Postoperative clostridium difficile gastroenteritis.

Clostridium difficile gastroenteritis can be the cause of an enigmatic postoperative syndrome of high temperature and marked leukocytosis, out of proportion to the initially mild constitutional symptoms. Patients may suffer delayed onset of diarrhea, which will test positive for the C. difficile enterotoxin by latex agglutination. We report 5 cases of C. difficile gastroenteritis that occurred within a 2-year period. We believe that the combination of preoperative bowel preparation, and intraoperative and postoperative systemic antibiotics is the primary operant factor. All patients responded

Hyperglycaemia in infantile gastroenteritis.

The prevalence and pathogenesis of hyperglycaemia were investigated in a consecutive series of 27 black infants admitted to hospital with gastroenteritis over a period of three months. Hyperglycaemia (plasma glucose concentration greater than 10 mmol/l) occurred in 15 (55%) of these patients. The pathogenesis was not clear but possible contributory factors included raised concentrations of the stress hormones pancreatic glucagon, growth hormone, and cortisol; hypokalaemia; and peripheral insulin resistance. Intravenous rehydration, without insulin, corrected the plasma glucose concentrations a

Gastroenteritis in Alice Springs.

One hundred and thirty admissions to Alice Springs Hospital of full-blood Aboriginal children with acute gastroenteritis during a six-months period in 1975-1976 were examined retrospectively. Sixty-two (48%) of the children were malnourished, including seven (5.4%) who were marasmic. An accelerated weight gain was demonstrated during the recovery phase, averaging 418% of that expected for age. Hypokalaemia occurred in 77 of 117 children (66%); it was severe (serum potassium levels being less than or equal to 2.5 mmol/l.) in 15 (13%), and did not correlate with dehydration as assessed by initia

An Unexpected Complication of Gastroenteritis.

A 37-year-old man, previously healthy, presented to the emergency department with retrosternal chest pain for 24 hours, in the context of watery diarrhea (five to six times a day), abdominal pain, and fever evolving over four days. Following medical assessment, a diagnosis of myopericarditis due to Campylobacter jejuni infection was made. This complication of C. jejuni infection is rare and poorly described, but given its rising incidence, increased vigilance is necessary. The affinity of C. jejuni for the myocardium and pericardium remains unclear. The underlying mechanisms are still under in

Admission to hospital with gastroenteritis.

A prospective study over a one year period examined preadmission illness and its treatment, social characteristics and referral patterns, and inpatient illness progression in 1148 children admitted with a primary diagnosis of gastroenteritis. Admissions were predominantly from socially disadvantaged families: 712 (62%) from social classes IV and V. Approximately a quarter were referred with minimal symptoms, only 12 (1%) with moderate to severe dehydration, and eight (less than 1%) with hypernatraemia. One hundred and ninety two of 1101 (17%) had not seen their general practitioner during the

A survey of infantile gastroenteritis.

In 1967 we admitted 339 cases of infantile gastroenteritis; one-third of these were dehydrated, and in this group the commonest biochemical abnormality found was hypernatraemia, sometimes with metabolic acidosis. A higher incidence of dehydration was found in the patients who had received oral glucose fluids before admission. EnteropathicEscherichia coliwere isolated from the faeces of 16% of the cases. Associated infections, especially of the respiratory tract, were common. Treatment was aimed at the restoration of fluid and electrolyte balance. Usually this was achieved with oral fluids, tho

Epidemiology of Norwalk gastroenteritis and the role of Norwalk virus in outbreaks of acute nonbacterial gastroenteritis.

Outbreaks of Norwalk gastroenteritis, which may involve persons of all ages, occur during all seasons and in various locations. Waterborne, foodborne, and person-to-person modes of transmission have been described, and secondary person-to-person transmission is common. Outbreaks generally end in about 1 week; longer outbreaks occur only when new groups of susceptible persons are introduced, usually in the setting of a persistent common source of infection. The illness is generally mild and characterized by nausea, vomiting, diarrhea, and abdominal cramps. Vomiting is the predominant symptom am

Campylobacteriosis in India: Beyond gastroenteritis.

BACKGROUND: Campylobacter species, particularly Campylobacter jejuni and Campylobacter coli, are among the leading global causes of bacterial gastroenteritis and are primarily transmitted through contaminated food, water and animal products. Despite their recognised role in enteric infections, the broader clinical and public health significance of Campylobacter remains underappreciated, particularly in developing countries such as India. Beyond gastrointestinal illness, these pathogens are increasingly associated with a range of extraintestinal complications, including bacteremia, neurological

[Noroviruses: leading cause of gastroenteritis].

Although noroviruses were the first viral agents to be linked to gastrointestinal disease, they were long considered a secondary cause far behind rotaviruses. Development of molecular-based diagnostic techniques has provided clearer insight into the epidemiological impact of noroviruses that are now recognized not only as the leading cause of non-bacterial gastroenteritis outbreaks but also as an important cause of sporadic gastroenteritis in both children and adults. Norovirus infection is generally characterized by mild acute vomiting and diarrhea usually lasting for only a few days, but it

[Acute viral gastroenteritis in children].

The article presents common etiologic agents of viral gastroenteritis in children. The actual knowledge on molecular characteristics of rotaviruses, noroviruses, sapoviruses, astroviruses, and adenoviruses as well as their epidemiology in the world and Poland is reviewed. The authors emphasize the need of greater use of molecular procedures (RT-PCR) for detection the common serotypes of rotavirus and other viral agents of gastroenteritis in Polish children. Familiarity with molecular epidemiology of viruses and with their antigenic diversity allows to elaborate and apply the adequate vaccines

Gastroenteritis due to Listeria monocytogenes.

It has been known for a long time that many patients experience diarrhea antecedent to the development of bacteremia or meningoencephalitis due to Listeria monocytogenes, but it was only recently that convincing evidence was obtained that this organism can cause acute, self-limited, febrile gastroenteritis in healthy persons. At least 7 outbreaks of foodborne gastroenteritis due to L. monocytogenes have been reported. Illness typically occurs 24 h after ingestion of a large inoculum of bacteria and usually lasts 2 days. Common symptoms include fever, watery diarrhea, nausea, headache, and pain

Eosinophilic gastroenteritis mimicking acute appendicitis.

Eosinophilic gastroenteritis is a rare entity that can be treated successfully with glucocorticoid therapy if the appropriate diagnosis is made. However, it may present with symptomatology mimicking acute surgical conditions. We present the case of a 26-year-old man who presented with diffuse epigastric pain, nausea, vomiting, and diarrhea. Extensive workup including upper endoscopy and imaging study revealed gastritis with ulcer and ascites. The patient developed right lower quadrant pain with localized peritonitis and leukocytosis. He underwent appendectomy and small bowel biopsy. Pathology

[Afebrile seizures in rotavirus gastroenteritis].

Rotavirus is an important cause of acute gastroenteritis in infants. Gastroenteritis has been reported in association with a variety of other disease conditions, such as respiratory infections and CNS involvement. A case of a child, thirteen months old, presenting afebrile seizures during a rotavirus gastroenteritis is described.

Eosinophilic gastroenteritis: 10 years experience.

Eight patients (five men, three women), mean age 36.9 +/- 13.5 (17-60) yr, were diagnosed to have eosinophilic gastroenteritis. The condition was proved in five patients by biopsies through endoscope, and in three, by operation. All had hypereosinophilia (absolute eosinophil count of 1,337-21,787/cm3). According to Klein classification, two had mucosal disease, three had muscle layer disease, and three, subserosal disease. The most common symptoms were abdominal pain (100%), diarrhea (62.5%), vomiting (62.5%), and nausea (50%). Four patients (50%) had symptoms for more than 1 yr before diagnos

[Eosinophilic gastroenteritis: two case reports].

Eosinophilic gastroenteritis is a rare lesion that may present a diagnostic problem to the surgeon. It may be confused with a malignant tumor because it often presents with intestinal obstruction. We treated two cases at the Hôpital de zone de Morges. In one case the lesion was in the gastric antrum and in the other the ascending colon. Only 27 cases of colonic eosinophilic gastritis are described in the medical literature.
