the peritoneal cavity, is uncommon in the pediatric literature. Adult case reports of nontraumatic abdominal apoplexy include visceral artery aneurysms from the aorta, splenic, celiac, hepatic, gastric, and gastroepiploic arteries as well as gynecologic pathology. We identified 2 case reports of spontaneous abdominal apoplexy due to injury of the short gastric arteries in adult patients with histories of vomiting after alcohol consumption. This is the first reported case of nontraumatic abdominal apoplexy due to injury

This study was undertaken to determine the subtype of prejunctional alpha2-autoreceptors in human blood vessels. Segments of gastric and ileocolic arteries were incubated with [3H]noradrenaline and subsequently perifused with modified Krebs-Henseleit solution containing cocaine (12 microM). Five periods of electrical stimulation (S1-S5) were applied (1 Hz, 1 ms, 50 V for 1 min). Concentration-response curves for the facilitatory effect of eight alpha-adrenoceptor antagonists [rauwolscine, 2-[2-(2-methoxy-1,4-benzodioxanyl)] imidazoline (RX821002), yohimbine, phentolamine, idazoxan, 2-(2',6'-di
