Assessment of hand-arm vibration syndrome in a northern Ontario base metal mine.

The objective of this study was to determine the prevalence of hand-arm vibration syndrome (HAVS) in 617 workers at a base metal mine in northern Ontario. Workers who were employed at the mine between the years 1989 and 1994 and who continued to live within a 100 km radius of the mine were sent a self-reported questionnaire to identify individuals with possible vibration-induced symptoms in their upper extremities. Of the 162 workers who attended the medical examination, 50% were diagnosed with HAVS, 26% had other diagnoses with some workers having multiple afflictions e.g., both HAVS and carp

Diagnosis and treatment of hand-arm vibration syndrome and its relationship to carpal tunnel syndrome.

BACKGROUND: Hand-arm vibration syndrome (HAVS) is a condition associated with the use of vibrating tools that occurs mainly in men. It consists primarily of 'occupational' Raynaud disease and digital polyneuropathy. Carpal tunnel syndrome (CTS) is also associated with hand transmitted vibration exposure and can coexist with HAVS. OBJECTIVE: This article examines recent papers on causation, diagnosis, relationship to CTS and treatment. A Medline search was conducted, as was a search of UK, USA and Australian government occupational health and safety websites. Published papers that were single

Carpal Tunnel Syndrome and Hand-Arm Vibration: A Swedish National Registry Case-Control Study.

OBJECTIVE: To investigate the increased risk for carpal tunnel syndrome (CTS) in men and women with hand-arm vibration (HAV) exposure. DESIGN: Case-control study of CTS where 4396 cases was obtained from National Outpatient Register between 2005 through 2016. Cases were matched to controls and exposure was estimated using a job exposure matrix. RESULTS: Exposure to HAV increased the risk of CTS with an OR of 1.61 (95% CI 1.46-1.77). The risk was highest in men <30 years of age and among women <30 years no increased risk was observed. The risk increased with a mean year exposure above 2.5 m/s

Nerve conduction studies and current perception thresholds in workers assessed for hand-arm vibration syndrome.

BACKGROUND: Workers exposed to hand-arm vibration are at risk of developing the neurological abnormalities of hand-arm vibration syndrome (HAVS). The Stockholm classification of the neurological component of HAVS is based on history and physical examination. There is a need to determine the association between neurological tests and the Stockholm scale. AIMS: The main objective of this study was to compare the Stockholm neurological scale and the results of current perception threshold (CPT) tests and nerve conduction studies (NCS). METHODS: Detailed physical examinations were done on 162 su

Cold-provocation testing for the vascular component of hand-arm vibration syndrome in health surveillance.

The aim was to investigate whether the use of infra-red thermography (I-R) and measurement of temperature gradients along the finger could improve the diagnostic accuracy of cold-provocation testing (15 degrees C for 5 min) in vascular hand-arm vibration syndrome (HAVS). Twenty-one controls and 33 individuals with stages 2/3V HAVS were studied. The standard measurement of time to rewarm by 4 degrees C (T4 degrees C) and temperature gradients between the finger tip, base and middle (measured using I-R) were calculated. Receiver Operating Characteristics (ROC) analysis to distinguish between the

Neurological diagnosis--aspects of quantitative sensory testing methodology in relation to hand-arm vibration syndrome.

OBJECTIVE: The objectives are to summarise the fundamental neurophysiological base for quantitative sensory testing (QST), and to discuss associated methodological and practical aspects necessary to consider with respect to applicability and reliability as a screening or diagnostic aid for vibration-induced sensory neuropathy. RESULTS: QST is the use of psychophysical techniques to measure the intensity of stimuli needed to produce specific sensory perceptions. The physical components are graded stimuli presented to the skin, eye or ear. The psychological component is mental recognition of th

A comparison between two methods of aesthesiometric assessment in patients with hand-arm vibration syndrome.

Hand-arm vibration syndrome (HAVS) is caused by prolonged exposure to vibration. The diagnosis and assessment of disease severity are subjective at present. The aim of this study was to determine sensorineural dysfunction in patients with HAVS using two methods of aesthesiometric assessment. We recruited three groups of age-matched subjects: 20 subjects diagnosed as having HAVS, 15 manual workers and 15 sedentary workers. We measured both two-point discrimination and depth sense perception using an aesthesiometer. We found that the two-point discrimination wheel was more accurate than the dept

Exposure-response relationship in the hand-arm vibration syndrome: an overview of current epidemiology research.

The complex of vascular, neurologic, and osteoarticular disorders occurring in the upper limbs of vibration-exposed workers is called hand-arm vibration syndrome. There is epidemiologic evidence for an increased occurrence of peripheral sensorineural disorders in occupational groups working with vibrating tools. An excess risk for wrist osteoarthrosis and for elbow arthrosis and osteophytosis has been reported in workers exposed to shocks and low-frequency vibration of high magnitude from percussive tools. However, there are too few epidemiology data to enable reliable conclusions to be drawn

Application of Stockholm criteria to patients with hand-arm vibration syndrome in a follow-up study.

We applied Stockholm criterion to patients with hand-arm vibration syndrome (HAVS) for assessing the therapeutic effects in comparison with the criterion of the Ministry of Labour in Japan (MLJ). Forty male patients recognized as an occupational disease were examined in two times (mean interval periods 3.0 years). The subjects were free from vibratory tasks during the mean period of 8.3 years. The clinical examinations and some laboratory tests were conducted with the same procedures by the two same occupational physicians. The criterion of MLJ was graded more severe than Stockholm one for the

Current perception threshold for assessment of the neurological components of hand-arm vibration syndrome: a review.

Current perception threshold (CPT) has been proposed as a quantitative method for assessment of peripheral sensory nerve function. The aim of this review of selected reports is to provide an overview of CPT measurement for the assessment of the neurological component of hand-arm vibration syndrome (HAVS). The CPT values at 2000 Hz significantly increased for patients with HAVS. This result supports the previous histological findings that demyelination is found predominantly in the peripheral nerves in the hands of men exposed to hand-arm vibration. Diagnostic sensitivity and specificity were h

A Blue Finger, Severe Cyanosis of Raynaud's Phenomenon with Hand-Arm Vibration Syndrome: A Case Report.

INTRODUCTION: Raynaud's phenomenon (RP) can be classified as either primary or secondary and is traditionally characterized by triphasic color changes, including white/ischemia (blanching), blue/cyanosis, and red/hyperemia. Secondary RP, resulting from the use of handheld vibrating tools, is also referred to as vibration white finger. Blanching typically occurs in the fingertips; however, there are no documented cases of severe bilateral cyanosis associated with hand-arm vibration syndrome (HAVS). CASE PRESENTATION: A 69-year-old male with a 40-year history of using vibrating tools such as ch
