Acute Hemorrhagic Manifestations in a Pregnant Patient With Type 1 von Willebrand Disease: Challenges in Management of Bleeding Disorders During Gestation.

This case reports a primigravida with Type 1 von Willebrand disease experiencing hematuria and gingival bleeding despite prophylaxis. Although pregnancy raises factor levels, her response was insufficient. Desmopressin successfully controlled mucosal hemorrhage, avoiding antifibrinolytics due to obstructive risk. Management requires multidisciplinary surveillance to tailor peripartum hemostatic interventions.

Periodontal considerations for Glanzmann's thrombasthenic patient.

Glanzmann's thrombasthenia (GT) was reported and described as a bleeding diathesis seen in children and characterized by diminished clot retraction. The disorder is caused by a deficiency in the platelet membrane glycoprotein IIb-IIIa complex, with bleeding due to defective platelet hemostatic plug formation. The recurrent features of GT include purpura, epistaxis, gingival hemorrhage, and menorrhagia. GT being an autosomal recessive trait is reported to be especially prevalent in populations where intermarriage is common. Typically, the patients are diagnosed in infancy within the age of five…

Tranexamic acid for management of acute hemorrhage in pediatric patients in the emergency department.

Tranexamic acid (TXA) is an antifibrinolytic medication with multiple routes of topical and systemic administration that can be used to treat acute hemorrhage across a broad range of clinical scenarios. Pediatric data are more limited than the adult literature, which benefits from larger, higher-quality studies; however, TXA remains a potentially effective, safe, and versatile option for temporizing bleeding and limiting blood loss in children. This review discusses the evidence on TXA use in traumatic hemorrhage, traumatic brain injury, posttonsillectomy bleeding, epistaxis, oral and gingival

Idiopathic thrombocytopenic purpura: report of case.

A patient with classic symptoms of blood dyscrasia, specifically idiopathic thrombocytopenic purpura, failed to receive laboratory tests because the condition had gone undetected by a dentist and a physician. However, after the patient was given a provisional diagnosis supported by laboratory test results, one of the general treatment procedures for idiopathic thrombocytopenic purpura that consisted of a regimen of corticosteroids was instituted and was proved successful. More radical procedures, such as transfusions of fresh whole blood or of fresh platelets, or a splenectomy, were unnecessar

Gingival Enlargement During Orthodontic Treatment: A Narrative Review.

Gingival enlargement is commonly seen during orthodontic treatment, specifically in individuals with fixed appliances. Its clinical significance stems from its multifactorial etiology. The purpose of this narrative review is to evaluate the available evidence related to the causes, influential components, symptomatic profiles, and therapeutic approaches to gingival enlargement during orthodontic treatment. An electronic literature research was carried out in PubMed without any limitations. A total of 10 studies were chosen, incorporating one randomized clinical trial and nine observational stu

A bleeding tendency as the first symptom of a choledochal cyst.

We report an 8-month-old male presenting with gingival hemorrhages and nasal bleeding as the first symptom of a choledochal cyst (CC). On physical examination, there was a large cystic mass in the right upper abdominal quadrant. Laboratory studies on admission revealed moderate liver dysfunction and a bleeding tendency due to vitamin K deficiency. After administration of 5 mg vitamin K the bleeding tendency disappeared. At laparotomy, a large CC 5 cm in diameter was found and the liver showed moderate cholestasis. The sudden onset of a bleeding tendency in infants with congenital liver or bili…

Foxtail-induced ulcerative stomatitis outbreak in a Missouri stable.

Twenty of 25 horses in a well-managed Missouri boarding stable were diagnosed with gingivitis/stomatitis. Gross examination of the affected horses revealed varying degrees of gingivitis ranging from mild periodontal swelling to marked swelling and erythema with ulceration and hemorrhage. Fine hair-like material was embedded within the intensely affected areas. Gingival biopsies from 4 affected horses contained pyogranulomatous inflammation with, in some cases, numerous eosinophils and several grass awns in cross and longitudinal section. Numerous foxtail seed heads were identified in hay sampl

Gaucher's disease: case report of mandibular trauma.

Gaucher's disease is a disturbance of lipid storage that results in the accumulation of histiocytes filled with glucosyl ceramide in various organs and bones. Clinical features include history of epistaxis, hemoptysis, and spontaneous gingival hemorrhage. This article reviews Gaucher's disease with reference to its importance to dental practitioners and a case involving mandibular trauma is presented.

Manifestation of acute monocytic leukemia in the oral cavity: a case report.

This case history describes a 26-year-old male with gingival bleeding who presented for care and was treated accordingly. Within a week he developed signs and symptoms of systemic disease and upon further investigation, he was diagnosed with acute monocytic leukemia to which he succumbed within 72 hours. The implications of gingival bleeding are discussed, and the necessity to consider systemic disease in the differential diagnosis is emphasized.

Epithelial and Fibrous Hyperplasia: An Oral Manifestation of Tuberous Sclerosis Complex. A Case Study.

The authors present a case study of a 13-year-old female with a past medical history of tuberous sclerosis complex (TSC), an autosomal dominant disorder. It usually presents with a triad of epilepsy, mental deficiency and facial angiofibromas that are often distributed around the nose, cheek and chin, and are frequently shaped like butterfly wings. In addition, oral manifestations include gingival enlargement and developmental enamel pitting on the facial aspect of the anterior permanent dentition in 50% to 100% of patients. The patient's chief complaint was gingival enlargement and gingival b

Hematologic Parameters and Blood Cultures from the Gingival Vein Compared with the Cranial Vena Cava in Guinea Pigs.

Blood collection methods in guinea pigs are limited due to the animals' compact neck, short limbs, and lack of a tail. Gingival venipuncture is a recently described blood sampling technique that is minimally traumatic with no significant alterations in hematologic parameters when multiple blood samples were collected weekly for 6 wk. The purpose of this study was to determine whether the gingival vein can be used as an alternative blood collection site in guinea pigs, such that: (1) hematologic parameters would be consistent with samples collected from the cranial vena cava; and (2) no contami

Oral manifestations of acute promyelocytic leukemia.

A study was conducted of 16 patients with acute promyelocytic leukemia. The most common oral manifestations were hemorrhagic diathesis and leukemic involvement. There was gingival bleeding, petechiae, and ecchymosis of the oral mucosa, massive infiltration-induced gingival swelling, ulcerative glossitis, swelling of the tonsils, and facial palsy. Inflammatory stomatitis also occurred. Patients with acute promyelocytic leukemia had a higher incidence and severity of oral bleeding than those with other types of acute leukemia. However, the oral findings in our series were not necessarily specifi

Challenges in Differential Diagnosis of Diffuse Gingival Enlargement: Report of Two Representative Cases and Literature Review.

Background/Objectives: The etiology of diffuse gingival enlargement is multifactorial, and the definitive diagnosis may be challenging. To highlight the nuances of the differential diagnosis, we present two cases of generalized gingival overgrowth and discuss the diagnostic dilemmas. Case description: In the first case, an 82-year-old male with a medical history of hypertension and prostatitis had a chief complaint of symptomatic oral lesions of a 20-day duration, accompanied by fever and loss of appetite. The clinical examination revealed diffusely enlarged, hemorrhagic, and focally ulcerativ
