Coverage for Gender-Affirming Care: Making Health Insurance Work for Transgender Americans.

Many transgender Americans continue to remain uninsured or are underinsured because of payers' refusal to cover medically necessary, gender-affirming healthcare services-such as hormone therapy, mental health counseling, and reconstructive surgeries. Coverage refusal results in higher costs and poor health outcomes among transgender people who cannot access gender-affirming care. Research into the value of health insurance coverage for gender-affirming care for transgender individuals shows that the health benefits far outweigh the costs of insuring transition procedures. Although the Affordab

A Community Survey of Referral Sources to Identify Primary Care and Gender-Affirming Care Providers.

INTRODUCTION: Barriers exist in access to primary care as well as specialty healthcare such as gender-affirming care. Understanding the referral sources used to identify new providers for these types of care can help healthcare systems facilitate access. METHODS: Using data from a community-based survey, demographics and information relevant to finding new healthcare providers were assessed. RESULTS: Data from 165 participants suggest that seeking a new primary care provider was perceived as challenging. The most common referral sources for primary care providers were family/friends, a docto

Mental Health Outcomes in Transgender and Nonbinary Youths Receiving Gender-Affirming Care.

IMPORTANCE: Transgender and nonbinary (TNB) youths are disproportionately burdened by poor mental health outcomes owing to decreased social support and increased stigma and discrimination. Although gender-affirming care is associated with decreased long-term adverse mental health outcomes among these youths, less is known about its association with mental health immediately after initiation of care. OBJECTIVE: To investigate changes in mental health over the first year of receiving gender-affirming care and whether initiation of puberty blockers (PBs) and gender-affirming hormones (GAHs) was

Health Care Experiences of Patients Discontinuing or Reversing Prior Gender-Affirming Treatments.

IMPORTANCE: Medical education, research, and clinical guidelines are available to support the initiation of gender-affirming care for transgender and nonbinary people. By contrast, little is known about the clinical experiences of those who discontinue or seek to reverse gender-affirming medical or surgical interventions due to a change in gender identity, often referred to as detransition. OBJECTIVE: To examine the physical and mental health experiences of people who initiated medical or surgical detransition to inform clinical practice. DESIGN, SETTING, AND PARTICIPANTS: Using constructivi

Assessment of Cardiovascular Risk in Transgender Patients Presenting for Gender-Affirming Care.

BACKGROUND: The transgender population is rapidly growing in the United States and abroad. Transgender men and women are marginalized as a result of their transgender status, with resultant health repercussions. This and other factors such as increased substance use, mental health disorders, violence, and chronic stress may place transgender individuals at higher risk for cardiovascular disease. Additionally, many transgender patients pursue gender-affirming hormone therapy, which has been linked to increased rates of some cardiovascular events such as metabolic syndrome, venous thromboembolis

The Effect of Gender-Affirming Medical Care on the Vaginal and Neovaginal Microbiomes of Transgender and Gender-Diverse People.

Transgender and gender diverse individuals may seek gender-affirming medical care, such as hormone therapy or surgery, to produce primary and/or secondary sex characteristics that are more congruent with their gender. Gender-affirming medical care for transmasculine individuals can include testosterone therapy, which suppresses circulating estrogen and can lead to changes in the vaginal epithelium that are reminiscent of the post-menopausal period in cisgender females. Among transfeminine individuals, gender-affirming medical care can include vaginoplasty, which is the surgical creation of a v

A Dialectical Behavior Therapy-Oriented Approach to Gender-Affirming Eating Disorder Care for Youth With Gender Dysphoria.

Sam is a 15-year-old, transmasculine teen who reports gender dysphoria since early elementary school. He was referred for psychotherapy following hospitalization for a suicide attempt secondary to bullying. Clinical history revealed long-standing major depressive disorder, social anxiety, gender dysphoria, and anorexia nervosa-restricting type (AN-R). His family exhibits gender-affirming beliefs and language publicly but has struggled significantly with accepting that their assigned female at birth (AFAB) child does not perceive himself as female; they worry about social repercussions for thei

Mental health implications of gender affirming care: bridging the communication and supportive gap.

Gender-affirming healthcare is a comprehensive approach that aims to support individuals in transitioning from the gender assigned to them at birth to their affirmed gender, aligning with their gender identity. This type of care involves medically necessary and scientifically supported interventions within a multidisciplinary framework. Gender dysphoria, also known as gender incongruence or transsexuality, refers to the experience of a mismatch between an individual's biological sex and their internal sense of gender, known as gender identity. Gender identity can encompass a range of expressio

Recent Findings on Psychosocial Outcomes of Gender-Affirming Medical Care for Transgender Youth.

Many transgender and gender-diverse (TGD) youth experience gender dysphoria, which can exacerbate mental health risk. Gender-affirming medical interventions, such as pubertal suppression using gonadotropin-releasing hormone agonists (GnRHas) and gender-affirming hormone therapy (GAHT), primarily aim to alleviate gender dysphoria, with overall goals of optimizing well-being. Here we discuss prospective, longitudinal studies published in the last 5 years examining mental health outcomes of GnRHas and GAHT for TGD youth and young adults, contextualize these findings within the current sociopoliti

Gender affirming surgery in non-binary patients: The importance of patient-centered care.

BACKGROUND: Gender-affirming mastectomies are a growing necessity for transgender and gender-diverse patients. The preoperative evaluation and surgical outcome must be tailored to the individual, taking into consideration previous medical history, medications, hormonal therapy, patient anatomy, and expectations. Although non-binary patients constitute a significant portion of patients referring for gender-affirming mastectomies, current literature rarely acknowledges them as a separate patient category from trans-masculine patients. METHODS: Retrospective cohort, demonstrating the single-surg

Clinician Perspectives on Integrating Biomedical HIV Prevention with Gender-Affirming Care Delivered via Telehealth.

Preexposure prophylaxis (PrEP) and postexposure prophylaxis (PEP) continue to be underutilized among transgender people despite elevated HIV incidence in this population. Many transgender individuals report that their gender-affirming care (GAC) is their highest health care priority, and an increasing number seek this care via telehealth. We sought to understand clinician perspectives and needs regarding integrating HIV PrEP and HIV PEP into GAC in a telehealth model. We conducted semi-structured interviews with clinicians who self-identified as providing gender-affirming care via telehealth.
