Spontaneous flexor tendon rupture due to an insufficiency fracture of the hamate hook in a patient with systemic lupus erythematosus: A case report.

INTRODUCTION: Spontaneous flexor tendon rupture is usually caused by trauma, systemic diseases, or carpal bone and joint disorders. Here we report a case of spontaneous flexor tendon rupture occurring in a systemic lupus erythematosus (SLE) patient following nonunion of the hamate hook after an insufficiency fracture, and which was also associated with tendon degeneration caused by SLE. CASE PRESENTATION: A 57-year-old woman was diagnosed with SLE 22 years ago and being treated with oral prednisolone. She became unable to flex her left little finger without any history of trauma or sporting a

An Anthropometric Assessment of the Proximal Hamate Autograft for Scaphoid Proximal Pole Reconstruction.

PURPOSE: Fragmentation of the scaphoid proximal pole secondary to avascular necrosis presents a difficult reconstructive problem. This anthropometric study assesses the utility of the ipsilateral proximal hamate for complete osteochondral scaphoid proximal pole reconstruction. METHODS: Twenty-nine cadaveric specimens underwent computed tomography scanning and 3-dimensional reconstruction of the carpus and distal radius. Scaphoid height was measured and a third of its height was used to simulate resection of the proximal scaphoid pole and extent of hamate autograft required. The proximal scaph

Hooked on Non-union: A Rare Case of Concurrent Scaphoid and Hook of Hamate Non-union.

Carpal fractures are common orthopaedic injuries, with the scaphoid being the most frequently affected bone. Hamate fractures are uncommon and easily missed due to their subtle clinical presentation and difficulty in detection on standard radiographs, often being misdiagnosed as soft tissue trauma. Simultaneous involvement of the scaphoid and hamate is uncommon, and progression to dual non-union is even more so. We report a case of concurrent scaphoid and hook of hamate non-union to highlight the importance of early recognition and appropriate imaging to prevent delayed diagnosis and long-term

Biomechanical analysis of sport induced hamate stress fractures.

BACKGROUND: Bouldering is known for a high risk of hamate injuries. Over 75 % of sport climbers have signs of upper extremity overuse injuries. Training overload corresponds with bone edema and was linked to fractures of the hamate. To date, no study has addressed the pathomechanism of hamate fracture in climbing. The aim of this study was to investigate the stability of the hamate in a biomechanical setup under the tension of the flexor tendons. METHODS: 20 fresh-frozen, unfixed human forearms were used for two test series. The test stand consisted of an modified isokinet. Tension was applie

Pulling Harder than the Hamate Tolerates: Evaluation of Hamate Injuries in Rock Climbing and Bouldering.

OBJECTIVE: Hamate hook fractures are rare injuries, comprising 2% to 4% of all carpal fractures. Climbing athletes seem to be affected more frequently than others, as they strain the passive and active anatomical structures of their hands and fingers to maximum capacity during training or competing. This stress is transmitted to the hook of the hamate by tightened flexor tendons, which creates high contact pressure to the ulnar margin of the carpal tunnel. Injuries of the hamate hook, caused by contact pressure of the anatomical structures, are rare and occur nearly exclusively during climbing

[Avascular necrosis of the hamate--case report].

Avascular necrosis of the hamate is a very rare condition. After repetitive stress a young male developed left wrist pain which was found to be an avascular necrosis of the hamate. After progression of the necrosis with cyst formation on follow-up magnetic resonance imaging (MRI) and persisting pain, the patient underwent successful revascularisation with a vascular bone graft of the distal radius.

Pediatric Nonscaphoid Carpal Fractures: Fracture Distribution and Operative Management in an Institutional Cohort and 2 National EHR Databases.

BACKGROUND: This study aimed to characterize fracture distribution, demographic patterns, and operative management of pediatric nonscaphoid carpal fractures by leveraging a single institutional database and 2 large electronic health record (EHR) databases. We hypothesized that fractures would be more common among adolescent males and operative intervention would be rare. METHODS: A retrospective review of patients under 18 years of age who presented to a single Level I trauma center with nonscaphoid carpal fractures between 2017 and 2025 was performed. Demographics, injury mechanism, fracture

Fracture of the body of hamate associated with a fracture of the base of fourth metacarpal: A case report and review of literature of the last 20 years.

INTRODUCTION: Fractures of the carpal bones are often difficult to diagnose and treat due to the complex bone architecture of this region. Hamate fractures, particularly body fractures, are extremely uncommon. PRESENTATION OF CASE: We present a case of a coronal fracture of the hamate associated with a fracture of the base of the fourth metacarpal, which was treated by open reduction and internal fixation. DISCUSSION: Some of hamate body fractures are associated with other injuries like metacarpal fractures. Its diagnosis is difficult and requires a high clinical suspicion and a proper radio

Symptomatic, partial union of the hook of the hamate fracture in athletes.

BACKGROUND: Many clinicians believe that after fracture of the hook of the hamate, nonunions are the rule rather than the exception. PURPOSE: To describe a previously unreported condition in which the fractured hook of the hamate unites only along its ulnar column and causes pain similar to that in cases of nonunion. STUDY DESIGN: Retrospective review. METHODS: Eight patients who had sustained a hook of the hamate fracture were identified, on the basis of their persistent pain, as having achieved only partial union. This diagnosis was made despite evidence on computed tomography of healed b

Hamate hook nonunion treated with a hook plate: case report and surgical technique.

Despite of its rarity, hamate hook nonunion can cause several complications like tendon rupture or loss of grip strength. Admitted treatments in the literature are excision of the bone fragment or its open reduction and internal fixation. We report a clinical case of a high-level baseball player with hamate hook nonunion treated with an original technique of fixation using a hook plate.

Medial femoral condyle free flap for carpo-metacarpal instability following hamate comminute fracture.

Complete reconstruction of the hamate bone has been reported in the literature mostly following cancer excision or avascular necrosis. For the exiguity of the tissue deficit, bone grafting has usually been used as treatment option for its rapidity and easiness to perform, even if a variable amount of bone resorption may occur. In traumatic cases, microbial contamination may jeopardize the success of a well performed bone graft and vascularised bone grafts may represent a better reconstructive option. Here we describe the first case reported in the literature of a patient underwent complete ham

Concomitant hook of hamate fractures in patients with scaphoid fracture: more common than you might think.

OBJECTIVE: The scaphoid is the most commonly fractured carpal bone. The presence of a concomitant hook of hamate fracture is of particular relevance given that it is often occult on routine wrist/scaphoid radiographs and that hook of hamate fractures are prone to symptomatic non-union, resulting in chronic ulnar wrist pain. Prompt diagnosis and immobilisation/fixation may minimise such complications. Our study is aimed at assessing the frequency of concomitant hook of hamate fractures in patients with scaphoid fractures. METHODS: Hook of hamate fracture is often occult on wrist/scaphoid radio

The vascular basis of the hemi-hamate osteochondral free flap. Part 1: vascular anatomy and clinical correlation.

PURPOSE: The treatment of comminuted fractures of the proximal interphalangeal joint is highly challenging due to the complexities of joint bio-mechanics and stability. The hemi-hamate osteochondral auto-graft has been popularised in this role, able to replace articular loss and restore joint stability. Recent evaluation of their long-term follow-up however has shown the early development of osteoarthritis and potential early cartilage loss, which may be due to the non-vascularised nature of the graft. We offer a new technique that maintains vascularisation of the transferred hamate fragment,

Subject-specific finite element analysis of the carpal tunnel cross-sectional to examine tunnel area changes in response to carpal arch loading.

BACKGROUND: Manipulating the carpal arch width (i.e. distance between hamate and trapezium bones) has been suggested as a means to increase carpal tunnel cross-sectional area and alleviate median nerve compression. The purpose of this study was to develop a finite element model of the carpal tunnel and to determine an optimal force direction to maximize area. METHODS: A planar geometric model of carpal bones at hamate level was reconstructed from MRI with inter-carpal joint spaces filled with a linear elastic surrogate tissue. Experimental data with discrete carpal tunnel pressures (50, 100,

First reported case of a coronal hamate fracture associated with intrusion of the fifth metacarpal base, together with a fracture of the fourth metacarpal base.

We present a case reporting a rare combination of base of fourth metacarpal fracture and coronal body of hamate fracture, treated conservatively with excellent results. High index of suspicion for undetected bony injuries drove the execution of a CT scan, which allowed us to make a full correct diagnosis and plan treatment. Moreover, this case becomes the first reported case in the literature of such injury causing the intrusion of the base of the fifth metacarpal within the hamate bone, thus causing the coronal pattern of the hamate fracture. Differently from the common surgical management of

Ununited fracture of the hook of hamate treated with low-intensity pulsed ultrasound in an older middle-aged patient.

A 63-year-old man presented with a complaint of left ulnar wrist pain after playing golf. Computed tomography was used to detect a fracture at the base of the hook of hamate. Low-intensity pulsed ultrasound without a splint for immobilization was started, and bone union was confirmed 12 months after the ultrasound treatment started. The patient returned to play golf without any symptoms or complications. The present case suggests that ultrasound treatment is effective for fractures of the hook of hamate even in an older middle-aged patient. Ultrasound treatment is more time consuming than othe

Hemi-hamate arthroplasty versus transarticular Kirschner wire fixation for unstable dorsal fracture-dislocation of the proximal interphalangeal joint in the hand.

Following the introduction of the hemi-hamate arthroplasty (HHA) technique to our unit, we sought to evaluate the early clinical outcomes achieved with this method of fixation and compare these with simple trans-articular Kirschner wire (K-wire) fixation for dorsal fracture dislocations (DFD) of the proximal interphalangeal joint (PIPJ). Ninteen patients underwent fixation of these injuries with either K-wire fixation (12/19) or hemi-hamate bone grafting (7/19) between 2005 and 2011. At a mean follow-up of 14 weeks median arc of movement at the PIPJ was 65° (range 31° to 108°) following HHA an

Atypical osteochondroma of the hamate that presented clinically as carpal tunnel syndrome: report of an extremely rare case and literature review.

BACKGROUND: Osteochondroma is a benign tumor that occurs mainly at the metaphysis of long bones and seldom arises from carpal bones. We describe an extremely rare case of osteochondroma of the hamate without a typical cartilaginous cap and with a spiky bony protrusion in an elderly patient. CASE PRESENTATION: A 78-year-old right-handed female housekeeper had a multilobed osteochondroma of the hamate, which caused carpal tunnel syndrome and irritation of the flexor tendons. Radiological examinations showed a morphological abnormality of the hamate comprising a spiky bony protrusion into the ca
