BACKGROUND: This cadaveric study aimed to assess the biomechanical impact of functional hallux limitus (FHLim), identified using the functional stretch test, on first metatarsophalangeal joint (MTP1) mobility and joint pressures, as well as on the first cuneometatarsal joint (C1M1). A secondary objective was to evaluate the effect of retrotalar pulley section of the flexor hallucis longus (FHL) tendon on these parameters.
METHODS: Seventeen cadaveric feet from 9 donors were analyzed. Each specimen was assessed in a resting position and during a functional stretch test, defined as passive hall

BACKGROUND: Normal dorsiflexion of the first metatarsophalangeal joint during dynamic activities is critical for effective propulsion. Therapeutic foot orthotics may address the pathomechanical loading and joint kinematics issues faced by this population. This study aims to evaluate the effect of two different types of Custom-made foot orthosis compared to shod condition on the stiffness of the rearfoot, midfoot, and 1st metatarsophalangeal joint during walking in patients with Structural Hallux Limitus.
METHODS: This quasi-experimental study used a repeated-measures design with a single coho

Many authors have hypothesized on the etiology of hallux limitus and have proposed grading systems for classifying the degree of first metatarsophalangeal joint derangement and dysfunction. Several opinions exist regarding surgical treatment, should a patient fail with conservative therapy. Hinged, Silastic implant arthroplasty and cheilectomy account for a significant proportion of procedures accomplished. However, little follow-up documents and critically analyzes the long-term results of these procedures. This study analyzes, retrospectively, the results of these procedures with a minimum f

Functional hallux limitus is a loss of metatarsophalangeal joint extension during the second half of the single-support phase, when the weightbearing foot is in maximal dorsiflexion. Functionally, it constitutes a sagittal plane blockade during gait. As a result, the mechanical support and stability mechanisms of the foot are disrupted, with important consequences during gait. Functional hallux limitus is a frequent, though relatively unknown condition that clinicians may overlook when examining patients with complaints that are not limited to their feet, for they can also present other sympto

BACKGROUND: Hallux limitus is one of the most common disorders affecting foot biomechanics. Custom-made foot orthoses can improve the function of the first metatarso-phalangeal joint.
OBJECTIVES: The objective underlying this study was to test whether custom-made foot orthoses increased the range of mobility of metatarso-phalangeal joint in patients with hallux limitus.
STUDY DESIGN: Randomized, double-blinded, and clinical trial.
METHODS: The study consisted of 20 participants (40 feet) diagnosed with hallux limitus. A control group and an experimental group both wore the same custom-made

UNLABELLED: Lateral prevalence has not been traditionally attributed to the development and presentation of pathologic forefoot complaints. The objective of this study was to determine if a laterality prevalence exists for surgically corrected forefoot deformities. All cases performed at the Inova Fairfax Hospital Ambulatory Surgery Center over a 76-month period were reviewed and classified into the following categories: hallux abductovalgus deformity correction, hallux limitus deformity correction, specific digital deformity correction, neuroma surgery, and combinations of these categories (n

UNLABELLED: The aim of this study was to associate structural forefoot surgical correction with theories related to the etiology of structural forefoot pathology. All forefoot surgical cases performed at the Inova Fairfax Hospital Ambulatory Surgery Center over a 76-month period (January 2001 through April 2007) were reviewed and classified according to the following surgical categories: hallux abductovalgus, hallux limitus, lesser digital deformity, and combinations of these categories (N = 1684 procedures in 1592 patients). The results suggested that the etiology of lesser digital deformity

The condition of hallux limitus is well understood and agreed on as visualized histologically and radiographically. But the historically described pathophysiology and anatomy that predisposes to hallux limitus has been challenged. Numerous investigators have proposed anatomic abnormalities of the foot as a primary cause of this condition, but perhaps trauma is the only unanimously agreed on cause. However, this accounts for only a small percentage of cases. To strive for better treatment outcomes, understanding the pathophysiology, assessing patient risk factors, and recognizing causative agen

We hypothesized an association between foot type, foot deformity, and foot ulceration and conducted an analysis of a well-characterized, high-risk diabetic population of 398 subjects. The average age was 62 years of age and 74% of the study population were males. Foot-type distributions were 19.5% pes cavus (high arch), 51.5% neutrally aligned (normal arch), and 29.0% pes planus (low arch). We quantified the presence of hallux valgus (23.9%), hammer/claw toes (46.7%), and hallux limitus (24.4%). A significant association was found between foot type and hallux valgus (p = 0.003); pes planus fee

AIMS: To evaluate the factors that predict reulceration beneath the hallux in people with a history of diabetic foot ulceration.
METHODS: A prospective study conducted between January 2012 and December 2014 was performed in a diabetic foot unit to assess the risk factors associated with hallux reulceration. Sixty patients with diabetic neuropathy and a history of previous ulcer were consecutively included. Sociodemographic factors and comorbidities plus the biomechanical and radiographic factors were obtained. Follow-up on participants was conducted every month, and they wore offloading thera

The primary conditions characterized by functional limitation of motion of the first metatarsal are hallux limitus and hallux rigidus. The authors discuss the etiology, clinical manifestations, biomechanical compensatory mechanisms, and radiographic findings. Because only temporary relief is provided by conservative treatment, the various surgical approaches to treatment are discussed.

The enclavement procedure was originally described by Bernard Regnauld in 1968. The basal portion of the proximal phalanx of the hallux is used as an osteochondral autogenous graft in the treatment of hallux limitus, hallux rigidus, and hallux valgus. This paper presents a review of Regnauld's techniques, emphasizing an in situ modification of the inverted graft that allows for greater technical ease, increased stability, and improved graft viability, with more rapid incorporation.

We carried out a cross-sectional study in 51 patients (81 feet) with a clawed hallux in association with a cavus foot after a modified Robert Jones tendon transfer. The mean follow-up was 42 months (9 to 88). In all feet, concomitant procedures had been undertaken, such as extension osteotomy of the first metatarsal and transfer of the tendon of the peroneus longus to peroneus brevis, to correct the underlying foot deformity. All patients were evaluated clinically and radiologically. The overall rate of patient satisfaction was 86%. The deformity of the hallux was corrected in 80 feet. Catchin

Forefoot disorders are often seen in clinical practice. Forefoot deformity and pain can deteriorate gait function and decrease quality of life. This review presents common forefoot disorders and conservative treatment using an insole or orthosis. Metatarsalgia is a painful foot condition affecting the metatarsal (MT) region of the foot. A MT pad, MT bar, or forefoot cushion can be used to alleviate MT pain. Hallux valgus is a deformity characterized by medial deviation of the first MT and lateral deviation of the hallux. A toe spreader, valgus splint, and bunion shield are commonly applied to

We present a radiographic analysis of 91 failed hallux abducto valgus surgeries. Patients were categorized by type of postoperative complication: hallux varus, hallux limitus, or recurrent hallux abducto valgus deformities. All deformities were radiographically evaluated preoperatively and after correction using tricorrectional bunionectomy. Analysis of the surgical revisions showed improvement in radiographic parameters, including the intermetatarsal angle, hallux abductus angle, proximal articular set angle, and tibial sesamoid position. We conclude that tricorrectional bunionectomy is a ver

Many articles have been published on the various treatments of hallux rigidus/limitus but few, if any, have focused solely on the osteotomies performed in the treatment of this disorder and provided a thorough review of the literature and critique of the procedures. Here, we describe the most commonly used, most widely accepted, and most effective osteotomies in the treatment of hallux limitus/rigidus. Along with this discussion are figures and tables to make the information accessible and user friendly. Among the procedures discussed are Keller arthroplasty, Keller interpositional arthroplast

Cheilectomy is the excision of an irregular osseous rim that interferes with joint motion. Twenty cases of hallux rigidus/limitus treated by cheilectomy have been reviewed 18 months after surgery. A satisfactory outcome was achieved in all but 2 cases, which were failures. Range of joint motion, in particular dorsi flexion, improved in all but 6 cases. Cheilectomy is a simple and effective alternative to arthrodesis in the treatment of hallux rigidus.

BACKGROUND: Valente Valenti proposed in 1976 a "V" resection of the first metatarsophalangeal joint (MPT1) on the sagittal plane for advanced stages of hallux limitus/rigidus, preserving length of the first ray, flexor hallux brevis and sesamoid function. The available literature concerning correct indications, management and clinical outcomes of the Valenti procedure (VP) and its modifications in patients affected by hallux limitus/rigidus was systematically analyzed.
METHODS: Titles and abstracts of all selected articles were independently screened by two authors to assess their suitability

This presentation summarizes the results discovered on implantation surgery of 18 digits on 16 patients for correction of hallux limitus, or hallux rigidus. Of those studied, 66.7% received excellent and good results, 22.2% had fair results, and 11.1% had poor results. Comparison with the literature shows good correlation between studies and allows for quantification of the probability of positive returns on this form of surgery.
