Fracture of fusion mass after hardware removal in patients with high sagittal imbalance.

OBJECT: As spinal fusions become more common and more complex, so do the sequelae of these procedures, some of which remain poorly understood. The authors report on a series of patients who underwent removal of hardware after CT-proven solid fusion, confirmed by intraoperative findings. These patients later developed a spontaneous fracture of the fusion mass that was not associated with trauma. A series of such patients has not previously been described in the literature. METHODS: An unfunded, retrospective review of the surgical logs of 3 fellowship-trained spine surgeons yielded 7 patients

Tips and Tricks for Hardware Removal and Allograft Fixation After a Failed Latarjet Procedure.

Although relatively rare, recurrent anterior shoulder instability requiring revision surgery after a failed Latarjet procedure can result in complications such as graft malposition, graft osteolysis, broken hardware and/or infection. Revision surgery after a failed Latarjet is a challenge due to a combination of technical factors such as alteration of the native anatomic landmarks and neurovascular structures, presence of extensive scar tissue and adhesions, and limited anterior glenoid real estate for a revision bone block. This technique outlines a systematic approach to the failed Latarjet

Does the Use of Locking Screws Decrease the Rate of Hardware Removal in Ankle Fractures?

Background. While biomechanical characteristics of locking screw fixation versus traditional plating has been studied extensively in orthopaedic literature, clinical outcome studies are lacking. The goal of this study was to evaluate the efficacy and complications rate of locking versus traditional nonlocking screws in complex ankle fractures employing distal fibula internal fixation with 1/3 semitubular small fragment plates. Methods. A retrospective review was performed between January 2010 and June 2013 of all patients in whom internal fixation of the fibula in an ankle fracture (open or cl

Effect of hardware removal and second-look arthroscopy after open-wedge high tibial osteotomy.

INTRODUCTION: Open-wedge high tibial osteotomy (OWHTO) is the standard and safe procedure for medial compartment osteoarthritis. Although hardware removal (HWR) is performed after post-OWHTO bone union, the effects of HWR on OWHTO have been rarely reported. We hypothesised that HWR would improve range of motion (ROM) and implant-related complications. Thus, this study aimed to investigate the effects of HWR on postoperative ROM, the Japan Orthopaedic Association (JOA) score/visual analogue scale (VAS) score and hardware-related complications after OWHTO. METHODS: Patients who underwent OWHTO

Contralateral Back Pain Following Trans-sacral Trans-iliac Screw Fixation: Resolution After Hardware Removal.

CASE: A 16-year-old girl presented to the emergency department after a 6-foot fall while skiing. The patient presented with left-sided pelvic pain and acute blood loss anemia. She was diagnosed with pelvic ring fracture (LC-2) and underwent open reduction internal fixation using a trans-sacral trans-iliac (TSTI) screw. She initially recovered but reported chronic right-sided back pain 4 years later. After hardware removal, she remained symptom-free for 1 year. CONCLUSION: Very few cases report contralateral pain associated with TSTI screws penetrating the opposite SI joint. This report highli

Total Knee Arthroplasty With Retained Tibial Implants: The Role of Minimally Invasive Hardware Removal.

Patients with retained tibial implants may later undergo total knee arthroplasty (TKA) after tibial osteotomy or fracture. Previous knee surgery can increase the rate of complications of joint replacement. Surgical scars may not be in optimal position for hardware removal and arthroplasty. The need for multiple incisions or larger subcutaneous skin flaps in at-risk areas may increase the possibility of wound complications and infection. In this article, we report 3 cases in which a patient who had knee arthrosis and retained tibial implants underwent single-stage conversion to TKA. In each cas

Complications and subsequent removal of retained shunt hardware after endoscopic third ventriculostomy: case series.

This case series highlights multiple complications and subsequent removal of retained shunt hardware in pediatric patients after successful endoscopic third ventriculostomy (ETV). Removal or retention of existing shunt hardware following ETV represents an important dilemma. Prior studies have reported infections and organ perforation related to nonfunctioning shunts but none in the context of successful ETV. Data obtained in 3 children with hydrocephalus treated at the authors' institution were retrospectively reviewed after the patients experienced complications due to retained shunt hardware

The association between plate location and hardware removal following ulna shortening osteotomy: a cohort study.

Hardware removal after ulna shortening osteotomy is common. We evaluated the association between plate location and hardware removal rate in 326 procedures in 321 patients with a median follow-up of 4.3 years (IQR 3.3) and corrected for confounding variables and did survival analyses. Complications were scored using the International Consortium for Health Outcome Measurement complications in Hand and Wrist Conditions tool. The 1-year and 5-year reoperation rates for hardware removal were 21% and 46% in the anterior group versus 37% and 64% in the dorsal group. Anterior plate placement was inde

Functional outcome following elbow release and hardware removal after bicolumnar fixation of distal humeral fractures.

INTRODUCTION: Intra-articular fractures of the distal humerus are typically treated with bicolumnar plate fixation. Despite prompt and accurate reduction and fixation, there is a high rate of complications post-surgical fixation. The purpose of this study was to determine the indications, technique, and outcomes of patients who had undergone an elbow release and hardware removal following bicolumnar plate fixation for an intra-articular fracture of the distal humerus. METHODS: Patients who had undergone an elbow release and hardware removal by a single surgeon following bicolumnar fixation of

Risk factors for hardware removal following operative treatment of middle- and distal-third clavicular fractures.

BACKGROUND: The incidence of hardware removal (HWR) after operative fixation of clavicular fractures varies widely. Risk factors related to HWR remain incompletely understood. The aim of this study was to evaluate the incidence of and risk factors for HWR after plate fixation of middle- and distal-third clavicular fractures. We hypothesized that (1) the total HWR incidence would be <20%, (2) the HWR incidence of operatively treated distal- and middle-third clavicular fractures would not be statistically different, and (3) symptomatic implants would be the most common HWR indication. METHODS:

Is Patient-Specific Hardware for Orthognathic Surgery More Frequently Removed Than Stock Plates?

BACKGROUND: Patient-specific implants (PSIs) offer predictability, accuracy, and ease of use, but their removal requirements after orthognathic surgery are unclear. Removing hardware has both a health care cost and negative impact on patients. PURPOSE: The purpose of this study was to compare hardware removal between orthognathic fixation with either PSIs or stock plates and identify risk factors associated with removal. STUDY DESIGN, SETTING, SAMPLE: This retrospective cohort study included patients with skeletal malocclusion who underwent orthognathic surgery with plate fixation between 20

What is the hardware removal rate after anteroinferior plating of the clavicle? A retrospective cohort study.

BACKGROUND: Plate position in the operative treatment of displaced midshaft clavicle fractures or nonunions is most often on the superior side. However, superior clavicular plating often results in complaints of plate prominence and local soft tissue irritation, necessitating hardware removal. We have used anteroinferior placement of the plate in the hope of increasing biomechanical stability and fixation and also of lowering complaints of plate prominence and soft tissue irritation. In this report, we set out to study the percentage of hardware removal in our group of patients treated with an

Syndesmosis fixation: a comparison of three and four cortices of screw fixation without hardware removal.

BACKGROUND: Great variability exists in methods of stabilization for syndesmotic disruptions of the ankle. We hypothesized that syndesmotic screw fixation with 3.5-mm fully threaded cortical screws through either three or four cortices would have similar strength and rate of mechanical failure and that retention of screws after fracture healing would not result in adverse clinical symptoms. METHODS: In a prospective, surgeon-randomized study at a Level-one trauma center, 127 patients with syndesmotic disruptions were treated surgically. Seven patients were lost to followup, leaving 120 for re

Innovative Removal of Non-Marginally Exposed Hardware From the Frontal Region With Successful Wound Closure.

RATIONALE: Titanium mesh is a common material for cranioplasty, but postoperative mesh exposure with infection poses a significant challenge-particularly for small, non-marginal defects where traditional instruments are ineffective. PATIENT CONCERNS: A 41-year-old female developed an infected, non-marginal titanium mesh exposure in the frontal region 60 days after implantation, which followed traumatic brain injury surgery. DIAGNOSES: Open craniocerebral injury; open frontal bone fracture; focal cerebral contusion and laceration; status post-cranioplasty with titanium mesh; and exposure of f

Have New Plate Designs Reduced the Rate of Hardware Removal Following Midshaft Clavicle Fracture Fixation?

Objectives: Operative fixation of displaced midshaft clavicle fractures has become increasingly the more acceptable choice of care in recent years, based on evidence supporting its effectiveness. However, this practice presents challenges due to the complex S-shaped morphology of the clavicle and its subcutaneous location. Despite the introduction of anatomically pre-contoured plates, achieving optimal implant-to-bone fit remains difficult, prompting the development of newer plate generations. The aim of this study was to compare the hardware removal rates of second-generation 2.7 mm thinner p
