Giant Cell Myocarditis in Children: Elusive Giant Cells Might Not Be the Only Clue.

Giant cell myocarditis (GCM) is a form of fulminant myocarditis that is rapidly progressive and frequently lethal even in children. Over the course of 20 years, a definitive histopathologic diagnosis of GCM has been made at our institution in only two pediatric patients, and in neither instance was the diagnosis of GCM rendered on initial cardiac biopsy. We present the two patients and highlight the similarities in their clinical presentation and their challenging and inconclusive- albeit histologically similar- initial cardiac biopsy findings.

Asymptomatic giant cell hepatitis: a subtype of post-infantile giant cell hepatitis?

Giant cell hepatitis in adults is considered a rapidly progressive and life-threatening disease, but there are few descriptions of a prolonged disease course. A 36-year-old Japanese man was referred to our hospital for further evaluation of abnormal liver function test results. Although asymptomatic, he had undergone follow-up for 9 years with these abnormalities. Because the cause of liver injury was not identified despite extensive noninvasive examinations, the patient underwent needle biopsy. He was finally diagnosed with post-infantile giant cell hepatitis (PIGCH) based on the presence of

Rare association between giant-cell aortitis and giant-cell aortic valvulitis.

Giant cell arteritis (GCA) is a type of chronic vasculitis that affects medium and large-caliber arteries, frequently related to aortic involvement and, consequently, to aneurysm formation. However, associated valvulitis with giant cells is uncommon. We describe the case of a 50-year-old female patient with aortic aneurysm and valvular insufficiency, whose anatomopathological examination revealed giant-cell aortic valvulitis associated with giant cell aortitis.

YKL-40 in giant cells and macrophages from patients with giant cell arteritis.

OBJECTIVE: YKL-40, a mammalian member of the family 18 glycosyl hydrolases, is secreted by activated macrophages at a late stage of differentiation. Macrophages are present in inflammation of the arterial wall and are thought to participate in the pathogenesis of giant cell arteritis (GCA). The aim of this study was to evaluate whether macrophages and giant cells of patients with GCA produce YKL-40, and whether serum YKL-40 concentrations are elevated in these patients. METHODS: Serum YKL-40 was determined by radioimmunoassay in 19 patients with GCA and 8 patients with polymyalgia rheumatica

The trophoblast giant cells of cricetid rodents.

Giant cells are a prominent feature of placentation in cricetid rodents. Once thought to be maternal in origin, they are now known to be trophoblast giant cells (TGCs). The large size of cricetid TGCs and their nuclei reflects a high degree of polyploidy. While some TGCs are found at fixed locations, others migrate throughout the placenta and deep into the uterus where they sometimes survive postpartum. Herein, we review the distribution of TGCs in the placenta of cricetids, including our own data from the New World subfamily Sigmodontinae, and attempt a comparison between the TGCs of cricetid

Gallbladder carcinoma with osteoclast-like giant cells.

Extraskeletal tumors containing multinucleated, osteoclast-like giant cells (OGCs) are uncommon. These neoplasms are most frequently reported in the breast and pancreas. Recently, some authors have suggested that carcinomas containing OGCs may represent a distinct clinicopathological entity with a more favorable prognosis. Occurrence in the gallbladder is extremely rare, with only one previous case. We report here on an additional case of gallbladder carcinoma with an infiltrate of OGCs. A 72-year-old woman presented with postprandial abdominal pain and was found to have a mass in the body of

Cardiovascular risk among Giant cells arteritis patients.

OBJECTIVES: Giant cell arteritis (GCA) is a chronic large-vessel vasculitis affecting older adults, associated with significant cardiovascular (CV) complications. Understanding CV risk drivers among GCA, including classical CV risk factors and inflammation, is essential for improved patient management. We (i) assessed the association between GCA and risks of cardiovascular events, including aortic events, major adverse cardiovascular events (MACE), peripheral artery disease (PAD) events, and visceral artery events (ii) assessed the impact of traditional CV risk factors and GCA disease activity

Uterine Leiomyoma With Osteoclast-like Giant Cells.

Numerous histologic variants of uterine leiomyomas have been described. The main interest in recognizing these variants is differentiating them from leiomyosarcoma. Osteoclast-like giant cells (OLGC) have been described in association with leiomyosarcoma but to our knowledge, never with leiomyoma. We here report the case of a 58-year-old woman who underwent an elective total hysterectomy with bilateral salpingo-oophorectomy and bilateral pelvic lymphadenectomy for endometrial atypical complex hyperplasia. Multiple typical uterine leiomyoma were identified. One of them showed numerous OLGC admi

Multinucleated epithelial giant cells in the duodenum.

Multinucleated epithelial giant cells mimicking viropathic effect or dysplasia have been documented in the gastrointestinal tract, specifically in the esophagus and colorectal polyps. This finding has been associated with either inflammation or hyperplasia in prior case series. The authors report the first occurrence in the small intestine involving a 30-year-old woman without a coexisting inflammatory or hyperplastic process. The multinucleated epithelial giant cells featured numerous homogenous nulei (mean 9 nuclei, range 5-18) limited to duodenal villi. Immunohistochemistry revealed strong

Multinucleated stromal giant cells in adolescent gynaecomastia.

Bizzare multinucleated stromal giant cells were observed in a case of adolescent gynaecomastia. Similar cells have been described in the female breast in both benign and malignant conditions where they are thought to originate either from reactive stromal cells or from the mononuclear phagocyte system, respectively. Immunohistochemistry and electron microscopy in this case support the idea that they originate from stromal fibroblasts. Their occurrence in gynaecomastia is further evidence that these changes in stromal cells may be due to hormonal influences.

Malignant meningioma with cartilage and giant cells.

An autopsy case of recurrent and malignant meningioma is reported. This case was originally typical benign transitional meningioma of the falx, however, the histology of the tumor changed to show malignant features during successive recurrences. At autopsy, the tumor revealed findings consistent with malignant meningioma. One of the most interesting features was the presence of cartilage and giant cells in some parts. Immunohistochemistry showed positive immunoreactivity for S-100 protein in some cartilage and giant cells and for cytokeratin in some giant cells. Multidifferential potential of

A Comparative Analysis of p63 Expression in Giant Cell Tumour (GCT), Central Giant Cell Granuloma (CGCG) and Peripheral Giant Cell Granuloma (PGCG).

Giant cell tumour (GCT) is locally aggressive benign neoplasm of long bones whereas giant cell granulomas; central giant cell granuloma (CGCG) and peripheral giant cell granuloma (PGCG); are tumour-like conditions of the oral cavity. This study aimed to evaluate and compare the immunohistochemical expression of p63 in GCT, CGCG, PGCG and determine whether p63 can be used as a diagnostic, prognostic and differential biomarker between these entities. Histopathologically diagnosed 10 cases of GCT, 20 cases of CGCG and 20 cases of PGCG were subjected to p63 immunohistochemical staining. The percen

Giant proteins in a giant cell: Molecular basis of ultrafast Ca2+-dependent cell contraction.

The giant single-celled eukaryote, Spirostomum, exhibits one of the fastest movements in the biological world. This ultrafast contraction is dependent on Ca2+ rather than ATP and therefore differs to the actin-myosin system in muscle. We obtained the high-quality genome of Spirostomum minus from which we identified the key molecular components of its contractile apparatus, including two major Ca2+ binding proteins (Spasmin 1 and 2) and two giant proteins (GSBP1 and GSBP2), which act as the backbone and allow for the binding of hundreds of spasmins. The evidence suggests that the GSBP-spasmin p

Giant Cell Tumor of Bone Versus Tenosynovial Giant Cell Tumor - Similarities and Differences.

Giant cell tumor of bone (GCTB) and tenosynovial giant cell tumor (TGCT) share misleadingly similar names, soft texture and brown color macroscopically, osteoclast-like multinucleated giant cells microscopically and localisation in the musculoskeletal system. However, these two tumor types are biologically and clinically two distinct entities with different natural courses of progression and considerably different modes of surgical and medical treatment. In this article, we provide a detailed update on the similarities and the differences between both tumor types.GCTB is a locally aggressive o

Giant cell reparative granuloma and other giant cell lesions of the bones of the hands and feet.

The present paper is a clinico-pathologic study of 52 osteolytic lesions containing giant cells, affecting the hands and feet and comprising 11 giant cell reparative granulomas, nine giant cell tumors, six chondroblastomas, and 26 aneurysmal bone cysts. The nature of osteolytic lesions of hands and feet must be carefully established in order to plan appropriate treatment without damaging the surrounding structures. Giant cell reparative granuloma has to be distinguished from other lesions containing giant cells. Clinical and radiologic findings proved of limited diagnostic value; the definitiv
