Automated Fall Detection Technology in Inpatient Geriatric Psychiatry: Nurses' Perceptions and Lessons Learned.

ABSTRACTHospitalized older adults are at high risk of falling. The HELPER system is a ceiling-mounted fall detection system that sends an alert to a smartphone when a fall is detected. This article describes the performance of the HELPER system, which was pilot tested in a geriatric mental health hospital. The system's accuracy in detecting falls was measured against the hospital records documenting falls. Following the pilot test, nurses were interviewed regarding their perceptions of this technology. In this study, the HELPER system missed one documented fall but detected four falls that wer

Sedatives and analgesics are major contributors to potentially inappropriate duplicate prescriptions in geriatric psychiatry.

BACKGROUND: This study sought to investigate the frequency and characteristics of duplicate prescriptions (DPs) in elderly psychiatric inpatients using a novel categorisation of DPs that differentiates between appropriate duplicate prescriptions (ADPs) and potentially inappropriate duplicate prescriptions (PIDPs). METHODS: The study was conducted as a monocentric retrospective cross-sectional pilot study on the gerontopsychiatric ward of the Department of Psychiatry, Social Psychiatry and Psychotherapy of Hannover Medical School, a large university hospital in northern Germany. The outcome me

[Interdisciplinary research strategies in geriatric psychiatry--studies and results in dementia and depressed patients].

Eighty-one patients (mean age: 66 +/- 9 years) who had been in gerontopsychiatric in-patient care were included in the study. As well as physical, psychiatric and neurological examinations, EEG, brain CT scanning and the determination of the Ischemic Scale were performed, in order to confirm the clinical diagnosis of dementia of Alzheimer type (DAT), dementia of vascular type (DVT) or multi-infarct dementia (MID), and depression in old age, as based on the DSM III criteria. A comprehensive psychological test battery was administered, to one section of the subjects. Our results indicate that EE

Characteristics and Satisfaction with Services of Patients Attending a Geriatric Psychiatry Clinic in Southern India.

BACKGROUND: Specialist services, such as geriatric psychiatric clinics, aim to provide a multi-disciplinary and person-centred approach. There is not enough information available about the number and type of such clinics in India. In this descriptive article, we aim to detail the services provided and the socio-demographic and clinical characteristics of patients who have attended a geriatric psychiatry outpatient clinic in southern India. METHODS: A specific data sheet was used to record the socio-demographic and clinical details of patients attending the weekly geriatric psychiatry outpatie

Effects of E-Learning Materials on Caregivers' Knowledge about the 3Ds of Geriatric Psychiatry.

BACKGROUND: E-learning has emerged as a crucial technique that educators embrace and apply. Two effective e-learning materials are video-based learning (VBL) and e-book-based remote learning (EBRL). While several studies have proven their effectiveness, none have compared these e-learning materials, especially regarding caregivers' knowledge of delirium, dementia, and depression (3Ds). Based on the extant literature, many caregivers are untrained and often lack the knowledge and skills to lessen the burden on delirious, demented, and depressed older adults. Hence, there is a growing need to ex

[Therapeutic use of carbamazepine for treatment of agitation and affective disorders in geriatric psychiatry patients].

Symptomatic behavioral disturbances including agitation, motor restlessness, outbursts and hostility remain a major problem in clinical management of gerontopsychiatric patients. Termination of home care and subsequent hospitalisation is caused largely by behavior disturbances and not cognitive deficits. Clinical efficacy of neuroleptic treatment seems to be modest and is limited by unwanted side effects. Carbamazepine shows psychotropic effects and seems to improve limbic pathology including aggression and poor impulse control in various psychiatric disorders. Treatment with carbamazepine was

Visual manifestations of occipital lobe infarction in three patients on a geriatric psychiatry unit.

The authors present three cases of hospitalized patients on a geriatric psychiatry floor who were found to have previously undiagnosed occipital lobe infarctions associated with visual manifestations. The manifestations discussed are visual field defects, visual hallucinations, and color anomia. The incidence of undiagnosed occipital lobe infarctions and the contribution of these infarctions to visual perception changes in this patient population are unknown. The authors suggest that for patients who present with visual perception changes, a high index of suspicion for occipital lobe infarctio

[When psychiatric symptoms mask a neurological cause: A case report of epilepsy in geriatric psychiatry].

Atypical neuropsychiatric presentations in older adults pose significant diagnostic challenges, particularly when psychiatric symptoms predominate early in the clinical course. We report the case of a 68-year-old woman referred to geriatric psychiatry for depressive symptoms associated with fluctuating cognitive complaints, brief paroxysmal episodes of confusion and language disturbance, and atypical somatic and motor manifestations. Initial investigations, including brain imaging and laboratory testing, were unremarkable, leading to preliminary consideration of primary psychiatric or function

Moving forward: evaluating a curriculum for managing responsive behaviors in a geriatric psychiatry inpatient population.

PURPOSE: The Gentle Persuasive Approaches (GPA) curriculum was developed as an adjunct to other educational initiatives that were part of Ontario, Canada's Alzheimer Strategy. GPA emphasizes that an individual's unique personal history has a direct application to the interpretation of and response to their behavior. It incorporates strategies into geriatric patient care to assist staff to respond effectively to verbal and physical expressions of need. DESIGN AND METHODS: A pre- and postintervention approach was used to evaluate the effectiveness of GPA: (a) Staff Satisfaction Surveys immediat

The geriatric psychiatry workforce in 2002: analysis from the 2002 National Survey of Psychiatric Practice.

OBJECTIVE: Using the 2002 National Survey of Psychiatric Practice (NSPP), authors update demographic and professional work activities of a nationally representative sample of the psychiatric workforce who treat geriatric patients. METHODS: The sampling frame for the 2002 NSPP used the American Medical Association's Masterfile of Physicians. Professional, demographic, and patient data are compared between general and self-reported geriatric psychiatrists. Authors also present detailed analyses comparing high geriatric providers (HGPs; geriatric caseloads >20%), and board-certified geriatric ps

Sarcopenia in geriatric psychiatry: feasibility of the diagnostic process and estimation of prevalence within a hospital context.

BACKGROUND: Sarcopenia is an age-related progressive and general skeletal muscle disease associated with negative consequences such as falls, disability, and mortality. An early-stage diagnosis is important to enable adequate treatment, especially in geriatric psychiatry. However, there presently is little information about the feasibility of diagnostic procedures and the prevalence of sarcopenia in clinical geriatric psychiatry settings. The aim of this study is to implement a diagnostic process for sarcopenia in a geriatric psychiatry hospital, to investigate its feasibility and to analyse t

Psychiatric diagnosis in the elderly referred to a consultation-liaison psychiatry service in a general geriatric hospital in Japan.

AIM: Because depression is a common disorder in later life, elderly patients with delirium can be misdiagnosed as having depression. This study aimed to compare psychiatric diagnoses in the elderly made by referring doctors and psychiatrists. METHOD: Consecutive non-psychiatric inpatients aged 65 years or older that were referred to a consultation-liaison (C-L) psychiatry service of a general hospital in Japan were enrolled. An attending psychiatrist recorded the physical and psychiatric diagnoses of the referring doctors, reason for referral, psychotropic medication and sociodemographics. Th

Optimizing antidepressant use in geriatric depression: expert consensus from French societies of geriatrics, old age psychiatry, and clinical pharmacy (SFGG, SF3PA, and SFPC).

BACKGROUND: Late-life depression refers to a depression occurring in older adults, defined as individuals aged 65 years and older. It is common and often mismanaged due to complex clinical profiles, polypharmacy, and limited evidence-based guidance. Existing tools poorly address psychiatric nuances in older adults. This work aims to develop French recommendations based on experts' consensus on the use of antidepressants in unipolar late-life depression, to guide safer prescribing practices. METHODS: This tool was developed using a Delphi survey, based on a review of literature published betwe

Mortality in association with antipsychotic medication use and clinical outcomes among geriatric psychiatry outpatients with COVID-19.

OBJECTIVES: Older adults are particularly vulnerable to the negative consequences of antipsychotic exposure and are disproportionally affected by higher mortality from coronavirus disease 2019 (COVID-19). Our goal was to determine whether concurrent antipsychotic medication use was associated with increased COVID-19 mortality in older patients with preexisting behavioral health problems. We also report on findings from post-COVID follow-ups. DESIGN: Retrospective observational study. PARTICIPANTS: Outpatients at a geriatric psychiatric clinic in New York City. MEASUREMENTS: Demographic and

[Cyclothymia of advanced age (based on data from observations at the geriatric psychiatry office of a general-type polyclinic)].

Cyclothymia was studied in 129 patients aged over 60 in the gerontology unit of an outpatient clinic in Moscow. Premanifest period of the disease was analyzed clinically to single out the functional and somatic disorders. The typology of the first cyclothymic phases and conditions of their development are described as related to the ages of the disease manifestation under 60 vs over 60. The data evidencing the polar nature of the affective disorders are presented to characterize the natural history of the ailment in senile cyclothymic patients.

Case series with late-onset psychosis hospitalized in a geriatric psychiatry unit in Turkey: experience in 9 years.

In this article we report the first series of Turkish inpatients with late-onset psychosis, and describe our 9-year experience at the only inpatient geriatric psychiatry department in Turkey. Among 420 patients hospitalized between 1993 and 2002, 27 were psychotic. In this group, eight patients were diagnosed as having late-onset schizophrenia (LOS) and six very-late-onset schizophrenia-like psychosis (VLOSLP). Five patients had early-onset schizophrenia and eight had delusional disorder. Females were more frequently seen in the group with LOS and the group with VLOSLP. Except for one patient

Short-term aerobic exercise as an adjunct treatment for depression in acute geriatric psychiatry: Results of a randomized controlled trial.

BACKGROUND: This randomized controlled trial examined whether short-term aerobic exercise provided additional clinical benefit over an active control in older inpatients with depression in geriatric psychiatry. METHODS: 100 patients (mean age 76 years) were randomized to 2-week supervised aerobic ergometer training (intervention group, IG) or a flexibility program (control group, CG), both delivered in addition to treatment as usual (TAU). Adherence, training exposure and adverse events were recorded to assess feasibility. The primary outcome was clinical improvement measured with the Clinica

[The right of the client in geriatric psychiatry to a personal, individual world view. How can we, as caregivers, learn to share this?].

"Validation" after Naomi Feil is the name of a method which is used and recognized the world over as an appreciative way of dealing with the highly aged. Validation is based on universally valid humanist principles. A person is viewed as an individual being having a good reason for his current behavior. This is true for "still oriented" as well as for "non-oriented" persons. It is necessary to recognize and categorize the different behavioral patterns in the various phases of disorientation. Here we speak of the physical and psychological characteristics of the four phases of disorientation. T
