Malfunctions in distributed clinical decision support: 3 cases from a multi‑component clinical decision support system.

INTRODUCTION: Clinical decision support (CDS) is increasingly delivered through distributed architectures that connect electronic health record (EHR) workflows to external services and community partners. While these designs enable rapid deployment and interoperability, they also introduce new failure modes that can silently disrupt time‑sensitive care processes. Practical, real‑world descriptions of these distributed CDS malfunctions remain limited. CASE REPORT: We describe 3 malfunctions in a distributed CDS system used to support smoking cessation referrals. The issues reflected common dis

Igniting Harmonized Digital Clinical Quality Measurement through Terminology, CQL, and FHIR.

BACKGROUND: Electronic clinical quality measures (eCQMs) seek to quantify the adherence of health care to evidence-based standards. This requires a high level of consistency to reduce the effort of data collection and ensure comparisons are valid. Yet, there is considerable variability in local data capture, in the use of data standards and in implemented documentation processes, so organizations struggle to implement quality measures and extract data reliably for comparison across patients, providers, and systems. OBJECTIVE: In this paper, we discuss opportunities for harmonization within an

Data Interoperability in COVID-19 Vaccine Trials: Methodological Approach in the VACCELERATE Project.

BACKGROUND: Data standards are not only key to making data processing efficient but also fundamental to ensuring data interoperability. When clinical trial data are structured according to international standards, they become significantly easier to analyze, reducing the efforts required for data cleaning, preprocessing, and secondary use. A common language and a shared set of expectations facilitate interoperability between systems and devices. OBJECTIVE: The main objectives of this study were to identify commonalities and differences in clinical trial metadata, protocols, and data collectio

Navigating Technical Challenges in FHIR-Based Patient Reported Outcome Measures: Towards Interoperability with the MII PRO Core Dataset.

INTRODUCTION: Patient-Reported Outcome Measures (PROMs) are vital for capturing subjective clinical data, yet electronic PROM tools often suffer from fragmentation and siloed data storage. To facilitate cross-site data sharing, large-scale interoperability must be integrated into these digital tools. METHODS: We developed a HL7 Fast Healthcare Interoperability Resources (FHIR) system for Mobile Patient Survey (MoPat), which supports multiple versions and is designed to integrate PROMs into various FHIR-based infrastructures. To evaluate the system, we used the Medical Informatics Initiative (

The Health Service Bus: an architecture and case study in achieving interoperability in healthcare.

Interoperability in healthcare is a requirement for effective communication between entities, to ensure timely access to up to-date patient information and medical knowledge, and thus facilitate consistent patient care. An interoperability framework called the Health Service Bus (HSB), based on the Enterprise Service Bus (ESB) middleware software architecture is presented here as a solution to all three levels of interoperability as defined by the HL7 EHR Interoperability Work group in their definitive white paper "Coming to Terms". A prototype HSB system was implemented based on the Mule Open

Electronic health records and cardiac implantable electronic devices: new paradigms and efficiencies.

The anticipated advantages of electronic health records (EHRs)-improved efficiency and the ability to share information across the healthcare enterprise-have so far failed to materialize. There is growing recognition that interoperability holds the key to unlocking the greatest value of EHRs. Health information technology (HIT) systems including EHRs must be able to share data and be able to interpret the shared data. This requires a controlled vocabulary with explicit definitions (data elements) as well as protocols to communicate the context in which each data element is being used (syntacti

Clinical and financial effects of smart pump-electronic medical record interoperability at a hospital in a regional health system.

PURPOSE: The pharmacist-led implementation of a smart pump-electronic medical record (EMR) interoperability program at a hospital within a regional health system is described. SUMMARY: Smart pump-EMR interoperability involves a wireless, bidirectional connection by which infusion information transmitted by the EMR prepopulates infusion devices, reducing keystokes and opportunities for manual programming errors. The smart pumps transmit time-stamped infusion data to the EMR for nurse documentation. Use of interoperability technology forces the use of dose-error reduction software so that 100%

An Architecture for Semantically Interoperable Electronic Health Records.

Despite the increasing adhesion of electronic health records, the challenge of semantic interoperability remains unsolved. The fact that different parties can exchange messages does not mean they can understand the underlying clinical meaning, therefore, it cannot be assumed or treated as a requirement. This work introduces an architecture designed to achieve semantic interoperability, in a way which organizations that follow different policies may still share medical information through a common infrastructure comparable to an ecosystem, whose organisms are exemplified within the Brazilian sc

Fragmented Data, Fragmented Care: Strengthening Health Information Systems for Universal Health Coverage in Somalia.

Health information systems (HISs) are essential for achieving universal health coverage (UHC), yet Somalia's health sector remains constrained by fragmented data systems, weak interoperability, limited infrastructure, workforce shortages, and poor governance. This commentary examines the challenges undermining Somalia's digital health transformation and argues that fragmented information systems perpetuate fragmented healthcare delivery. Despite emerging reforms and adoption of digital platforms, such as DHIS2, progress toward UHC remains limited. This commentary draws on a narrative synthesis

Semantic Harmonization Workflow of Health Data: Application in the Post-COVID Network Netherlands.

Semantic interoperability in multi-source health datasets is a key barrier to data integration and reuse in biomedical research. FAIR principles promote standardization, but operationalizing them for heterogeneous clinical and patient-reported data remains challenging. We introduced a structured workflow that yields metadata-rich codebooks to support semantic interoperability. It integrates manual curation, expert-driven semantic annotation using biomedical ontologies and contextual suffixes for measurement nuances. The workflow was applied to the Post-COVID Network Netherlands (PCNN) dataset.

Using national electronic health care registries for comparing the risk of psychiatric re-hospitalisation in six European countries: Opportunities and limitations.

Psychiatric re-hospitalisation rates have been of longstanding interest as health care quality metric for planners and policy makers, but are criticized for not being comparable across hospitals and countries due to measurement unclarities. The objectives of the present study were to explore the interoperability of national electronic routine health care registries of six European countries (Austria, Finland, Italy, Norway, Romania, Slovenia) and, by using variables found to be comparable, to calculate and compare re-hospitalisation rates and the associated risk factors.
