[Dermatitis herpetiformis (author's transl)].

Detailed investigations of 9 patients with dermatitis herpetiformis are presented. In all cases cutaneous lesions were controlled by dapsone alone or by dapsone and gluten free diet. Granular IgA deposits were found in 7 patients, linear IgA deposits in one, and C3 component of complement in one. 3 patients out of 8 tested, carried the specific HLA-B8 antigen. Despite an extensive investigation, no malabsorption was detected. Jejunal biopsies were performed in 8 cases. Jejunal villous flattening was observed in one patient. It improved after a 2 months gluten free diet on subsequent jejunal bi

Recurrent erythema multiforme: tissue typing in a large series of patients.

Previous reports have shown an increased frequency of certain HLA antigens in association with erythema multiforme, including HLA-B15(B62), HLA-B35, HLA-A33, HLA-DR53 and, more recently, HLA-DQB1*0301. A strong association with HLA-DQ3 has been documented in patients with recurrent erythema multiforme. We have performed HLA typing in 39 patients with recurrent erythema multiforme, of whom 33 were associated with herpes simplex virus infection. The results were compared with 309 controls. In the recurrent erythema multiforme patients there was a statistically significant increase in HLA-B62 and

HLA association of idiopathic Peyronie's disease: an indication of autoimmune phenomena in etiopathogenesis?

HLA typing for class I and class II antigens was done in 52 unrelated patients suffering from idiopathic Peyronie's disease. The controversially discussed association with the HLA-B7 cross-reacting group could not be confirmed. Marked deviations of antigen frequencies were observed for HLA-A1, B8, Cw7, DR3 and DQw2 compared to healthy local controls. After correction of p-values, A1 (pc less than 0.05) and DQw2 (pc less than 0.01) remained significant. A possible association of Peyronie's disease with markers of the HLA-A1, B8, Cw7, DR3, DQw2 haplotype, as first described here, would suggest a

Frequencies of HLA-DR3, -DR4, -B8 and -Bw62 in diabetic children diagnosed between 1960 and 1990.

An analysis of the HLA-types of 351 children in whom a diagnosis of insulin dependent diabetes mellitus (IDDM) had been made between 1960 and 1990 revealed that although the frequencies of HLA-DR3, -DR4, -DR3/4, -B8 and -Bw62 were increased there was, depending on the year of diagnosis, a marked fluctuation in the frequencies of these HLA-antigens and in the frequency with which -B8 was associated with -DR3 and -Bw62 with -DR4 suggesting heterogeneity/variation in agents initiating/triggering IDDM.

Graves' disease with ophthalmopathy following radiotherapy for Hodgkin's disease.

The number of patients achieving long-term survival following neck irradiation for Hodgkin's disease and other malignancies is increasing. Paralleling this increase in survivors is the development of late complications of the therapy itself. Eleven patients have previously been reported who developed Graves' ophthalmopathy 18 months to seven years after receiving neck radiotherapy for nonthyroidal malignancies. The seven patients who had HLA typing were all HLA-B8 negative, despite the reported association of the HLA-B8 antigen with Graves' disease. A patient who is HLA-B8 positive who develop

Hidradenitis suppurativa. Glucose tolerance, clinical, microbiologic, and immunologic features and HLA frequencies in 27 patients.

Glucose tolerance, lymphocyte populations, and HLA types were studied in 27 patients with untreated hidradenitis suppurativa; 18 of these patients had a negative history for acne vulgaris. Six patients (22%) had an increased incidence of impaired glucose tolerance. We failed to confirm a previous report of high incidence of flat glucose tolerance curves. The frequency of HLA-A and HLA-B antigen loci and the median values of peripheral blood T lymphocytes were similar to the control population. However, seven patients with moderate or severe clinical disease had a marked reduction in T lymphocy

Immunogenetic markers in patients with Graves' disease.

110 carefully characterized Caucasoid patients with Graves' disease were tested for HLA class I and class II antigens. Compared with Caucasian controls (n = 193), the frequencies of HLA B8, Cw7 and DR3 were significantly increased (pc less than 0.05). In the subgroups with and without exophthalmos, HLA A3 exhibited a negative but insignificant association with the eye involvement, while A19 and Cw2 showed positive, however even weaker correlations with eye disease. HLA DR5 was associated with relapsing thyrotoxicosis, whereas HLA DR7 and B12 were negatively correlated with relapse. These resul

Characterization of T lymphocyte and monocyte populations in HLA B8/DRw3 normal individuals and in patients with dermatitis herpetiformis.

Normal individuals who possess the HLA B8/DRw3 haplotype as well as patients with dermatitis herpetiformis have been found to have a number of immunologic abnormalities including decreased numbers of E rosette-positive, Fc IgG receptor-bearing lymphocytes (referred to as TG cells), and increased numbers of cells which spontaneously secrete immunoglobulin. HLA B8/DRw3-positive normal individuals also have an increased risk for the development of a number of immunologically mediated diseases. Since many of these findings are suggestive of B-cell hyperreactivity and since TG cells were initially

Risk of coeliac disease in children of patients and effect of HLA genotype.

Among 73 offspring of parents with coeliac disease, 4 were affected, including 1 in whom the diagnosis was made as a result of the study. The coeliac parents of all 4 were HLA-B8 positive, but 1 affected child had failed to inherit his affected parent's B antigen. The findings are consistent with the hypothesis that an allele affecting liability to coeliac disease is in linkage disequilibrium with HLA-B8.

Graves' disease and HLA: clinical and epidemiologic associations.

Correlation between clinical and epidemiological features of Graves' disease and HLA were sought in 175 patients, eighty-three of whom were typed for HLA DR antigens. The relative risks (x) conferred by HLA-B8 and DR3 were 3.1 and 5.7 respectively (P less than 0.0005 uncorrected). The disease occurred at an earlier age in HLA-DR3 positive patients compared to negative patients (P less than 0.005 in females greater than and less than 30-years-old). Eighty-four out of ninety-two patients could specify the season of onset of hyperthyroidism; an excess of HLA-B8 positive patients in the summer and

HLA antigens and atopy in children with coeliac disease.

HLA antigens and various aspects of atopy were studied in 42 Finnish children and adolescents with coeliac disease, and the results were compared with findings of recent population studies. The HLA associations were as expected: relative risks for coeliac disease in individuals with HLA-B8, DR3, and DR7 were 8 . 0, 18 . 6, and 15 . 0, respectively. Children with coeliac disease were significantly more often atopic than unselected schoolchildren. Atopy was significantly more frequent and the onset of coeliac disease later for B8/DR3- patients than B8/DR3+ patients. There was no obvious relation

HLA-A, B and DR antigens and properdin factor B allotypes in Caplan's syndrome.

Seventy-nine cases of Caplan's lung were typed for HLA-A and B antigens. The antigen Bw45 was present only in those patients with rheumatoid factor and was of significantly higher frequency (13.6%) when compared to a non-coal dust exposed population of 316 (1.0%). Those patients without rheumatoid factor showed an increase in HLA-A1 and B8 (58.6% and 51.7% respectively) when compared to the rheumatoid factor positive group (29.6% and 25.0% respectively). Clinical and radiological reassessment were performed on 49 of these patients who were also typed for HLA-DR antigens and properdin factor B

Human histocompatibility antigen associations in subacute cutaneous lupus erythematosus.

We have identified a clinically distinct subset of lupus erythematosus patients marked by the presence of a histologically proven, nonscarring variety of cutaneous LE (subacute cutaneous LE) in which there is a very high frequency of the human leukocyte antigens (HLA) B8 and DR3. Differences in the configuration of their skin lesions allowed a separation of the patients into two clinical subgroups; annular and papulosquamous. HLA-B8 was increased in the annular subgroup (81%, corrected P (Pc) < 0.007) and combined group (65%, Pc < 0.004). HLA-DR3 was present in all 11 of the annular patients (

Immunogenetic findings in cutaneous lupus erythematosus.

The serologic features and histocompatibility antigen associations of 93 patients with cutaneous lupus erythematosus are reported. Of them, 37 had chronic cutaneous lupus erythematosus and 56 had subacute cutaneous lupus erythematosus. Subacute cutaneous lupus erythematosus with an annular pattern occurred in 32 patients; 24 had a papulosquamous morphology. Sixty percent of patients with annular subacute cutaneous lupus erythematosus had anti-Ro antibodies. The incidence of this antibody in the other two groups was lower. In addition, patients with annular skin lesions had significant antigen

A systematic survey of HLA-A,B,C and D antigens and drug toxicity in rheumatoid arthritis.

One hundred sixty-two consecutive patients with rheumatoid arthritis (RA) were studied for possible association between HLA antigens, particularly DR antigens, and disease characteristics and adverse reactions to gold or D-penicillamine treatment. The frequency of HLA-DR4 was significantly increased: 62% in RA compared to 23% in controls. An association of HLA-DR4 with a positive family history for RA was also found. HLA-DR4 was not associated with subcutaneous nodules or keratoconjunctivitis, presence of rheumatoid factor, or ANA positivity. No increased prevalence of HLA-DR3 was found in pat

Primary biliary cirrhosis associated with HLA-DRw3.

Twenty-one unrelated Caucasian patients with Primary Biliary Cirrhosis were typed for HLA-A, B, C and HLA-DRw antigens. The antigens HLA-Al, HLA-B8 and HLA-DRw3 were found in increased frequency in relation to the control group. When the P values were corrected for the number of antigens tested only the increase of HLA-DRw3 remained significantly different from the control group (P crr. < 0.004). The antigen HLA-DRw3 was carried by 12 out of 21 patients (57.1%) in comparison to 11 out of 74 (14.8%) normal unrelated Caucasian subjects.

[The spectrum of histocompatibility antigens (HLA) in Wegener's granulomatosis (author's transl)].

HLA-typing in 14 of 25 patients with histologically proven Wegener's granulomatosis showed a significantly increased frequency of HLA B7 and B8. No striking relation was found between the clinical course and the occurrence of HLA B7. The increased association between the HLA B7 antigen and Wegener's granulomatosis might be of diagnostic value.

[HLA system and collagen diseases (author's transl)].

Association of histocompatibility antigen (HLA antigens) with collagen diseases is not a casual event. Even known data, the practical conclusions have a limited usefullness because there is not a definitive correspondence between HLA antigens and rheumatic diseases. With HLA typing is possible to determine risk populations. The pauciarticular type II of juvenile reumatoid arthritis (J.R.A) is associated with HLA-B27 in the 75% of cases. Also, this antigen is associated with ankylosing spondylitis, Reiter's disease, Behçet disease, psoriasic arthritis, reactive arthritis (yersinia, shigella, sa
