As endocrinologists we have focused on biological contributors to disparities in diabetes, obesity and other endocrine disorders. Given that diabetes is an exemplar health disparity condition, we, as a specialty, are also positioned to view the contributing factors and solutions more broadly. This will give us agency in contributing to health system, public health, and policy-level interventions to address the structural and institutional racism embedded in our medical and social systems. A history of unconsented medical and research experimentation on vulnerable groups and perpetuation of eug

Health care in Brazil is financed from many sources--taxes on income, real property, sales of goods and services, and financial transactions; private insurance purchased by households and firms; and out-of-pocket payments by households. Data on household budgets and tax revenues allow the burden of each source except firms' insurance purchases for their employees to be allocated across deciles of adjusted per capita household income, indicating the progressivity or regressivity of each kind of payment. Overall, financing is approximately neutral, with progressive public finance offsetting regr

Structural racism-which encompasses discriminatory laws/policies, institutional practices, and social norms-is a driver of racialized health inequities in the U.S. Notably, laws/policies at the federal, state, and local levels perpetuate structural racism across all societal domains and are key structural determinants of racial health equity. Health equity practitioners play a critical role in mitigating the impacts of structural racism-related laws/policies through their legal, policy, and community-based work at state and local health departments, community-based organizations, and health po

BACKGROUND: Health systems commonly evaluate justice using quantitative health equity indicators such as healthcare coverage, financial protection, population health outcomes, and measures of the social determinants of health. These indicators are often treated as competing operationalisations of a single moral standard. This article argues that this assumption is conceptually mistaken because different indicators evaluate different normative objects rather than alternative dimensions of the same conception of justice.
METHODS: Using a comparative normative methodology, the article analyses s

In 2018, the Community Health Representative (CHR) workforce celebrated their 50th year and serve as the oldest and only federally funded Community Health Worker (CHW) workforce in the United States. CHRs are a highly trained, well-established standardized workforce serving the medical and social needs of American Indian communities. Nationally, the CHR workforce consists of ~1,700 CHRs, representing 264 Tribes. Of the 22 Tribes of Arizona, 19 Tribes operate a CHR Program and employ ~250 CHRs, equivalent to ~30% of the total CHW workforce in the state. Since 2015, Tribal CHR Programs of Arizon

Prior to a fall semester graduate course in public health, students are encouraged to complete a summer reading assignment based on a book chosen from a curated list of popular literature relevant to public health concepts. From 2020 to 2024, this assignment enabled students to explore the history of structural and systemic racism in the United States and its impacts on current public health policy and programs. Themes from these books are woven into class discussions and further explored in subsequent course assignments. This paper describes the assignment and shares responses from the 1,141

Autism spectrum disorder (ASD) is a common, lifelong neurodevelopmental condition whose recorded prevalence, diagnostic delays, and uneven distribution of specialist services create a growing public health challenge. Conventional screening and diagnostic pathways depend heavily on scarce specialist expertise, contributing to long waiting times and unequal access across income settings, regions, sex, ethnicity, language, and social position. This narrative review synthesises current applications of artificial intelligence (AI) and machine learning in autism screening, diagnostic support, interv

PURPOSE OF REVIEW: Chronic kidney disease (CKD) is a widespread health issue, affecting one out of every 10 adults. This prevalence is even higher among vulnerable and underserved populations, including low-income individuals, racial and ethnic minorities, and immigrants. Urban areas such as New York City and Los Angeles County offer municipal safety-net healthcare systems for these groups.
RECENT FINDINGS: Safety-net providers are essential to the healthcare landscape for vulnerable populations with chronic diseases including the Los Angeles County Health Services that exemplifies how effect

OBJECTIVES: COVID-19 created an urgent element of clinical and financial strain to the public health system, forcing it into rapid response mode. Key public health decisions were quickly made, with limited data and guidance to address decision trade-offs and community inequities. Gaps identified in the pandemic confirmed the need for a new tool, like this study's Dashboard Instrument to Review Equity (DIRE) Framework, to ensure decision-makers are given quick and equitable decision-making guidance.
STUDY DESIGN: Scoping review and tool development.

BACKGROUND: Hypertension is a significant risk factor for cardiovascular disease, which remains the leading cause of death in the United States. Inequities in blood pressure control and cardiovascular mortality are seen across racial and ethnic groups. One health network, a not-for-profit integrated system in the Southeastern United States, identified a disparity in population-level rates of attainment of hypertension control in their Black adult patients. About 65% of Black patients achieved blood pressure goal, defined as < 140/90 mmHg, compared to 75.6% of patients in other populations. The

Governmental data generated during the COVID 19 pandemic from 2019 to 2021 demonstrated that the virus disproportionately impacted communities of color. Specifically, Black, Hispanic, and Asian people were found to have substantially higher rates of infection, hospitalization, and death compared with White people. The extent of these disparities drew attention to other disparate outcomes including maternal mortality and morbidity, cancer outcomes, and overall life expectancy. Various professional organizations, including the Institute for Healthcare Improvement, began to add health equity to t

This review summarizes the evidence of health disparities in cystic fibrosis (CF), an autosomal recessive genetic disorder with substantial variation in disease progression and outcomes. We review disparities by race, ethnicity, socioeconomic status, geographic location, gender identity, or sexual orientation documented in the literature. We outline the mechanisms that generate and perpetuate such disparities across levels and domains of influence and assess the implications of this evidence. We then recommend strategies for improving equity in CF outcomes, drawing on recommendations for the g

When the COVID-19 pandemic first took the world by storm, the World Health Organization (WHO) issued a Solidarity Call to Action to realize equitable global access to COVID-19 health technologies through pooling of knowledge, intellectual property and data. At the dawn of 2022, 70% of rich countries' populations were vaccinated but only 4.6% of poor countries (Our World In Data, Coronavirus (COVID-19) vaccinations, 2022). Vaccine nationalism and rampant self-interest grew and our ineffective global response led to new variants of concern - like Omicron - emerging. Rather than abandon the idea

For almost five decades, the US Supreme Court had protected the right to abortion care in the United States. However, the Court's decision in 'Dobbs versus Jackson Women's Health Organization' in 2022 established that the US Constitution does not confer a right to abortion, effectively overturning the Court's own previous judgment in 'Roe versus Wade' from 1973. How could a decision that overturns the precedent for nationwide access to abortion affect women, children, and physicians? How will state laws impact healthcare equity regarding reproductive rights going forward? How will geography af

PURPOSE OF REVIEW: We review recent community interventions to promote mental health and social equity. We define community interventions as those that involve multi-sector partnerships, emphasize community members as integral to the intervention, and/or deliver services in community settings. We examine literature in seven topic areas: collaborative care, early psychosis, school-based interventions, homelessness, criminal justice, global mental health, and mental health promotion/prevention. We adapt the social-ecological model for health promotion and provide a framework for understanding th

Health equity, in the context of public health in the United States, can be characterized as action to ensure all population groups living within a targeted jurisdiction have access to the resources that promote and protect health. There appear to be several elements in program design that enhance health equity. These design elements include consideration of sociodemographic characteristics, understanding the evidence base for reducing health disparities, leveraging multisectoral collaboration, using clustered interventions, engaging communities, and conducting rigorous planning and evaluation

A growing body of epidemiological studies shows that noise increases the risk for broad-ranging physiological and psychological illnesses. According to the World Health Organization, at least 1.6 million healthy years of life are lost annually in Western Europe due to traffic noise alone. Scholars from selected Expert Panels of the American Academy of Nursing convened to update the information regarding the effects of environmental noise on health disparities, physiological changes in the human body and in animals, and environmental justice issues, and to inform the public and policy makers' e
