Ménétrier's disease: a narrative review of molecular pathogenesis, clinical spectrum and evolving therapeutic strategies.

Ménétrier's disease (MD) is a rare, acquired premalignant condition characterized by hypertrophic gastropathy. Its defining features include giant gastric rugal folds, predominantly in the fundus and body of the stomach, foveolar hyperplasia with glandular atrophy and a protein-losing gastropathy leading to hypoalbuminemia and peripheral edema. The pathogenesis is primarily driven by the overactivation of the epidermal growth factor receptor (EGFR) signaling pathway, most commonly due to the overexpression of its ligand, transforming growth factor-alpha (TGF-α). The clinical presentation varie

[Ultrasonic patterns of giant hypertrophy of the gastric mucosa (Ménétrier's disease) (author's transl)].

Since it can display the gastric folds, as well as the gastric wall and the exogastric area, sonography is a reliable way of diagnosing the giant hypertrophy of the gastric mucosa (Ménétrier's disease). The characteristic pastterns are: --a considerable thickening of the gastric folds and wall, --a high sonolucency, --the presence of the intra-mural cysts, --some "hilly" deformations in the exogastric part of the stomach, --occasionally, a mesenteric lipomatosis.

Immunolocalization of transforming growth factor-alpha in normal and diseased human gastric mucosa.

Roles for transforming growth factor-alpha (TGF alpha) in the stomach include cell migration and proliferation, inhibition of acid secretion, and cytoprotection. The authors have previously shown increased TGF alpha expression in rat gastric mucosa in response to acute gastric injury. They also have shown that TGF alpha immunoreactivity is increased in the gastric mucosa of four patients with Ménétrier's disease. To further characterize TGF alpha immunoreactivity in human gastric mucosa, the authors have performed immunohistochemical analysis with an anti-TGF alpha monoclonal antibody on human

Hyperplastic foveolar gastropathies and hyperplastic foveolar gastritis.

Thirteen gastrectomy specimens having diffuse (n = 5), focal (i.e., nodular, n = 6) or combined (n = 2) giant hypertrophic folds at gross examination were reviewed. Of the five specimens with grossly diffuse hypertrophic fundic mucosal folds, two had at histology tortuous foveolar hyperplasia (without intraepithelial lymphocytosis) and prominent glandular cysts; they were classified as Ménétrier's gastropathy. The other three specimens with diffuse foveolar hyperplasia had serrated foveolar infoldings with marked intraepithelial lymphocytosis; they were classified as Ménétrier-like lymphocytic

Benign lymphoid hyperplasia of the gastric antrum - another cause of "etat mammelonne".

The radiological and endoscopic appearance of "etat mammelonne" has been attributed to hypertrophic gastritis or prominence of the areae gastricae. An additional cause for this appearance is described in two patients with benign lymphoid hyperplasia of the gastric antrum. The significance of these lesions is discussed.

Evaluation of gastroscopic biopsy for diagnosis of Cryptosporidium sp. infection in snakes.

Cryptosporidium sp. is a protozoal parasite that can cause hypertrophic gastritis, regurgitation, and chronic wasting in snakes. Diagnosis can be challenging because of intermittent shedding of the organism in the feces. The purpose of this study was to evaluate gastroscopic biopsy as a diagnostic method for Cryptosporidium sp. screening during an outbreak at the Oklahoma City Zoo. Twenty-two snakes were immobilized by chamber induction with an inhalant anesthetic. Gross lesions were seen in seven snakes and consisted of hyperemia, thickening of the gastric mucosa as observed by lack of rugal

Ménétrier Disease: Rare Manifestations and Mechanistic Insights From 2 Adult Cases.

BACKGROUND Menetrier disease is a rare gastric disorder characterized by hypertrophic hyperplastic changes in the stomach mucosa, hypoproteinemia, and symptoms like abdominal pain and diarrhea. It predominantly affects men aged 40-60 years and involves protein loss from the stomach. There have been few reports of cases complicated with severe anemia and secondary gastric outlet obstruction. The pathogenesis in adults remains unclear, although pediatric cases often are associated with cytomegalovirus (CMV) infection. CASE REPORT We present 2 adult cases. Case 1 was a 35-year-old man with a 2-mo

Hypertrophic pyloric stenosis in the adult.

In view of the low incidence of hypertrophic pyloric stenosis, we present a case of this pathology in a male aged 74. Stenosis was of the diffuse type, associated with gastric ulcer and chronic atrophic gastritis. The patient was admitted to our Service with upper digestive tract hemorrhage after deterioration of the ulcer.

Menetrier's disease in childhood: report of two cases and a review of the literature.

Two new cases plus 13 previously reported cases of Menetrier's disease in childhood are reviewed. The most common features of this illness are severe upper gastrointestinal symptoms (abdominal pain, nausea, vomiting, hematemesis) followed by the onset of generalized edema and ascites. Significant laboratory findings include hypoalbuminemia, eosinophilia, and anemia. Gastrointestinal protein loss and decreased gastric acid secretion can be documented. Upper gastrointestinal radiographs demonstrate the characteristic hypertrophic gastric rugae. Histologic features include hypertrophic tortuous g

A clinicopathologic study of 42 patients with granulomatous gastritis. Is there really an "idiopathic" granulomatous gastritis?

Idiopathic granulomatous gastritis (IGG) is a diagnosis made only by excluding other causes of granulomatous gastritis, such as infection, foreign bodies, and systemic granulomatous diseases. Recently, several investigators have questioned the existence of IGG. We reviewed the slides and clinical data of all cases of granulomatous gastritis seen at the Cleveland Clinic between 1975 and 1994. In addition to routine hematoxylin and eosin stains, slides from all cases were stained with Ziehl-Neelsen, Gomori's methenamine silver, and Giemsa stains. Clinical information and follow-up were available

Malignant lymphoma associated with reactive lymphoreticular hyperplasia of the stomach--a case report.

We report a case of early malignant lymphoma of the stomach, associated with reactive lymphoreticular hyperplasia. This case seems to prove that there is some evidence of sarcomatous change in reactive lymphoreticular hyperplasia.

Menetrier's disease in children: report of a case and review of the pediatric literature.

A case of Menetrier's disease (giant hyperplasia of the gastric mucosa) in a 2-day-old infant is presented. The lesion was primarily confined to the gastric antrum. It resulted in outlet obstruction and necessitated partial gastrectomy. The pertinent literature is critically evaluated, and probably only 2 of the 9 previously reported cases of Menetrier's disease in children qualify fully.

Intramucosal carcinoma of the gastric antrum complicating Menetrier's disease.

A 48-year-old man presented with haematemesis and was found to have Menetrier's disease. Three and a half years later early gastric cancer was detected at routine follow-up endoscopy. This is the first report of the development of early gastric cancer during follow-up in a case of Menetrier's disease. It is probable that patients with Menetrier's disease are at increased risk of gastric cancer, and should have regular endoscopic follow-up with gastric biopsy.

[Morphology of chronic gastritis].

The objective of the paper is the specification of morphological criteria for the diagnostics of chronic gastritis. Biopsies, taken from the antral and corporal mucosa of 317 patients, were studied. Light chronic superficial gastritis was established in 184 cases. Manifested chronic superficial gastritis--in 178 cases, 119 of them in the antrum. Chronic gastritis with initial atrophy of the glands was established in II cases, and complete--in one. Chronic gastritis with intestinal metaplasia was observed in the antral mucosa mainly, and with various degrees of glandular atrophy both in case of

Future candidates for indications of Helicobacter pylori eradication: do the indications need to be revised?

Since the discovery of Helicobacter pylori in 1982, the development of several treatment guidelines has allowed a consensus on the indications for H. pylori eradication. Beyond these currently accepted indications, including various upper gastrointestinal disorders and extragastric diseases, a significant amount of new information regarding H. pylori eradication is emerging. Certain types of acute gastritis, such as nodular gastritis, hypertrophic gastritis, Ménétrier's disease, hemorrhagic gastritis, and granulomatous gastritis are reversible after H. pylori eradication. Further, for chronic …
