Chronic subdural hematoma through the hallmarks of aging: a geroscience framework for the commonest neurosurgical condition of older adults.

Chronic subdural hematoma (CSDH) is among the commonest cranial neurosurgical conditions of older adults and is projected, on the basis of incidence modeling, to become the most common cranial neurosurgical condition in adults by 2030. Despite this clinical centrality, CSDH has not been formally examined through the conceptual lens of geroscience. This minireview reframes CSDH as a geroscience disease by mapping its pathogenesis onto the twelve hallmarks of aging articulated by López-Otín and colleagues. Two hallmarks have strong direct support in the existing CSDH literature. Chronic inflamma

Investigating the combination of plasma amyloid-beta and geroscience biomarkers on the incidence of clinically meaningful cognitive decline in older adults.

We investigated combining a core AD neuropathology measure (plasma amyloid-beta [Aβ] 42/40) with five plasma markers of inflammation, cellular stress, and neurodegeneration to predict cognitive decline. Among 401 participants free of dementia (median [IQR] age, 76 [73-80] years) from the Multidomain Alzheimer Preventive Trial (MAPT), 28 (7.0%) participants developed dementia, and 137 (34.2%) had worsening of clinical dementia rating (CDR) scale over 4 years. In the models utilizing plasma Aβ alone, a tenfold increased risk of incident dementia (nonsignificant) and a fivefold increased risk of

Preserving joint healthspan in knee osteoarthritis: a geroscience-guided review of non-surgical therapies and genicular artery embolization.

Knee osteoarthritis (KOA) is a leading cause of late-life pain, mobility loss, and disability, and is increasingly recognized as a phenotype-dependent manifestation of joint aging rather than a purely mechanical disorder. Cellular senescence, inflammaging, mitochondrial dysfunction, synovial inflammation, vascular remodeling, and impaired repair capacity converge to shape KOA symptoms and progression. This narrative review synthesizes evidence on conservative and minimally invasive KOA therapies within a geroscience-guided, phenotype-informed framework. A structured search of PubMed/MEDLINE an

CT-based prediction of hematoma expansion and adverse outcomes after intracerebral hemorrhage: evidence appraisal, artificial intelligence translation, and GeroScience perspectives.

Spontaneous intracerebral hemorrhage (ICH) is a highly lethal and disabling form of stroke, in which hematoma expansion (HE) is a major and potentially modifiable determinant of early neurological deterioration and poor functional outcome. Computed tomography (CT) remains the first-line imaging modality for acute ICH and provides essential information for early HE risk stratification. However, current evidence is dispersed across conventional CT signs, composite scores, radiomics, machine learning, and deep learning approaches, many of which have been reported descriptively without sufficient

Expanding Geroscience to Vulnerable Populations in Mid-Late Life: Medicaid Home- and Community-Based Service Users and Formerly Incarcerated Individuals.

A wide range of geroscience-guided interventions, or gerotherapeutics, including repurposed drugs, natural products, and lifestyle changes are now being tested in small-scale proof-of-concept studies. If successful, these efforts may help maintain or restore function across numerous health-related domains, thus extending human healthspan. To date, little attention has been paid to exploring the potential of gerotherapeutics to improve healthspan-related outcomes in vulnerable populations that have accumulated experiences detrimental to health in adulthood and later life. We contend that 2 vuln

The INSPIRE Bio-Resource Research Platform for Healthy Aging and Geroscience: Focus on the Human Translational Research Cohort (The INSPIRE-T Cohort).

BACKGROUND: The Geroscience field focuses on the core biological mechanisms of aging, which are involved in the onset of age-related diseases, as well as declines in intrinsic capacity (IC) (body functions) leading to dependency. A better understanding on how to measure the true age of an individual or biological aging is an essential step that may lead to the definition of putative markers capable of predicting healthy aging. OBJECTIVES: The main objective of the INStitute for Prevention healthy agIng and medicine Rejuvenative (INSPIRE) Platform initiative is to build a program for Geroscien

Resveratrol engages SIRT1/AMPK networks to restore BDNF signaling, autophagy, and mitochondrial function in age-related cognitive decline: A geroscience perspective.

Age-related cognitive decline and neurodegenerative disorders present significant challenges in neuropsychiatry, as current therapeutic strategies largely fail to adequately address the complexities of cerebral aging. Resveratrol, a natural polyphenol, has become a promising multi-target neuroprotective agent that can affect multiple important pathways that are involved in aging and neurodegeneration at the same time. This review consolidates evidence indicating that resveratrol, particularly through bioavailability-enhanced formulations, activates SIRT1, stimulates AMPK, inhibits mTORC1, rest

Insights on geroscience pre-clinical and clinical trials to promote healthy aging from the Intrinsic Capacity, Frailty and Sarcopenia Research Task Force 2025.

The Intrinsic Capacity, Frailty and Sarcopenia Research (ICFSR) Task Force convened in March 2025 to discuss geroscience-oriented pre-clinical and clinical trials aimed at promoting healthy aging. Key topics included the use of aging biomarkers in clinical trials, senotherapeutics, metabolism-targeting medications, and therapeutic strategies beyond pharmacological approaches. The discussions highlighted the growing interest in, and accumulating evidence for, geroscience interventions. Several future challenges were identified, including the need for a deeper understanding of the biology of agi

Rationale and protocol for a safety, tolerability and feasibility randomized, parallel group, double-blind, placebo-controlled, pilot study of a novel ketone ester targeting frailty via immunometabolic geroscience mechanisms.

BACKGROUND: Frailty is a geriatric syndrome characterized by chronic inflammation and metabolic insufficiency that creates vulnerability to poor outcomes with aging. We hypothesize that geroscience interventions, which target mechanisms of aging, could ameliorate frailty. Metabolites such as ketone bodies are candidate geroscience interventions, having pleiotropic effects on inflammo-metabolic aging mechanisms. Ketone esters (KEs) induce ketosis without dietary changes, but KEs have not been studied in an older adult population. Our long-term goal is to examine if KEs modulate geroscience mech

We hypothesize that interventions which target common underlying mechanism of aging could ameliorate frailty. Effects of β-hydroxy-β-methylbutyrate (HMB) and/or 2-hydroxybenzylamine (2-HOBA) on markers of healthspan in older adults (H2AGE trial): study protocol of a randomised controlled trial at the Canadian Translational Geroscience Network.

INTRODUCTION: Ageing is associated with progressive declines in muscle mass, cognitive performance, cardiorespiratory fitness and immune function, which together compromise quality of life and increase healthcare burden. Nutritional interventions such as β-hydroxy β-methylbutyrate (HMB) and 2-hydroxybenzylamine (2-HOBA) have shown promise in mitigating muscle atrophy and neuroinflammation, respectively. HMB has demonstrated anabolic effects on skeletal muscle, while 2-HOBA has been shown to reduce oxidative stress and inflammation. Importantly, chronic low-grade inflammation and oxidative stre

Promoting healthy aging in occupational settings: satisfaction with life as a strategic target in the Semmelweis-EUniWell framework.

The growing burden of aging-related disease and functional decline across Europe underscores the urgent need to optimize aging trajectories rather than simply extend lifespan. Achieving this goal requires identifying modifiable determinants that shape biological, psychological, and functional aging across the life course. Satisfaction with life (SWL) is emerging as one such integrative factor, linking adaptive capacity, resilience, and long-term healthspan outcomes. In this conceptual and translational paper, we integrate evidence from geroscience, psychology, and occupational health to positi

Effect of long-term caloric restriction on DNA methylation measures of biological aging in healthy adults from the CALERIE trial.

The geroscience hypothesis proposes that therapy to slow or reverse molecular changes that occur with aging can delay or prevent multiple chronic diseases and extend healthy lifespan1-3. Caloric restriction (CR), defined as lessening caloric intake without depriving essential nutrients4, results in changes in molecular processes that have been associated with aging, including DNA methylation (DNAm)5-7, and is established to increase healthy lifespan in multiple species8,9. Here we report the results of a post hoc analysis of the influence of CR on DNAm measures of aging in blood samples from t

Inflated expectations: the strange craze for translational research on aging : Given existing confusion about the basic science of aging, why the high optimism in the private sector about the prospects of developing anti-aging treatments?

The geroscience research program of the last decade has entailed a shift of focus in research on aging, away from understanding its underlying biology and towards translation into anti-aging treatments—a shift that is premature. [Image: see text]
