[Medical assessment prior to gastroplasty: indications and contraindications].

Because of the common failure of the various medical procedures used for the treatment of morbid obesity, surgeons have searched alternatives among which gastroplasty appears to offer now the best efficacy/safety ratio. Such approach has become more and more popular in our country, but success is almost highly variable from patient to patient. A careful medical assessment is necessary before performing gastroplasty. It mainly aims at choosing the appropriate indications (failure of previous well-applied medical therapy, exclusion of any hormonal cause of obesity, presence of complications due

Carcinoma developing in the neo-oesophagus following gastroplasty.

Carcinoma developing after gastroplasty and fundoplication has been documented in only five cases. We report a further case occurring in a patient 4 years after initial surgery. The tumour originated in the newly created tubular extension. There was no evidence of a columnar lining in the true oesophagus prior to surgery. The development, technique and complications of gastroplasty and partial fundoplication are discussed with special mention of the diagnostic problems when a tumour develops in a patient who has undergone this procedure.

The role of endoscopy after vertical banded gastroplasty.

Since 1984, a total of 99 patients underwent vertical banded gastroplasty (VBG) through protocol (pouch 8 ml in size, band 4.3 cm in circumference) to treat morbid obesity. Follow-up was obtained in 95 patients. Thirty upper gastrointestinal endoscopies were performed post-operatively in 17 patients. Indications were nausea/vomiting in 11, epigastric pain in 4, acute obstructive symptoms in 4, and miscellaneous in three. Findings included food impaction in 10, distal esophagitis in 8, gastritis in 4, and a normal examination in 2. Only 4 of 10 food impactions were associated with an excessivel

An experimental study of the correction of gastroesophageal reflux by gastroplasty.

The rationale and efficacy of gastroplasty in controlling gastroesophageal reflux were studied by manometry and cinefluorography in the dog. The pressure in a 7 by 3 centimeter gastric tube, constructed from the lesser curvature of the stomach and interposed between the esophagus and the stomach, was 3.6 +/- 1.0 centimeters higher than in the stomach. When abdominal compression was applied, the pressures were similar in the tube and the adjacent stomach, 19.0 +/- 4.0 and 19.4 +/- 4.0 centimeters, and free reflux was demonstrated between the two. When the gastric fundus was wrapped around the g

Gastropericardial Fistula After Collis Gastroplasty and Nissen Fundoplication.

Abnormal fistulization is a rare complication of Nissen fundoplication. A high index of suspicion and timely diagnosis are key factors to guide appropriate management. We describe a 70-year-old woman with a remote history of Collis gastroplasty and Nissen fundoplication presenting with chest pain and a pericardial effusion. She was transferred to our hospital after a pericardiocentesis drained gastric content. She underwent an emergent pericardial window for hemodynamic instability and a left thoracotomy to manage the fistula. We describe the successful management of her gastropericardial fist

Long-Term Results of Primary Vertical Banded Gastroplasty.

BACKGROUND: The vertical banded gastroplasty (VBG) used to be a common restrictive bariatric procedure but has been abandoned by many due to a high failure rate, a high incidence of long-term complications, and the newer adjustable gastric band (AGB) and sleeve. However, potential favorable long-term results and the upcoming banded gastric bypass, with a similar mechanical outlet restriction and control of the pouch size, renewed our interest in the VBG. Therefore, we investigated the long-term outcome of primary VBG at the Catharina Hospital in the Netherlands. METHODS: Patients that underwe

Incidence of incisional hernia following vertical banded gastroplasty.

BACKGROUND: Our aim was to determine which patient-related factors influence the incidence of incisional hernia after vertical banded gastroplasty for morbid obesity. METHODS: We reviewed the medical records of 80 morbidly obese patients operated on between 1986 and 1993. All the operations were performed by only one surgeon, and the midline laparotomy was closed by means of continuous polyglactin 910 suture. Statistical analysis was performed using the Fisher exact test, and significance was assigned for values of P<0.05. RESULTS: Incidence of incisional hernia in: obese 24%, superobese 51%

Endoscopic Sleeve Gastroplasty Leading to Gastric Ischemia and Perforation.

Endoscopic sleeve gastroplasty (ESG) is performed by using endoscopic suctioning to reduce the volume of the stomach by plicating the greater curvature with an endoscopic suturing device or stapler. This allows for the endoscopist to perform an elective outpatient weight loss procedure. We will describe a single case of post-procedure day zero complication involving ESG resulting in ischemia, perforation, and peritonitis; what was discovered intra-operatively; and our operative management.

Laparoscopic Magenstrasse and Mill Gastroplasty (M&M): Midterm Results.

BACKGROUND: The Magenstrasse and Mill gastroplasty (M&M) is a gastric restrictive procedure without band or stomach resection. Short-term evaluation of the laparoscopic procedure showed low morbidity and satisfactory results on weight loss. Evidence of the validity of the technique in the longer term is scarce. METHODS: Data from patients who underwent M&M procedure from May 2012 to September 2015 were retrospectively reviewed. Preoperative clinical characteristics and data up to 4 years after operation were analyzed. RESULTS: A total of 132 patients were included in this study with a mean a

Patients are well served by Collis gastroplasty when indicated.

BACKGROUND: Debate persists over the impact of Collis gastroplasty (CG) on outcomes after anti reflux surgery. This study analyzed operative and quality of life (QOL) outcomes from one of the largest series of laparoscopic anti reflux surgery (LARS) with CG reported to date. METHODS: A retrospective review was conducted to compare outcomes between patients undergoing LARS with CG versus without CG at two large centers with expertise in foregut surgery from October 2004-December 2011 and July 2012-September 2016. Demographic, perioperative, and QOL data were reviewed. Four validated surveys we

Vertical isolated gastroplasty with gastro-enteral bypass: preliminary results.

BACKGROUND: The authors present a new restrictive and malabsorptive operation for treatment of morbid obesity, called vertical isolated gastroplasty (VIG). METHODS: From Feb 2004 to May 2005, 30 patients with BMI > or = 40 kg/m2 or >35 kg/m2 with co-morbidities underwent VIG via laparoscopy or laparotomy. The technique consisted in creation of a gastric tube preserving pylorus, and a Roux-limb of 300 cm to the bypassed stomach with the division 30 cm distal to the ligament of Treitz. Excess weight loss (EWL), BMI, complications and co-morbidities were assessed. RESULTS: BMI and average preop

Silastic ring vertical gastroplasty- long-term results and complications.

BACKGROUND: Silicone ring vertical gastroplasty (SRVG) in some reports is associated with significant morbidity and a tendency to regain weight in the late postoperative period. The present study aims to evaluate our long-term results, along with early and late postoperative complications. METHODS: The early and late postoperative complications of 300 patients undergoing SRVG and followed for an average of 3.2 years, were reviewed retrospectively. The pre- and post-operative weight and body mass index (BMI) were recorded in a subgroup of 131 patients with an average follow-up of 5.2 years. R

[Gastric adenocarcinoma following "silastic vertical ring gastroplasty": case report].

OBJECTIVES: Bariatric surgery is considered as the most effective therapy for morbid obesity. But, each procedure carries both short-and long-term complications. And, it remains unclear if the late occurrence of gastric adenocarcinoma could be linked to bariatric surgery. We described a case of a female who developed a gastric adenocarcinoma after a silastic ring vertical gastroplasty (SRVG). METHODS: A 54-year-old female presented with postprandial vomiting, poor appetite, dysphagia and weight loss 10 year after a SRVG. A gastroscopy with biopsy disclosed a juxta-pyloric adenocarcinoma. Unusual late-onset Wernicke's encephalopathy following vertical banded gastroplasty.

Wernicke's encephalopathy (WE) related to bariatric surgery is the consequence of thiamine depletion occurring usually after restrictive surgical procedures with gastric outlet impairment causing frequent vomiting. We present a 35-year-old man with body mass index of 47.2 who developed a WE 7 years after a vertical banded gastroplasty. Late stenosis of the outlet due to gastric band inclusion was the precipitating mechanism. Poor compliance of dietary pattern and vitamin supplementation along with episodic vomiting both contributed to progressive symptoms of instable gait and mental changes. M

Long-term symptomatic outcomes after Collis gastroplasty with fundoplication.

Collis gastroplasty with fundoplication is an accepted treatment for gastroesophageal reflux disease (GERD) complicated by short esophagus. The procedure can be done either via left thoracotomy or using minimally invasive laparoscopic techniques. Few centers have reported long-term follow-up for patients undergoing a Collis gastroplasty using both the open and minimal access techniques. Retrospective review of prospectively collected data at Creighton University was done to identify patients who underwent Collis gastroplasty with fundoplication for GERD. After approval from the institutional r

Determinants of long-term satisfaction after vertical banded gastroplasty.

BACKGROUND: The long-term usefulness of vertical banded gastroplasty (VBG) in achieving weight loss is controversial, and adverse effects related to the procedure may attenuate patient satisfaction. Our objective was to evaluate patient satisfaction, and to identify parameters that are related to such satisfaction, 3 to10 years after VBG. METHODS: All consecutive patients who underwent VBG in one surgical ward were invited for a follow-up study 3 to 10 years after surgery. Questions relating to symptoms and quality of life were evaluated in a personal interview using a structured questionnair
