Effects of artificial gravity on the vascular system during 60 days of strict head-down tilt bedrest.

During spaceflight, the lack of gravity-induced hydrostatic pressure gradients in the body results in a headward fluid shift hypothesized to play a role in pathophysiological changes in the cardiovascular, ocular, and central nervous systems. Here, 24 subjects (n = 8 control) underwent 60 days of strict 6° head-down tilt (HDT) bedrest, an analog of spaceflight where we tested the ability of daily 30-min artificial gravity (AG) exposure via short-arm human centrifugation (intermittent AG, n = 8, and continuous AG, n = 8) to reverse the HDT-induced headward fluid shift. Head-down tilt reduces the heart rate in postural tachycardia syndrome in acute setting: a pilot study.

BACKGROUND: Reduced preload and thoracic blood volume accompany postural tachycardia syndrome (POTS). Head-down tilt (HDT) increases both preload and intrathoracic blood volume. The objective of this study was to assess the safety and efficacy of HDT in POTS in acute settings. METHODS: This retrospective study evaluated POTS patients. Analyzed data included heart rate, blood pressure, cerebral blood flow velocity (CBFv) in the middle cerebral artery, and capnography. The baseline supine hemodynamic data were compared with the data obtained at the second minute of the -10° HDT. A linear mixed-

Effects of 28-day head-down tilt with and without countermeasures on lower body negative pressure responses.

This study was performed to determine the effects of 28 d of head-down tilt (HDT) (simulated weightlessness) on cardiovascular responses to orthostatic stress induced by lower body negative pressure (LBNP) (before, during, and after HDT) and +60 degrees head-up tilt (before and after HDT) in 12 subjects. Half of them underwent countermeasures (CM) of regular muscular exercise (isometric and isokinetic training) and LBNP sessions (-30 mm Hg) as generally performed during spaceflight; the other six were a control group (C). The countermeasure effect on the orthostatic responses to LBNP and tilt

Head-down tilt 15° increases cerebral perfusion before recanalization in acute ischemic stroke. A pre-clinical MRI study.

We investigated the therapeutic effect of head-down tilt at -15° (HDT15) on cerebral collateral flow and early infarct growth in a rat model of large vessel occlusion stroke, using multi-modal MRI. Endovascular occlusion of the proximal middle cerebral artery was induced for 90 min in Wistar rats (n = 28), followed by reperfusion. Rats were randomly assigned to HDT15 or flat position for 60 min, starting 30 min after occlusion. Multi-modal brain MRI, including perfusion, angiographic and structural sequences were acquired before treatment, after 60 min of treatment and 24 h after reperfusion.

Effect of lower body negative pressure against orthostatic intolerance induced by 21 days head-down tilt bed rest.

BACKGROUND: Exposure to actual or simulated weightlessness is known to induce orthostatic intolerance in humans. Many different methods have been suggested to counteract orthostatic hypotension. The repetitive or prolonged application of lower body negative pressure (LBNP) has shown beneficial effects to counter orthostatic intolerance, but devoting so much time to countermeasures is not compatible with space mission objectives or costs. The purpose of the present study was to assess the effects of brief LBNP sessions against orthostatic intolerance during a 21-d head-down tilt (HDT) bed rest.

Biomechanical analysis of combining head-down tilt traction with vibration for different grades of degeneration of the lumbar spine.

In recent years, a combination of traction and vibration therapy is usually used to alleviate low back pain (LBP) in clinical settings. Combining head-down tilt (HDT) traction with vibration was demonstrated to be efficacious for LBP patients in our previous study. However, the biomechanics of the lumbar spine during this combined treatment is not well known and need quantitative analysis. In addition, LBP patients have different grades of degeneration of the lumbar spinal structure, which are often age related. Selecting a suitable rehabilitation therapy for different age groups of patients h

Effect of head-down-tilt bed rest and hypovolemia on dynamic regulation of heart rate and blood pressure.

Adaptation to head-down-tilt bed rest leads to an apparent abnormality of baroreflex regulation of cardiac period. We hypothesized that this "deconditioning response" could primarily be a result of hypovolemia, rather than a unique adaptation of the autonomic nervous system to bed rest. To test this hypothesis, nine healthy subjects underwent 2 wk of -6 degrees head-down bed rest. One year later, five of these same subjects underwent acute hypovolemia with furosemide to produce the same reductions in plasma volume observed after bed rest. We took advantage of power spectral and transfer functi

Quantitative ultrasound image assessment of the optic nerve subarachnoid space during 90-day head-down tilt bed rest.

The elevation in the optic nerve sheath (ONS) pressure (ONSP) due to microgravity-induced headward fluid shift is the primary hypothesized contributor to SANS. This longitudinal study aims to quantify the axial plane of the optic nerve subarachnoid space area (ONSSA), which is filled with cerebrospinal fluid (CSF) and expands with elevated ONSP during and after head-down tilt (HDT) bed rest (BR). 36 healthy male volunteers (72 eyes) underwent a 90-day strict 6° HDT BR. Without obtaining the pre-HDT data, measurements were performed on days 30, 60, and 90 during HDT and at 6 recovery time point

Linking cerebral hemodynamics and ocular microgravity-induced alterations through an in silico-in vivo head-down tilt framework.

Head-down tilt (HDT) has been widely proposed as a terrestrial analog of microgravity and used also to investigate the occurrence of spaceflight-associated neuro-ocular syndrome (SANS), which is currently considered one of the major health risks for human spaceflight. We propose here an in vivo validated numerical framework to simulate the acute ocular-cerebrovascular response to 6° HDT, to explore the etiology and pathophysiology of SANS. The model links cerebral and ocular posture-induced hemodynamics, simulating the response of the main cerebrovascular mechanisms, as well as the relationshi

Intracranial blood flow measured with single photon emission computer tomography (SPECT) during transient -6 degrees head-down tilt.

Regional cerebral blood flow (CBF) during a transient head-down tilt of -6 degrees (-6 degrees HDT) was measured with single photon emission computer tomography (SPECT). CBF was measured and averaged for both sides of the brain areas; e.g., the bilateral anterior cerebral artery (bACA) area, the middle cerebral artery (bMCA) area, the posterior cerebral artery (bPCA) area, bilateral basal ganglia, and the cerebellum. Among these areas, a significant increase in CBF was observed in the basal ganglia and the cerebellum during -6 degrees HDT compared to pre-HDT. When CBF was measured separately i

Altered cerebral perfusion in response to chronic mild hypercapnia and head-down tilt Bed rest as an analog for Spaceflight.

PURPOSE: Following prolonged stays on the International Space Station (ISS), some astronauts exhibit visual acuity changes, ophthalmological findings, and mildly elevated intracranial pressures as part of a novel process called spaceflight-associated neuro-ocular syndrome (SANS). To determine the pathophysiology of SANS, NASA conducted a multi-investigator study in which 11 healthy participants underwent head-down tilt bed rest, mimicking microgravity-induced cephalad fluid shifts, combined with elevated ambient CO2 levels similar to those on the ISS (HDT+CO2). As part of that study, we examin

Daily use of countermeasures to prevent headward fluid shifts throughout 30 days of strict head-down tilt bed rest.

Strict 6° head-down tilt bed rest (HDTBR) induces a chronic headward fluid shift similar to weightlessness that may underlie the development of spaceflight-associated neuro-ocular syndrome. We tested the efficacy of two headward fluid shift countermeasures, using either 25 mmHg lower body negative pressure (LBNP, 2 × 3 h daily, n = 12) or use of venoconstrictive thigh cuffs (VTC) following 1 h of moderate aerobic cycle exercise (Ex + CUFF, 6 h daily for 6 of 7 days, n = 12) across 30 days of HDTBR. The study also included a seated posture group (Upright, 2 × 3 h daily, n = 11), which was condu

Combining head-neck position and head-down tilt to prevent pulmonary aspiration of gastric contents during induction of anaesthesia: a volunteer and manikin study.

CONTEXT: Although a life-threatening complication, pulmonary aspiration of gastric contents caused by vomiting or regurgitation during induction of anaesthesia cannot be prevented. It may be prevented if the mouth is placed more inferiorly than the larynx and tracheal bifurcation by the use of head-down tilt and head-neck positioning. OBJECTIVE: We aimed to determine the head-down tilt required to prevent aspiration in the neutral, simple extension, sniffing and full cervical spine extension (Sellick) positions and to investigate the relationship between pulmonary aspiration and the vertical

Effect of 60 days of head down tilt bed rest on amplitude and phase of rhythms in physiology and sleep in men.

Twenty-four-hour rhythms in physiology and behaviour are shaped by circadian clocks, environmental rhythms, and feedback of behavioural rhythms onto physiology. In space, 24 h signals such as those associated with the light-dark cycle and changes in posture, are weaker, potentially reducing the robustness of rhythms. Head down tilt (HDT) bed rest is commonly used to simulate effects of microgravity but how HDT affects rhythms in physiology has not been extensively investigated. Here we report effects of -6° HDT during a 90-day protocol on 24 h rhythmicity in 20 men. During HDT, amplitude of li

[The pharmacodynamics and pharmacokinetics of furosemide during ordinary life activities and during head-down tilt hypokinesia in a healthy subject].

The object of the work was comparative study of the characteristics of pharmacodynamics and pharmacokinetics of the diuretic furosemide during ordinary vital activity of man and under conditions of antiorthostatic hypokinesia. Six practically healthy males were examined. They took part successively in two experimental series: series 1--during an ordinary motor regimen under ambulatory conditions; series 2--under conditions of antiorthostatic hypotension (ANOH, 12 degrees). In both series with absolute correspondence of the procedures and order of manipulations the subjects were given 40 mg fur

MRI-based quantification of ophthalmic changes in healthy volunteers during acute 15° head-down tilt as an analogue to microgravity.

Spaceflight is known to cause ophthalmic changes in a condition known as spaceflight-associated neuro-ocular syndrome (SANS). It is hypothesized that SANS is caused by cephalad fluid shifts and potentially mild elevation of intracranial pressure (ICP) in microgravity. Head-down tilt (HDT) studies are a ground-based spaceflight analogue to create cephalad fluid shifts. Here, we developed non-invasive magnetic resonance imaging (MRI)-based techniques to quantify ophthalmic structural changes under acute 15° HDT. We specifically quantified: (i) change in optic nerve sheath (ONS) and optic nerve (

Optic disc edema and chorioretinal folds develop during strict 6° head-down tilt bed rest with or without artificial gravity.

Spaceflight associated neuro-ocular syndrome (SANS) is hypothesized to develop as a consequence of the chronic headward fluid shift that occurs in sustained weightlessness. We exposed healthy subjects (n = 24) to strict 6° head-down tilt bed rest (HDTBR), an analog of weightlessness that generates a sustained headward fluid shift, and we monitored for ocular changes similar to findings that develop in SANS. Two-thirds of the subjects received a daily 30-min exposure to artificial gravity (AG, 1 g at center of mass, ~0.3 g at eye level) during HDTBR by either continuous (cAG, n = 8) or intermit
