Primary angle-closure glaucoma (PACG) is responsible for half of the glaucoma-related blindness worldwide. Cataract surgery with or without trabeculectomy has been considered to be the first-line treatment in eyes with medically uncontrolled PACG. While minimally invasive glaucoma surgery has become an important surgical approach for primary open-angle glaucoma, its indications and benefits in PACG are less clear. This review summarizes the efficacy and safety profile of minimally invasive glaucoma surgery in PACG to unfold new insights into the surgical management of PACG.

To help differentiate mild angle-closure glaucoma from open-angle glaucoma in patients with elevated intraocular pressure (IOP) and angles so narrow that the structures cannot be seen adequately, we used a thymoxamine hydrochloride eyedrop that induces miosis and tends to widen the angle. Thymoxamine does not contract the ciliary muscle and does not alter the IOP or facility of outflow in open-angle glaucoma, but it can relieve angle-closure glaucoma. A test with thymoxamine was performed in 26 patients because of this differential diagnostic dilemma. When gonioscopic, tonometric, and tonograp

AIM: To estimate the risk of blindness with primary angle-closure glaucoma (PACG) compared to primary open-angle glaucoma (POAG) in those population-based studies that reported blindness rates for both PACG and POAG.
METHOD: A systematic search was performed in PubMed for articles published in English between 2000 and 2020 reporting the prevalence of POAG as well as PACG among various ethnic populations. A study was included if it was (1) population-based (2) had published prevalence and blindness rates for both PACG and POAG in the same cohort. (3) Glaucoma was defined as per the Internation

Glaucoma is the leading cause of irreversible blindness worldwide. It is an ocular disease characterized by an increase in intraocular pressure or, in some cases, normal intraocular pressure, which leads to optic nerve damage and progressive constriction of the visual field (VF). Primary Open-Angle Glaucoma (POAG) and Primary Angle-Closure Glaucoma (PACG) represent the predominant forms of glaucoma. Numerous hypotheses have been posited to elucidate the pathogenic mechanisms underlying these conditions. There is an emerging understanding of the distinct pathological processes that differentiat

To review the literature devoted to the problem of primary anterior chamber angle closure (PAC) and the development of this pathology from glaucoma suspect to primary angle closure glaucoma. The paper includes a trend analysis of the studies concerning primary angle closure suspects (PACS). The concept of this review is conditioned by the conflicting strategies for treating patients with initial PAC without glaucomatous optic neuropathy. Solving the problem of angle closure plays a key role in preventing the development of PAC glaucoma, which is the world's leading cause of irreversible blindn

A new definition and classification system was proposed recently for primary angle closure glaucoma. This system emphasizes on the characteristic progressive end-organ damage - "glaucomatous optic neuropathy", and attempts to adopt a uniform definition of glaucoma, synonymous with primary open angle glaucoma. This approach represents a departure from convention, in which individuals with a narrow drainage angle combine with either raise intraocular pressure or peripheral anterior synechiae but without glaucomatous optic neuropathy, are said to have primary angle closure rather than primary ang

Cataract or clear Lens extraction has been suggested as a treatment option for different spectrums of primary angle closure diseases. It might reduce the risk of progression of angle closure and/or glaucoma by helping to open the angle and control the intraocular pressure (IOP). Conventionally, medically uncontrolled primary angle closure glaucoma was treated with trabeculectomy or phacotrabeculectomy and acute primary angle closure was treated with laser peripheral iridotomy. However, recent randomized controlled trials have demonstrated greater promise of phacoemulsification cataract surgery

The satisfactory control and cure of attacks of primary angle closure glaucoma spans one hundred years following von Graefe's iridectomy in 1856. Gonioscopy was required to validate the theories of shallow anterior chamber and pupillary obstruction, but the gonioscopists looked for the causes of angle closure in their own territory at the angle in the periphery of the anterior chamber and disregarded the center. General agreement of the importance of pupil block was thereby delayed for over ten years. Advances in pharmacology and the invention of laser surgery gave opthalmologists a comfortabl

Primary angle-closure disease (PACD) remains a significant cause of visual morbidity globally, particularly in Asia, where >18.5 million will have primary angle-closure glaucoma (PACG) by 2050. Although glaucomatous optic neuropathy is the most widely recognized cause of visual loss, PACD significantly impacts a range of anterior and posterior segment structures and physiological processes, such as corneal endothelial cell loss, trabecular meshwork structural changes and functional derangement, lens opacities, iris ischemia causing a dilated pupil and consequent degradation in vision, retinal

PURPOSE: Congenital ectropion uvea is a rare anomaly, which is associated with open, but dysplastic iridocorneal angles that cause childhood glaucoma. Herein, we present 3 cases of angle-closure glaucoma in children with congenital ectropion uvea.
OBSERVATIONS: Three children were initially diagnosed with unilateral glaucoma secondary to congenital ectropion uvea at 7, 8 and 13 years of age. The three cases showed 360° of ectropion uvea and iris stromal atrophy in the affected eye. In one case, we have photographic documentation of progression to complete angle closure, which necessitated pla

Primary angle closure glaucoma is one of the common diseases causing blindness. The pathogenesis inducing primary angle closure glaucoma has not been entirely clear. Traditionally identified risk factors include shallow anterior chamber, short axial length and thicker lens. Recent studies begin to pay attention to other new risk factors, not only including static anatomical factors, such as anterior chamber volume, iris curvature and lens vault, but also including dynamic changing factors, such as dynamic dilation of iris volume and choroidal effusion. Comprehensive assessment of these risk fa
