Rapidly progressive glomerulonephritis (RPGN) is characterized by the rapid deterioration of the renal function associated with crescent formation on renal biopsies. This report describes a case of RPGN caused by anti-glomerular basement membrane (GBM) glomerulonephritis in an elderly man with severe thrombocytopenia and a platelet count of 1.4 × 10(4)/µL. Thrombotic microangiopathy (TMA) and heparin-induced thrombocytopenia (HIT) were implicated in the severe decrease in platelets. This report also discusses the pathological background and clinical management of TMA and HIT among patients wit

Anti-glomerular basement membrane (GBM) disease is characterized by the production of an autoantibody with very restricted specificity, with no evidence of polyclonal B cell activation. It was therefore surprising to find that in a solid-phase ELISA a proportion of anti-GBM sera showed significant binding to pyruvate dehydrogenase (PDH), a reactivity usually associated with the antimitochondrial autoantibodies (AMA) found in primary biliary cirrhosis (PBC). The specificity of this reactivity was confirmed by inhibition and competition experiments. The AMA found in anti-GBM sera were of much lo

The characteristic electron microscopic abnormality in Alport's syndrome is thickening of the glomerular basement membrane with splitting and lamellation of the lamina densa. It is commonly accompanied by severe attenuation of the basement membrane and sometimes by disruption of capillary walls. The ultrastructural abnormality is secondary to a more basic defect in the chemical structure of the glomerular basement membrane. Clues to chemical abnormalities have come from immunochemical studies, which suggest a lack of or alteration in that part of the noncollagenous portion of type 4 collagen r

A review was made of the clinical histories, treatment, and renal histologic features of 12 patients with anti-glomerular basement membrane antibody-induced glomerulonephritis (anti-GBM disease). Two patients had milder histopathologic changes in their kidney biopsy specimens and achieved clinical remission. The other ten patients with severe lesions in their biopsy specimens progressed to renal failure. In addition, vascular lesions of thrombotic microangiopathy were present in six of 12 patients. Six patients had clinical laboratory evidence of microangiopathic hemolytic anemia. There are tw

The glomerular basement membrane was subjected to digestion with specific enzymes to determine the chemical nature (sialoglycoproteins, collagenous peptides, or glycosaminoglycans) of the anionic sites previously demonstrated in the laminae rarae. Enzyme digestion was carried out both in situ and in vitro. Kidneys were perfused in situ with enzyme solutions followed by perfusion with fixative containing the cationic dye, ruthenium red, to detect the anionic sites. Glomerular basement membranes were isolated by detergent treatment of glomeruli and incubated with enzyme solutions, followed by in

To determine the specificity and significance of widespread attenuation of the glomerular basement membrane on electron microscopy, 240 renal biopsies from 218 children were studied retrospectively. Twenty-three patients showed diffuse attenuation and three of them are cases of hereditary nephritis. The other 20 patients are characterized by persistent microscopic haematuria, absence of proteinuria, normal blood pressure and renal function, and minimal glomerular changes. In 10 of these 20 children, microscopic haematuria was also present in the family. We conclude that widespread attenuation

Diabetic nephropathy (DN) is a leading cause of chronic kidney disease and end-stage renal disease worldwide, characterized by progressive injury across all renal compartments. Traditionally considered a glomerular disorder marked by basement membrane thickening, mesangial expansion, and nodular sclerosis, DN is now recognized as a multifaceted process involving tubulointerstitial, vascular, and podocyte pathology. This narrative review integrates current histopathological insights, highlighting key glomerular lesions, tubular basement membrane thickening, interstitial fibrosis, arteriolar hya

During the heterologous phase of experimental anti-glomerular basement membrane (anti-GBM) nephritis, leukocyte influx peaks within hours, whereas albuminuria occurs within 1 day. In the subsequent autologous phase, endogenous anti-GBM IgG develops and albuminuria persists. Heparan sulfate (HS) proteoglycans like syndecan-1 play multiple roles during inflammation and we evaluate its role in experimental anti-GBM disease using syndecan-1 knockout (sdc-1-/-) mice. During the heterologous phase, glomerular leukocyte/macrophage influx was significantly higher in the sdc-1-/- mice and this was asso

Two Indian male children with infantile-onset heavy proteinuria (with nephrotic syndrome in 1) had thickening of the glomerular basement membrane with splitting and basket-weave appearance of lamina densa on electron microscopic evaluation of kidney tissue (like Alport's syndrome), with normal light microscopic findings and negative immunofluorescence. The proteinuria was non-familial and was not associated with microhaematuria in patient 1; transient microhaematuria, perhaps associated with urinary tract infection, was noted in patient 2. There was no neurosensory deafness in the patients or

Many reports confirm that thin basement membrane nephropathy (TBMN) commonly occurs together with other glomerular diseases such as minimal change glomerulonephritis, diabetes, membranous nephropathy, immunoglobulin (Ig)A glomerulonephritis, and focal segmental glomerulosclerosis. We postulate 3 explanations for these observations. The association of minimal change glomerulonephritis with TBMN probably is artifactual whereas the association with diabetes and membranous glomerulonephritis probably is coincidental. However, the link between TBMN and IgA disease and focal segmental glomeruloscler

Using indirect immunofluorescence, cryostat sections from renal biopsy specimens of 14 adult patients showing marked diffuse thinning of the glomerular basement membrane (GBM) on ultrastructural analysis were examined for the presence of the Goodpasture (GP) epitope M2 using anti-M2 antiserum. In no case was a total absence of M2 noted. The fluorescence pattern was fine but homogeneously linear along the GBM in 12 cases, intensity varying from +-++, as compared with for the control specimen GBM. A faint, broken line of stain, intensity+, was observed in biopsy specimens of two patients, one of

Hereditary nephritis (HN) is associated with antigenically abnormal glomerular basement membrane (GBM) manifest by reduced or absent binding of MCA-P1, a mouse monoclonal antibody which recognizes Goodpasture antigen. In the present studies, immunoblotting has been used to analyze antigenic and biochemical composition of renal tissue from patients with HN in whom binding of MCA-P1 could not be demonstrated by indirect immunofluorescence (IIF). Pooled normal collagenase-solubilized GBM (CS-GBM) and CS-GBM from three patients with either end-stage renal failure (ESK1-3) or HN (HNK1-3), were exam

Measurements of glomerular basement membrane (GBM) thickness in children with minimal change nephrotic syndrome were made on electron micrographs of 43 biopsy specimens obtained from 35 children aged 1-13 years, of whom 13 were in remission at the time of biopsy. Measurements were also made in 2 children with non-glomerular disorders. A mean of 189 measurements per biopsy were made, using prints of x6,400-12,000 magnification. Quadratic curves were fitted by regression analysis to the mean GBM thickness for each of the 35 initial nephrotic biopsies, plotted against age for the whole group and

Wegener's granulomatosis and Goodpasture's syndrome represent two major causes of a pulmonary-renal syndrome. We describe the clinical course and morphologic features of a patient in whom pulmonary manifestations of Wegener's granulomatosis developed and were followed six months later by anti-glomerular basement membrane disease. Although we regard this as a unique and probably fortuitous association, a genetic predisposition or a secondary form of anti-GBM disease cannot be excluded.

The distribution of human IgG subclasses among the glomerular deposits of 53 patients with glomerulonephritis was examined by immunofluorescence (IF) with subclass-specific monoclonal antibodies (Mab). A subclass restriction was observed in idiopathic membranous nephropathy (MN) with glomerular deposits predominantly containing IgG4 (81% of the studied biopsies) and IgG1 (75%). In de novo MN, occurring after transplantation, the restriction was markedly different, with a predominance of IgG1 (100%) and IgG2 (69%). In anti-glomerular basement membrane (a-GBM) nephritis the restriction was consi

We have previously shown that lymphocytes from idiopathic minimal-lesion nephrotic patients produce a lymphokine (supernatant factor) that increases the 35sulfate uptake in the glomerular basement membrane (GBM). The purpose of this report was to further characterize the supernatant factor by studying the effects of interleukins (IL) 2-4, 6, and 8, granulocyte-macrophage colony stimulating factor, and tumor necrosis factor on the 35sulfate incorporation by rat glomeruli in vitro. A significant increase in GBM 35sulfate uptake was only seen when the glomeruli were cultured with the addition of

We investigated nephritogenic potential of antibodies to heparan sulfate-proteoglycan of glomerular basement membrane. Glomeruli were isolated, basement membranes were prepared, proteoglycans extracted, and purified core protein was obtained. We immunized rabbits with the core protein, IgG fraction prepared from the antisera and specificity of the antibody determined. A single immunoprecipitin line in agar diffusion plate and a single band (approximately 18,000 mol wt) on the immunoblot autoradiograms were visualized. The antibody showed precise reactivity with the glomerular basement membrane
