Reassessment of H-reflex recovery curve using the double stimulation procedure.

We conducted two types of experiments to assess the validity of the H-reflex recovery test, using double stimulation to test soleus motoneuron pool excitability in healthy and spastic subjects. One type dealt with the mechanical effect of the conditioning H reflex on the ankle joint; the other type with the effect of change in reflex size. The mechanical effect was tested both with the ankle joint fixed (FX) and free to move (FR). Differences between FX and FR conditions commenced with relaxation of soleus muscle contraction by the conditioning H reflex. In the FR condition, abrupt facilitatio

H-reflex and F-wave potentials in leg and arm muscles.

The systematic analyses of secondary muscle potentials of H-reflex and F-wave type were done in multicentric study. The examinations were carried out in healthy volunteers with 9 muscles analysed on the legs and 9 on the lower arms and hands. The H-reflex potential was found regularly in thigh muscles (vastus medialis 100%, biceps femoris 97%, semitendinosus 93%). Less frequently but still with high incidence it appeared in posterior lower leg muscles (soleus 93%, caput mediale gastrocnemii 73%). In anterior tibial muscle and extensor digitorum brevis it did not appear at all. Only exceptional

[Effect of electric stimulation of auricular acupuncture points on H-reflex].

The non-nociceptive H-reflex in the musculus soleus were recorded before and after auricular electroacupuncture (AEAP) in patients with vertebrogenic pain syndrome. Changes of the H-reflex on the side of the pain syndrome were observed after ipsi- and contralateral AEAP, the latter producing effect more often. Changes of H-reflex are mainly of facilitating character (71.4%) and, rarely, of inhibitory character (14.3%). In the part of patients AEAP produced distinct facilitation of M-response.

H-reflex study in normal children and patients with cerebral palsy.

The H-reflex was studied in 53 children with cerebral palsy, and the results were compared with those obtained for 56 normal control subjects. Pairs of identical stimuli were delivered and the time course of recovery of the amplitude of the H-reflex was determined. Recovery of the H-reflex was increased in the normal control group aged 0 to 12 mos compared to that in the normal control group aged 1 to 9 yrs, especially at interstimulus intervals from 100 to 800 msec. In children with spasticity, marked recoveries of the H-reflex were observed in both age groups, 0 to 12 mos and 1 to 9 yrs, com

Responsiveness of the H reflex to loading and posture in patients following stroke.

The objective of the research was to examine the effects of loading and posture on motoneuronal excitability of the triceps surae (TS) for patients with hemiplegia. Twelve healthy subjects and 12 patient subjects with post-stroke hemiparesis (onset period: 3-60 months) were enrolled in this study. The subjects were instructed to remain in quiet sitting with the test knee straight and three standing conditions of different superincumbent loads by shifting body weight to the test leg (10%, 50%, and 90% of body weight), while the H reflexes and M waves of the TS were measured. The results clearly

Facilitation of the spinal H-reflex by auditory stimulation in dyslexic adults.

Dyslexic subjects show a variety of mild sensory and motor deficits that have been assumed to reflect dysfunction of the large-diameter 'magnocells' in different parts of the brain. Hearing as a warning sense relies on rapidly-conducting fibers, and on the basis of the magnocellular deficit theory, we wondered whether auditory alerting would be weakened in dyslexic adults. We quantified the strength of sound-induced spinal facilitation in seven dyslexic and eight normal-reading adults by measuring the amplitudes of H-reflex, a monosynaptic spinal reflex, after loud binaural sounds. The audiosp

Pentobarbital-induced modulation of flexor and H-reflexes in spinal rats.

Electrophysiological recordings of the H-reflex and nonnociceptive flexion reflex were obtained from pentobarbital-anesthetized Intact rats and from both, anesthetized and unanesthetized groups of Acute and Chronic Spinal rats. Results showed that the flexor, but not H-reflex, of Chronic Spinal rats was significantly larger than that of all other groups, which did not differ among themselves. The antispastic drug baclofen dose-dependently decreased the flexion response of Chronic Spinal rats (A(50)=4.3 mg/kg+/-2.1 and 9.0 mg/kg).

Recruitment curve of the soleus H reflex in patients with neurogenic claudication.

The diagnosis of neurogenic claudication (NC) remains uncertain when no definite signs of radicular lesions are found in electrophysiological testing. However, the functional deficit could be demonstrated during the brief time in which the patients complain of pain and weakness in the muscles of the lower limbs after walking. We have used electrophysiological testing of the H reflex and the F wave to document the transient functional derangement expected to occur at the radicular level in patients with NC after walking. We examined the recruitment curve of the soleus H reflex, and the chronodi

On the mechanism of the post-activation depression of the H-reflex in human subjects.

It was demonstrated that the soleus H-reflex was depressed for more than 10 s following a preceding passive dorsiflexion of the ankle joint. This depression was caused by activation of large-diameter afferents with receptors located in the leg muscles, as an ischaemic block of large-diameter fibres just below the knee joint abolished the depression, whereas a similar block just proximal to the ankle joint was ineffective. The depression of the H-reflex was not caused by changes in motoneuronal excitability, as motor-evoked potentials by magnetic brain stimulation were not depressed by the same

Proximally evoked soleus H reflexes in the evaluation of axonal neuropathy.

It is sometimes difficult to differentiate physiologically distal axonopathy from proximal root or nerve disease. Standard soleus H reflexes evoked by distal tibial nerve stimulation at the popliteal fossa are often absent in both syndromes. In this study we distinguish the two by the presence or absence of soleus H reflexes evoked by proximal sciatic nerve stimulation at the gluteal fold. In 12 normal subjects maximum H reflex amplitudes evoked by distal tibial and proximal sciatic stimulation were essentially equal. In 12 patients with suspected distal polyneuropathy, proximal but not distal

[Diagnostic significance of the H-reflex in traumatic lesions of the spinal cord].

The study of H-reflexes, their latent periods and M-responses involved 69 patients with trauma to the spinal cord. The results showed that these parameters were normal in 39 patients. The remaining patients lacked H- or M-responses, which was indicative of inhibition of neural and muscular activity due to disorders of hemodynamic and metabolic processes in the spinal cord or in muscles of the lower extremities. The study of latent periods and the speed of impulse conduction along the peripheral nerve revealed that after an injury to the spinal cord some patients presented impairment of impulse

Novel human models for elucidating mechanisms of rate-sensitive H-reflex depression.

BACKGROUND: This study used novel human neurophysiologic models to investigate whether the mechanism of rate-sensitive H-reflex depression lies in the pre-synaptic or post-synaptic locus in humans. We hypothesized that pre-synaptic inhibition would suppress Ia afferents and H-reflexes without suppressing alpha motor neurons or motor evoked potentials (MEPs). In contrast, post-synaptic inhibition would suppress alpha motor neurons, thereby reducing H-reflexes and MEPs. METHODS: We recruited 23 healthy adults with typical rate-sensitive H-reflex depression, 2 participants with acute sensory-imp

H-reflex to S1-root stimulation improves utility for diagnosing S1 radiculopathy.

OBJECTIVE: The H-reflex on stimulation of the tibial nerve in the popliteal fossa is routinely used in the diagnosis of first sacral (S1) nerve-root radiculopathy. The H-reflex latency, however, is considered to lack sensitivity since a small change from the focal root pathology can be diluted in a relatively long reflex latency. We have studied the soleus H-reflex elicited by stimulation of the S1 nerve root at the S1 foramen. The normal values for the S1-foramen H-reflex have been reported in a previous study, but there are no definitive reports in patients with S1 radiculopathy. This study

Utility of flexor carpi radialis h-reflex in diagnosis of cervical radiculopathy.

PURPOSE: The sixth and seventh roots are among the most common roots involved in cervical radiculopathy. The aim of this study was to evaluate the diagnostic value of flexor carpi radialis H-reflex in patients with suspected C6 or C7 root lesion. METHODS: This study was performed on 42 controls and 38 patients with suspected C6 or C7 radiculopathy. All patients were evaluated by physical examination, electromyogram, nerve conduction study and flexor carpi radialis H-reflex (superficial technique), and cervical magnetic resonance imaging. Effects of test H-reflex size on reciprocal inhibition in forearm muscles.

We studied effects of test H-reflex size on reciprocal Ia inhibition in forearm muscles. In both healthy control subjects and hemiplegic patients, the amount of Ia inhibition decreased as the test H-reflex size increased. It is possible that forearm reciprocal Ia inhibition in hemiplegics reported previously might be underestimated due to larger test H-reflexes used in the hemiplegics than in the controls.

Soleus H-reflex excitability during standing with stable and unstable light touch.

Light touch reduces sway when participants stand with their eyes closed. Unexpected displacement of the touch reference induces a "false-positive" balance correction in most participants. However, this response is only observed with the first displacement, with subsequent displacements eliciting an arm-extension response. This suggests that the sensorimotor processes involved in stabilizing sway from light touch are rapidly adapted within a single trial. We hypothesized that this change in sensorimotor behaviour would be reflected in changes in the excitability of the soleus H-reflex. Soleus H

PAK1 inhibition with Romidepsin attenuates H-reflex hyperexcitability after spinal cord injury.

Hyperreflexia associated with spasticity is a prevalent neurological condition characterized by excessive and exaggerated reflex responses to stimuli. Hyperreflexia can be caused by several diseases including multiple sclerosis, stroke and spinal cord injury (SCI). Although we have previously identified the contribution of the RAC1-PAK1 pathway underlying spinal hyperreflexia with SCI-induced spasticity, a feasible druggable target has not been validated. To assess the utility of targeting PAK1 to attenuate H-reflex hyperexcitability, we administered Romidepsin, a clinically available PAK1 inh
