Bias and fairness assessment of a natural language processing opioid misuse classifier: detection and mitigation of electronic health record data disadvantages across racial subgroups.

OBJECTIVES: To assess fairness and bias of a previously validated machine learning opioid misuse classifier. MATERIALS & METHODS: Two experiments were conducted with the classifier's original (n = 1000) and external validation (n = 53 974) datasets from 2 health systems. Bias was assessed via testing for differences in type II error rates across racial/ethnic subgroups (Black, Hispanic/Latinx, White, Other) using bootstrapped 95% confidence intervals. A local surrogate model was estimated to interpret the classifier's predictions by race and averaged globally from the datasets. Subgroup analy

Toward a Deeper Understanding of Predicting Risk of Cardiovascular Disease Events for 10-Year Atherosclerotic Cardiovascular Risk: Subgroup Fairness and Predictive Value of Social Determinants of Health.

BACKGROUND: The American Heart Association's Predicting Risk of Cardiovascular Disease Events model offers a modern, race-free approach to risk prediction, but its subgroup fairness and the added value of social determinants of health (SDOH) remain underexplored. METHODS: To evaluate Predicting Risk of Cardiovascular Disease Events in 10-year atherosclerotic cardiovascular disease prediction regarding fairness and value of SDOH predictors, we conducted a retrospective cohort study of 554 675 adults aged 30 to 79 years, using deidentified electronic health records from Truveta, a multisystem U

Pathways to Health Reported by Patients With Borderline Personality Disorder With a Good Overall Outcome Versus a Fair-Poor Outcome Over 24 Years of Prospective Follow-Up.

Our objective was to determine pathways to health reported by patients with borderline personality disorder (BPD) who had and had not attained a good overall outcome over 24 years of prospective follow-up. Overall outcome symptomatically and psychosocially and 11 pathways to health related to vocation, relationships, activities, and psychiatric treatment that patients reported were helpful to their functioning or feeling better about themselves were assessed at 12 contiguous 2-year follow-up periods using a semistructured interview. Good outcome patients reported significantly higher rates of

Brief behavioral interventions at free medical fairs.

Free medical fairs have emerged to compensate for the lack of access to affordable health care in rural areas of the United States. Mental health services are offered less frequently than other medical services, despite a documented need, perhaps due to a belief that mental health interventions could not be effective in a single session. We examined the types of problems presented at three rural medical fairs, and whether single session mental health interventions affected participants' health confidence, distress, or progress toward health-related goals. Problems presented included mental hea

Harambee!: A pilot mixed methods study of integrated residential HIV testing among African-born individuals in the Seattle area.

BACKGROUND: African-born individuals in the U.S. are disproportionately affected by HIV yet have low HIV testing rates. We conducted a mixed methods study to assess the uptake and feasibility of a novel strategy for integrating HIV testing into residential health fairs among African-born individuals in Seattle, WA. METHODS: From April to May 2018, we held six health fairs at three apartment complexes with high numbers of African-born residents. Fairs included free point-of-care screening for glucose, cholesterol, body mass index, blood pressure, and HIV, as well as social services and health

Community-based health screening and all-cause mortality in rural South Africa: a longitudinal cohort study.

INTRODUCTION: South Africa is experiencing a significant shift in its disease burden from high HIV and tuberculosis (TB) mortality to increased burden of non-communicable diseases (NCDs). Despite this, screening, treatment and control for NCDs remain limited. Community health fairs are often used to identify and refer individuals with chronic diseases in underserved areas, but their impact on morbidity and mortality is unclear. METHODS: From 2018 to 2020, individuals aged 15+ in the Africa Health Research Institute's demographic surveillance area in rural KwaZulu-Natal were invited to Vukuzaz

Shortening Duration of Swine Exhibitions to Reduce Risk for Zoonotic Transmission of Influenza A Virus.

Reducing zoonotic influenza A virus (IAV) risk in the United States necessitates mitigation of IAV in exhibition swine. We evaluated the effectiveness of shortening swine exhibitions to <72 hours to reduce IAV risk. We longitudinally sampled every pig daily for the full duration of 16 county fairs during 2014-2015 (39,768 nasal wipes from 6,768 pigs). In addition, we estimated IAV prevalence at 195 fairs during 2018-2019 to test the hypothesis that <72-hour swine exhibitions would have lower IAV prevalence. In both studies, we found that shortening duration drastically reduces IAV prevalence i

Harambee! 3.0: Addressing HIV-Stigma and Increasing HIV-Testing in Three African Immigrant Communities in King County.

BACKGROUND: In King County, Washington, the new HIV diagnosis rate is 10 times higher among African immigrants than the general population. Many African immigrants report never testing for HIV due to various reasons, including inaccessible testing, stigma around HIV and testing, and concerns over jeopardizing their immigration status. OBJECTIVES: We partnered with the Ethiopian Community in Seattle, the Eritrean Health Board, and the Congolese United Foundation to deliver community-based HIV testing via health fairs at faith-based organizations in King County. The presence of family history and the development of type 2 diabetes mellitus risk factors in rural children.

Type 2 diabetes mellitus is reaching epidemic proportions among children and adolescents. School health fairs offer an opportunity to identify children with risk factors for the development of type 2 diabetes mellitus. This study identified selected risk factors (i.e., high-risk racial/ethnic group, obesity, elevated blood pressure, elevated casual blood glucose, elevated total cholesterol, and the presence of acanthosis nigricans) for development of type 2 diabetes mellitus in rural children with or without a family history of diabetes during annual school health fairs. A Heterogeneous Swine Show Circuit Drives Zoonotic Transmission of Influenza A Viruses in the United States.

Influenza pandemics are associated with severe morbidity, mortality, and social and economic disruption. Every summer in the United States, youths attending agricultural fairs are exposed to genetically diverse influenza A viruses (IAVs) circulating in exhibition swine, resulting in over 450 lab-confirmed zoonotic infections since 2010. Exhibition swine represent a small, defined population (∼1.5% of the U.S. herd), presenting a realistic opportunity to mitigate a pandemic threat by reducing IAV transmission in the animals themselves. Through intensive surveillance and genetic sequencing of IA

Influenza A(H3N2) Variant Virus Outbreak at Three Fairs - Maryland, 2017.

On September 17, 2017, the Maryland Department of Agriculture (MDA) was notified by fair and 4-H officials of ill swine at agricultural fair A, held September 14-17. That day, investigation of the 107 swine at fair A revealed five swine with fever and signs of upper respiratory tract illness. All five respiratory specimens collected from these swine tested positive for influenza A virus at the MDA Animal Health Laboratory, and influenza A(H3N2) virus was confirmed in all specimens by the U.S. Department of Agriculture National Veterinary Services Laboratory (NVSL). On September 18, MDA was not

Introduction, Evolution, and Dissemination of Influenza A Viruses in Exhibition Swine in the United States during 2009 to 2013.

UNLABELLED: The swine-human interface created at agricultural fairs, along with the generation of and maintenance of influenza A virus diversity in exhibition swine, presents an ongoing threat to public health. Nucleotide sequences of influenza A virus isolates collected from exhibition swine in Ohio (n = 262) and Indiana (n = 103) during 2009 to 2013 were used to investigate viral evolution and movement within this niche sector of the swine industry. Phylogenetic and Bayesian analyses were employed to identify introductions of influenza A virus to exhibition swine and study viral population d

Sun Smart Day: a pilot program for photoprotection education.

Excessive exposure to the sun's ultraviolet radiation (UVR) contributes to the etiology of melanoma and nonmelanoma skin cancers. Many behaviors that increase lifetime risk of skin cancer--sun exposure, sunburn, and lack of sun protection--occur early in childhood. A 1-day school-based skin cancer prevention effort--Sun Smart Day--was implemented and evaluated in three elementary schools to improve fourth-graders' knowledge, attitudes, and behaviors related to skin cancer prevention. A classroom-based skin cancer prevention lesson was compared to an interactive sun safety fair was vehicles for

Risk of exposure to Coxiella burnetii from ruminant livestock exhibited at Iowa agricultural fairs.

Coxiella burnetii is a zoonotic pathogen typically associated with clinical and asymptomatic infection in ruminant livestock. A re-emerging pathogen of significant public health importance, C. burnetii has caused recent epidemics in the United States and Europe, and public livestock exhibitions are increasingly scrutinized as a potential source of C. burnetii exposure. Although C. burnetii prevalence data among North American domestic ruminants are extremely limited, contemporary studies suggest that this pathogen is both geographically widespread and highly prevalent on a herd basis, especial

Librarians, Dentists, Journalists, and Undergrads: A Case Study Implementing a Journalism Teaching Hospital Within Marginalized Communities.

This study considers a practice-based learning project in which 173 undergraduates enrolled in a journalism class reported on oral health issues in marginalized communities. Health fairs at local libraries provided a starting place for students to cover oral health care, an underreported topic in the media. Three nonprofit journalism organizations partnered with the effort to publish student-produced content. Through written responses from all students and 30 in-depth interviews with professional journalists, librarians, health professionals, and students, this qualitative case study presents

Quantitative ultrasound of the calcaneus as a screening tool to detect osteoporosis: different reference ranges for caucasian women, african american women, and caucasian men.

The interpretation of results measured by quantitative ultrasound (QUS) of the heel depends on the population studied. We measured estimated bone mineral density (BMD) of the heel using the Hologic Sahara sonometer. People were studied at county fairs, health fairs, and churches. Subjects were not on treatments that would affect bone density, other than calcium supplementation. This included 823 Caucasian women, 131 African American women, and 301 Caucasian men. In contrast to women, for Caucasian men the squared term for age was not significant, and a straight line of decline was the best fit
