BACKGROUND: Cluster headache (CH) is a rare primary headache disorder characterized by recurrent episodes of strictly unilateral excruciating pain accompanied by trigemino-autonomic signs, which significantly impacts the quality of life, social interactions, and occupational functioning of those who are affected. To promote a better understanding of this disabling condition and to foster research on the topic, this review provides a comprehensive description of the hallmarks of CH, including its clinical presentation, diagnostic challenges, pathophysiology, and current and novel therapeutic ta

OBJECTIVE: Medication-overuse headache (MOH) in cluster headache (CH) patients is incompletely described, perhaps because of the relatively low prevalence of CH.
METHODS: The authors describe a retrospective series of 17 patients (13 men, 4 women) with CH who developed MOH in association with overuse of a wide range of monotherapies or varying combinations of simple analgesics (n = 9), caffeine (n = 1), opioids (n = 10), ergotamine (n = 3), and triptans (n = 14). The series includes both episodic (n = 7) and chronic (n = 10) CH patients.
RESULTS: A specific triptan-overuse headache diagnosis

The prevalence and the clinical features of chronic daily headache (CDH) were studied in 968 children and adolescents observed during a period of one year in the Headache Centre of the Anna Meyer Paediatric Hospital of Florence. Nine hundred and forty-four patients (97.52%) had primary headache according to ICHD-II, 24 subjects had secondary headache and 56 patients had CDH (5.93% of primary headaches). The mean age of subjects with CDH was higher than general (13.5 vs. 11.5 years), with a female preponderance (69.6% vs. 30.4%). According to the ICHD-II, headaches were classified as chronic mi

Chronic daily headache (CDH) is an overarching term that includes multiple types of frequent primary headaches that are not trigeminal-autonomic cephalgias. The components of typical CDH can be divided into a more severe or "big" headache and a less severe or "little" headache. The big headaches tend to have features of migraine while the little headaches have features of tension-type headache (TTH). Whether this represents a spectrum or continuum or whether it is the superimposition of two unique headache entities is open to debate. For subjects with big and little headache, the concept that

OBJECTIVE: To determine the feasibility of developing a headache management program and to assess the outcomes of patients referred to the program for treatment of chronic headache.
BACKGROUND: Effective headache treatment requires that the patient receives the correct headache diagnosis; that appropriate acute and, if indicated, preventive medications be prescribed; and that the patient receives adequate education, including headache self-management skills.
DESIGN/METHODS: A headache management program was established at a northern California staff-model health maintenance organization. Fif

Background Headache attributed to aeroplane travel (AH) is currently codified in Chapter 10.1.2 of the International Classification of Headache Disorders 3 beta (ICHD 3 beta). After our previous papers, the investigation was enlarged to 200 patients. Material and methods People affected by AH contacted us by mail and agreed to fill in a detailed anonymous questionnaire that allowed a thorough definition of the headache features. Results This extended case series allows us to confirm most of the clinical features of AH, reported in our previously published article, and to partially modify other

Although classification can be difficult, determining the type of headache, such as tension, migraine or cluster, aids in the formulation of a therapeutic approach. Treatment involves regulation rather than cure. Acute and prophylactic pharmacotherapy and control of stress through behavior modification, relaxation training and biofeedback are methods of management.

Headaches are common in the pediatric population and can cause significant distress to the patient and their family. The ability of pediatricians to appropriately diagnose and differentiate headaches is important to guide management. This article reviews the epidemiology, classification, and treatment of headaches, including migraines. [Pediatr Ann. 2021;50(12):e479-e485].

Migraine and other headaches affect 54 million people in Germany. They rank among the ten most severely disabling complaints and the three most expensive neurological disorders. Nevertheless, they are not adequately recognized in the healthcare system with sketchy diagnoses and inadequate treatment. This inadequate care is not primarily due to a lack of medical and scientific knowledge on the development and treatment of headaches but is predominantly due to organizational deficits in the healthcare system and in the implementation of current knowledge. To overcome the organizational barriers

Tension-type headache and migraine are the most common types of primary headaches. Apart from these well known diseases, the group of primary headaches includes other relatively rare headache disorders. Some of these seldom syndromes have been described for the first time within the last twenty years and have been newly included in the revised IHS classification from 2004. Their typical symptomatic is less known, but offers an excellent opportunity to diagnose these syndromes. The importance of recognising these disorders is underlined by the fact, that rare primary headaches response often co

Cervicogenic headache (CGH) has proven to be an elusive diagnosis without the aid of diagnostic interventional procedures. This article highlights the key components of the clinical presentation and workup to aid the clinician in distinguishing CGH from its differential diagnoses. It also discusses the evidence and outcomes behind the treatment options to guide physicians when establishing a plan for patients. Fortunately, most cases will resolve with conservative care and physical therapy, but severe or chronic cases will likely require the expertise of an interventional spine physician or su

Most patients presenting with headaches to the emergency department (ED) are experiencing benign exacerbations of chronic conditions. The ED focus is on identifying dangerous secondary causes of headaches and providing pain relief. Given the high recurrence rate, emergency physicians must optimize acute treatment, address unmet needs, and ensure appropriate outpatient follow-up.

OBJECTIVE: This article provides an overview of the epidemiology, diagnosis, clinical presentation, pathophysiology, prognosis, and treatment of posttraumatic headache attributed to mild traumatic brain injury (mTBI).
LATEST DEVELOPMENTS: The International Classification of Headache Disorders, Third Edition requires that posttraumatic headache begin within 7 days of the inciting trauma. Although posttraumatic headache characteristics and associated symptoms vary, most commonly there is substantial overlap with symptoms of migraine or tension-type headache. New insights into posttraumatic head

INTRODUCTION: Cervicogenic headache, first proposed as a distinct headache in 1983, is a secondary headache to a primary cervical musculoskeletal disorder. Research into physical impairments was integral to clinical diagnosis and to develop and test research informed conservative management as the first line approach.
PURPOSE: This narrative presents an overview of the body of cervicogenic headache research from our laboratory which was undertaken in the context of a broad program of research into neck pain disorders.
IMPLICATIONS: Early research validated manual examination of the upper cer

After sustaining a concussion or mild traumatic brain injury, headaches are one of the most common complaints. The pathophysiologic changes that occur in the setting of injury likely contribute to or cause posttraumatic headaches. Posttraumatic headaches often present as migraine or tension-type headaches. Unlike pain from other types of injuries, headaches following mild traumatic brain injury are more likely to persist. Preexisting conditions such as migraine and mood disorders may influence posttraumatic headache and complicate management. Patients are at high risk to overuse abortive medic

The headache associated with intercourse or masturbatory activity is a well-recognized clinical entity but pornography headache is barely mentioned. We report a young man who suffered preorgasmic headache pertaining only to pornography of specific erotic contents but not to other sexual or nonsexual act. An antecedent activation of sexual arousal and vasoconstriction during pain were found. Finally, oral indomethacin favorably prevented the pain. Therefore, pornography headache is a distinguished headache disorder distinct from other sexual-related headache disorders. Sexual arousal-mediated c

A precondition for the successful treatment of headaches is the correct headache diagnosis. Triptans are effective for attack treatment of migraine and cluster headache. However, there are not effective for the treatment of tension-type headache. For the prevention of frequent episodic migraine betablockers, flunarizine, topiramate and amitriptyline are recommended. For the prevention of chronic migraine evidence is only available for onabotulinumtoxinA and topiramate. For prophylactic treatment of tension-type headaches tricyclic antidepressants are used. A patient with fever, headache and discrete neurologic symptoms developed a coma followed by severe paralysis. The cause was a tick-borne encephalitis. In the follow-up, the patient required supportive care on the intensive care unit for almost two months and further on, a rehabilitation of almost seven months was needed. The patient has not been vaccinated, even though he fulfilled the criteria for the recommendation of vaccination because he occasionally visited areas at risk.
