Eight patients with drug intoxication were hemoperfused on ten occasions at a blood-flow rate of 300 ml/min with a 650gm column of Amberlite XAD-4 resin, which is a macroreticular resin with a specific adsorptive attraction for lipid-soluble organic molecules. Column clearances of glutethimide and a variety of barbiturates ranged from 207 to 300 ml/min for treatment sessions extending from 2 1/2 to ten hours. After ingestion of 75 gm of glutethimide, one patient received hemoperfusion on three successive days for nine, ten, and eight hours, respectively. She recovered after the column removed

We examined the efficacy of a new, fixed-bed, uncoated charcoal device in experimentally intoxicated dogs and in drug-intoxicated as well as chemically poisoned patients. In the animal studies, 4 h of hemoperfusion resulted in a significant decrease in the blood level of phenobarbital, salicylate, pentobarbital and glutethimide. The drug clearances varied between 97 +/- 10 and 129 +/- 6 ml/min. Suicidal attempt with old (currently unused) drug is described. The suicidal attempts are usually performed with the use of contemporary pharmacotherapeutics. In the report a case of suicidal attempt with old drug Tardyl is presented. Tardyl (Glutethimid) was prescribed to the patient and has been stored for 20 years. Thalidomide was given to 15 patients with severe orogenital ulceration (OGU). 4 patients underwent a double-blind controlled trial with glutethimide as placebo and 11 were treated openly. Treatment with thalidomide produced complete resolution of ulcers in 14 and significant improvement in the remaining patient. No peripheral neuropathies developed. Patients did not respond to glutethimide. Thalidomide is an effective treatment for severe OGU. Adequate contraceptive measures should be taken during treatment.

The intensity, uniformity and time course of anticoagulant interference by phenobarbital, secobarbital, glutethimide, chloral hydrate and methaqualone were systematically investigated in 16 patients receiving coumarin therapy. The clinical use of uncoated charcoal haemoperfusion systems, despite their efficacy, has hitherto been prevented by the occurrence of a number of adverse effects including charcoal embolism and marked thrombocytopenia. Charcoal coated with a synthetic hydrogel overcomes many of the disadvantages associated with the use of uncoated material in that there is a much reduced thrombocytopenia and no evidence of charcoal embolism. The potential thyrotoxic effect of SK&F 93479 (an H2-receptor antagonist), aminoglutethimide (AG) and glutethimide were studied by examining their effects on 125iodide uptake and organification (incorporation into thyroglobulin) in cultured thyroid cells from the pig and rat. Few studies of prescription-drug misuse have taken into account the numbers of prescriptions dispensed for specific drugs. A rapid gas-chromatographic method has been developed for the determination of butobarbitone, amylobarbitone, hexabarbitone, pentobarbitone, quinalbarbitone, phenobarbitone glutethimide, and methaqualone in ;finger-prick' samples of plasma. This has been applied to the analysis of some of these drugs in plasma taken from patients after therapeutic dosage and over-dosage.

A gas-liquid chromatographic method has been devised whereby both the qualitative and the quantitative analysis of a number of hypnotic drugs in the blood may be accomplished simultaneously. Small (50-mul) plasma aliquots are required. The technique is rapid and will detect 2 mg/l concentrations of these compounds. The barbiturates glutethimide, methaqualone, and meprobamate are the principal drugs covered by the scheme. After overdose the concentrations that occur in man are at, or above, this minimum level of detection.

This report discusses characteristics of a custom-designed vidicon spectrometer and evaluates its applicability to several clinical analysis problems. Dogs were given large doses of barbiturates, glutethimide, ethanol, methaqualone, ethchlorvynol, meprobamate, chloral hydrate, paracetamol and aspirin. Eleven patients who had taken overdoses of barbiturates, glutethimide, tricyclic antidepressants, and chloroquine were treated by resin haemoperfusion using an R-004 haemoperfusion cartridge containing XAD-4 resin. Our previously described HPLC method for the simultaneous analysis of several antiarrhythmic drugs and metabolites in serum [Clin. Biochem. 14. 113-118 (1981)] was correlated with an enzyme immunoassay technique (EMIT) for quinidine, procainamide, N-acetyl procainamide, and disopyramide.
