Mineralocorticoid receptor-antagonism prevents COVID-19-dependent glycocalyx damage.

Proinflammatory cytokines target vascular endothelial cells during COVID-19 infections. In particular, the endothelial glycocalyx (eGC), a proteoglycan-rich layer on top of endothelial cells, was identified as a vulnerable, vasoprotective structure during infections. Thus, eGC damage can be seen as a hallmark in the development of endothelial dysfunction and inflammatory processes. Using sera derived from patients suffering from COVID-19, we could demonstrate that the eGC became progressively worse in relation to disease severity (mild vs severe course) and in correlation to IL-6 levels. This

Plant heteropolysaccharides and their bioadhesion to animal glycocalyx visualized at the nanoscale.

The plant-derived biopolymer pectin has been explored as sealant in lung surgery. Interpenetration of pectin's heteropolysaccharide chains with sugar-chains on components of the mesothelial glycocalyx is thought to mediate adhesion. To visualize a potential adhesion complex, we first establish a method to preserve pectin ultrastructure by limiting hydration during sample preparation for electron microscopy. Depending on actual hydration states and pectic domains selected we find distinct morphologies: Citrus pectic polysaccharides, combining both, smooth homogalacturonan and hairy rhamnogalact

Degradation of endothelial glycocalyx in Tanzanian children with falciparum malaria.

A layer of glycocalyx covers the vascular endothelium serving important protective and homeostatic functions. The objective of this study was to determine if breakdown of the endothelial glycocalyx (eGC) occurs during malaria infection in children. Measures of eGC integrity, endothelial activation, and microvascular reactivity were prospectively evaluated in 146 children: 44 with moderately severe malaria (MSM), 42 with severe malaria (SM), and 60 healthy controls (HC). Biochemical measures of eGC integrity included plasma syndecan-1 and total urinary glycosaminoglycans (GAG). Side-stream dark

Endothelial Dysfunction in Fabry Disease Is Related to Glycocalyx Degradation.

Fabry disease (FD) is an X-linked multisystemic lysosomal storage disease due to a deficiency of α-galactosidase A (GLA/AGAL). Progressive cellular accumulation of the AGAL substrate globotriaosylceramide (Gb3) leads to endothelial dysfunction. Here, we analyzed endothelial function in vivo and in vitro in an AGAL-deficient genetic background to identify the processes underlying this small vessel disease. Arterial stiffness and endothelial function was prospectively measured in five males carrying GLA variants (control) and 22 FD patients under therapy. AGAL-deficient endothelial cells (EA.hy9

Glycocalyx in Atherosclerosis-Relevant Endothelium Function and as a Therapeutic Target.

PURPOSE OF REVIEW: The cell surface-attached extracellular glycocalyx (GCX) layer is a major contributor to endothelial cell (EC) function and EC-dependent vascular health and is a first line of defense against vascular diseases including atherosclerosis. Here, we highlight our findings regarding three GCX-dependent EC functions, which are altered when GCX is shed and in atherosclerosis. We discuss why the GCX is a viable option for the prevention and treatment of atherosclerosis. RECENT FINDINGS: GCX regulated EC activities such as barrier and filtration function, active cell-to-cell communi

Congenital disorders of glycosylation: a concise chart of glycocalyx dysfunction.

Glycosylation is a ubiquitous modification of lipids and proteins. Despite the essential contribution of glycoconjugates to the viability of all living organisms, diseases of glycosylation in humans have only been identified over the past few decades. The recent development of next-generation DNA sequencing techniques has accelerated the pace of discovery of novel glycosylation defects. The description of multiple mutations across glycosylation pathways not only revealed tremendous diversity in functional impairments, but also pointed to phenotypic similarities, emphasizing the interconnected

Exfoliative epitheliopathy of bullous keratopathy with breaches in the MUC16 Glycocalyx.

PURPOSE: Expression of cellular adhesion molecules is altered in bullous keratopathy. The hypothesis that epithelial alterations in bullous keratopathy compromise the surface of the cornea and its glycocalyx was tested. METHODS: Studies were performed on eight cases each of pseudophakic bullous keratopathy and healthy corneas. The number of epithelial cell layers was determined with a stereological method of point counting. The minimum distance between points was established by estimates of cell size with variable pressure scanning electron microscopy performed in backscatter mode. The mean n

Secondary thrombotic microangiopathy in children-a central role for the glycocalyx.

Thrombotic microangiopathy (TMA) is classically defined by the triad of hemolytic anemia, thrombocytopenia, and organ injury, most notably kidney involvement. Secondary TMA, which arises in association with specific diseases or external triggers, is more common than primary or complement-mediated TMA. Current evidence supports an important role for complement activation in the pathogenesis of secondary TMA, and emerging data highlight the contribution of the endothelial glycocalyx to complement dysregulation at the endothelial surface. The endothelial glycocalyx is an integrated matrix lining

Plasma for prevention and treatment of glycocalyx degradation in trauma and sepsis.

The endothelial glycocalyx, a gel-like layer that lines the luminal surface of blood vessels, is composed of proteoglycans, glycoproteins, and glycosaminoglycans. The endothelial glycocalyx plays an essential role in vascular homeostasis, and its degradation in trauma and sepsis can lead to microvascular dysfunction and organ injury. While there are no proven therapies for preventing or treating endothelial glycocalyx degradation, some initial literature suggests that plasma may have a therapeutic role in trauma and sepsis patients. Overall, the literature suggesting the use of plasma as a the

Vascular Dysfunction in Malaria: Understanding the Role of the Endothelial Glycocalyx.

Malaria caused by Plasmodium falciparum results in over 400,000 deaths annually, predominantly affecting African children. In addition, non-falciparum species including vivax and knowlesi cause significant morbidity and mortality. Vascular dysfunction is a key feature in malaria pathogenesis leading to impaired blood perfusion, vascular obstruction, and tissue hypoxia. Contributing factors include adhesion of infected RBC to endothelium, endothelial activation, and reduced nitric oxide formation. Endothelial glycocalyx (eGC) protects the vasculature by maintaining vessel integrity and regulati

Hydrocortisone preserves the vascular barrier by protecting the endothelial glycocalyx.

BACKGROUND: Hydrocortisone protects against ischemia-reperfusion injury, reduces paracellular permeability for macromolecules, and is routinely applied in the prevention of interstitial edema. Healthy vascular endothelium is coated by the endothelial glycocalyx, diminution of which increases capillary permeability, suggesting that the glycocalyx is a target for hydrocortisone action. METHODS: Isolated guinea pig hearts were perfused with Krebs-Henseleit buffer. Hydrocortisone was applied in a stress dose (10 microg/ml) before inducing 20 min of ischemia (37 degrees C). Hearts were reperfused

Glycocalyx degradation: exploring related mechanisms, pathophysiological significance, and therapeutic prospects.

The glycocalyx is a fuzzy structure covering the luminal surface of vascular endothelial cells. Its name was first proposed by Bennett, derived from the Latin word meaning ‘sweet shell’. This was confirmed by Luft in 1966 through ruthenium red staining under electron microscopy. In recent years, the role of the glycocalyx in the pathophysiological processes of various diseases, such as cardiovascular diseases, chronic kidney disease, sepsis, COVID-19 infection, and diabetes, has gradually attracted widespread attention, and its importance has been increasingly recognized. In these acute and ch

Endothelial glycocalyx in perioperative medicine current understanding and future direction.

The endothelial glycocalyx (EG) is a dynamic, gel-like layer that lines the luminal surface of blood vessels, playing a crucial role in vascular biology. Previously considered a passive barrier, it is now recognized as a key regulator of vascular tone, permeability, inflammation, and coagulation. Composed mainly of proteoglycans, glycoproteins, and glycosaminoglycans, the EG serves as a semipermeable interface between blood and the endothelium, maintaining microvascular flow and modulating nitric oxide (NO) production while protecting against oxidative and inflammatory damage. This review high

Glycocalyx dysregulation impairs blood-brain barrier in ageing and disease.

The blood-brain barrier (BBB) is highly specialized to protect the brain from harmful circulating factors in the blood and maintain brain homeostasis1,2. The brain endothelial glycocalyx layer, a carbohydrate-rich meshwork composed primarily of proteoglycans, glycoproteins and glycolipids that coats the BBB lumen, is a key structural component of the BBB3,4. This layer forms the first interface between the blood and brain vasculature, yet little is known about its composition and roles in supporting BBB function in homeostatic and diseased states. Here we find that the brain endothelial glycoc

Glycocalyx Interactions Modulate the Cellular Uptake of Albumin-Coated Nanoparticles.

Albumin-based nanoparticles (ABNPs) represent promising drug carriers in nanomedicine due to their versatility and biocompatibility, but optimizing their effectiveness in drug delivery requires understanding their interactions with and uptake by cells. Notably, albumin interacts with the cellular glycocalyx, a phenomenon particularly studied in endothelial cells. This observation suggests that the glycocalyx could modulate ABNP uptake and therapeutic efficacy, although this possibility remains unrecognized. In this study, we elucidate the critical role of the glycocalyx in the cellular uptake

Mass intraoperative endothelial glycocalyx shedding affects postoperative systemic inflammation response.

BACGROUND: Off-pump coronary artery bypass graft (OPCABG) has a high incidence of postoperative systemic inflammation response syndrome (SIRS), and perioperative endothelial glycocalyx layer (EGL) disruption can be one of the predisposing factors. We hypothesized that EGL shedding happened earlier in OPCABG which can influence on postoperative SIRS, and sevoflurane might preserve EGL better than propofol. METHODS: We randomly allocated 50 patients undergoing OPCABG to receive either sevoflurane-sufentanil or propofol-sufentanil anesthesia. Plasma syndecan-1, heparan sulfate (HS), atrial natri

ENDOTHELIAL GLYCOCALYX SHEDDING IN INTRA-ABDOMINAL SEPSIS: A FEASIBILITY STUDY.

Background: The endothelial glycocalyx layer (EGL) is a complex meshwork of glycosaminoglycans and proteoglycans that protect the vascular endothelium. Cleavage or shedding of EGL-specific biomarkers, such as hyaluronic acid (HA) and syndecan-1 (SDC-1, CD138) in plasma, have been shown to be associated with poor clinical outcomes. However, it is unclear whether levels of circulating EGL biomarkers are representative of the EGL injury within the tissues. The objective of the present feasibility study was to describe a pathway for plasma and tissue procurement to quantify EGL components in a coh
