Spatial health inequities among older adults in Thailand: a composite vulnerability index and relationship with local development.

BACKGROUND: Thailand is undergoing a rapid demographic transition toward an aging society, with substantial challenges in ensuring health equity for older adults. While national-level health policies have expanded coverage and services, spatial disparities in health risks and local development remain underexplored, particularly through integrative spatial frameworks. OBJECTIVE: This study aimed to examine the spatial distribution of health vulnerability across Thai provinces and its spatial association with local development. METHODS: Data were obtained from the 2024 nationwide community-based

Community governance and nutrition-oriented cancer care for women: policy pathways to reduce survival disparities.

Nutrition is increasingly recognized as a modifiable determinant of treatment tolerance, metabolic health, quality of life, and survivorship outcomes among women with cancer. However, nutrition-oriented cancer care is often delivered as an individual-level clinical recommendation, while many determinants of dietary behavior and nutritional vulnerability arise outside oncology clinics. Food insecurity, unequal food environments, limited dietitian availability, fragmented referral systems, low nutrition literacy, transport barriers, and gendered caregiving roles can prevent women with breast, ce

When is health adequate for older adults? Optimal standards, patient-defined thresholds, and the weight of inequality.

This perspective distinguishes two standards that are often used as if they were interchangeable. Optimal health is a standard built by medicine and expressed through guideline targets and biomarkers. Adequate health is the threshold of functioning that allows a person to live in a way they themselves experience as sufficient and dignified. Building on the concept of homeodynamic space as the biological substrate of adequate independence, on the older salutogenic tradition that asked what sustains health rather than only what causes disease, and on clinical evidence that aligning care with pat

Development of the Kunonga framework for operationalising approaches to health inequality and/or inequity evidence syntheses.

BACKGROUND: Health inequalities and inequities are shaped by intersecting social determinants and cumulative life-course experiences. However, conventional evidence synthesis methods often lack the conceptual and analytical tools to capture this complexity. This limits their ability to inform inequality and/or inequity-sensitive policy and practice. In response, we developed a methodological framework to support the systematic integration of intersectionality and life-course perspectives into evidence synthesis. METHODS: Framework development followed a three-phase process. First, a systematic

Dissecting racial disparities in multiple myeloma.

Multiple myeloma (MM) is a fatal plasma cell dyscrasia with a median overall survival of 5 to 10 years. MM progresses from the more common but often subclinical precursor states of monoclonal gammopathy of undetermined significance (MGUS), and smoldering multiple myeloma (SMM) to overt MM. There are large racial disparities in all stages of the disease. Compared with Whites, Blacks have an increased MGUS and MM risk and higher mortality rate, and have not experienced the same survival gains over time. The roots of this disparity are likely multifactorial in nature. Comparisons of Black and Whi

Marxist concepts of alienation in relation to health: a narrative review of the evidence.

Public health scholars are increasingly interested in the Marxist concept of alienation to explain the existence of health inequities. Marx's use of the term stresses the crucial role of political economic structural factors in creating alienation. This article reports on a narrative review of literature that used Marx's theory of alienation and focuses on the mental and/or physical health effects of alienation. Our search resulted in 34 relevant articles. Collectively, they demonstrate the ongoing relevance of the concept of alienation to the creation of health inequities in contemporary soci

Addressing Racial and Ethnic Inequities in Opioid Overdose Mortality: Strategies for Equitable Interventions and Structural Change.

PURPOSE OF REVIEW: This review synthetizes findings reflecting the increasing racial and ethnic inequities in opioid overdose mortality and emphasizes the necessity for tailored interventions as well as other policy-level and structural strategies to stem this trend. RECENT FINDINGS: Factors contributing to inequities in overdose mortality include changes in drug supply, persistent social-structural vulnerabilities stemming from structural racism, and inequities in access to medication for opioid use disorder and harm reduction services. Key strategies to address these inequities include the

Is there a health inequality in gambling related harms? A systematic review.

BACKGROUND: Here we present a systematic review of the existing research into gambling harms, in order to determine whether there are differences in the presentation of these across demographic groups such as age, gender, culture, and socioeconomic status, or gambling behaviour categories such as risk severity and participation frequency. PRIMARY AND SECONDARY OUTCOME MEASURES: Inclusion criteria were: 1) focus on gambling harms; 2) focus on harms to the gambler rather than affected others; 3) discussion of specific listed harms and not just harms in general terms. Exclusion criteria were: 1)

Inequality in America: the contribution of health centers in reducing and eliminating disparities in access to care.

Reducing and eliminating health status disparities by providing access to appropriate health care is a goal of the nation's health care delivery system. This article reviews the literature that demonstrates a relationship between access to appropriate health care and reductions in health status disparities. Using comprehensive site-level data, patient surveys, and medical record reviews, the authors present an evaluation of the ability of health centers to provide such access. Access to a regular and usual source of care alone can mitigate health status disparities. The safety net health cente

Exploring what works, for whom, under what circumstances to transform systems: realist synthesis protocol of four ongoing studies and literature addressing health inequalities.

INTRODUCTION: Health inequalities remain resistant to interventions that primarily target individual behaviour. Although systems approaches are increasingly promoted, their application in practice is often not well grounded in real-world settings. In this protocol paper, we present the approach we will take in an overarching project that synthesises the combined insights of four ongoing systems-based research projects on system-based approaches for reducing health inequalities in the Netherlands. By bringing together and comparing findings across diverse contexts, populations and interventions

Social inequalities in cancer in Germany: a call to action.

BACKGROUND: Social inequalities in cancer constitute a major public health challenge. A lower socioeconomic position (SEP) is consistently associated with higher exposure to cancer risk factors, lower participation in screening, more advanced stage at diagnosis, poorer survival, and adverse survivorship outcomes. In Germany, these inequalities remain insufficiently addressed in research and health policy. METHODS: This paper synthesises evidence and expert perspectives derived from a national workshop organised by the Cancer Epidemiology Working Group of the German Society for Epidemiology (D

Understanding cognitive ageing: individuals, institutions, and inequalities.

Population ageing has drawn growing attention to cognitive ageing as a major public health and social policy issue. However, cognitive ageing is marked by substantial heterogeneity, reflecting not only biological and genetic processes but also the cumulative influence of social, economic, and institutional conditions across the life course. This editorial introduces a Special Issue that advances a social determinants perspective on cognitive ageing by integrating life-course, inequality, and contextual approaches across diverse geographical settings. Drawing on longitudinal surveys, administra

Harms and Health Inequalities in the Night-Time Economy: A Qualitative Exploration of Professional Stakeholders' Experiences and Explanations.

Goods and services consumed in the night-time economy (NTE) are associated with adverse health outcomes. These outcomes are more prevalent in areas with greater deprivation and among minoritized population subgroups. This study was aimed at exploring the views of local authority stakeholders on the harms and health inequalities in the NTE. We focused on inequalities from alcohol and gambling, their relationship with the NTE and how different forms of power might underpin the inequalities generated. We interviewed 17 local authority and third sector stakeholders from two case study areas (Swind

Health disparities among incarcerated populations: a crucial laboratory and pathology review.

BACKGROUND: The United States notoriously has one of the highest rates of incarceration in the world, yet scant attention to the health care needs of those incarcerated exists within laboratory medicine and pathology training and education. This article explores health disparities among incarcerated and released individuals regarding diagnostic laboratory testing and pathology services. METHODS: A literature search was conducted for articles published between 2002 and 2023 using keywords including "healthcare," "incarcerated," "laboratory services," "pathology services," and "health insurance

Disparities in Cancer Mortality Worldwide: A Novel Metric for Measuring Global Disparities and Prioritizing Cancer Control Efforts.

PURPOSE: Cancer incidence is rising worldwide and estimated to double by 2040. A systematic method of allocating resources and prioritizing cancer control efforts is needed. We aimed to develop and test a simple metric to quantify disparities in cancer mortality. METHODS: We extracted country-specific incidence and mortality rates for 33 cancers from 185 countries using data from Global Cancer Observatory (GLOBOCAN) 2020. Mortality-to-incidence ratios (MIRs) were calculated for each cancer in every country. Delta MIRs (dMIRs) were calculated as the difference between a country's MIR and the M

Nutrition inequities in the Deaf community: a call for inclusive public health action.

Research shows that understanding the nutritional status and eating habits of people with disabilities is essential for creating effective preventive healthcare strategies. Communication barriers in healthcare settings, low health literacy and socioeconomic inequities are among the challenges this community faces. These difficulties can lead to poor nutritional knowledge, food insecurity and chronic diseases. Deaf individuals also experience higher rates of undernutrition, obesity and micronutrient deficiencies, primarily due to limited access to linguistically appropriate nutrition education.

From Tragedy To Opportunity: Hospital-based Violence Intervention Programs May Address Root-Cause Health Disparities for Violent Traumatic Injury Patients.

Violence is a growing public health issue that disproportionately affects low-income and racialized communities across North America. While trauma centers appropriately respond to acute violent injuries, many patients are discharged back into the same environments that put them at risk. Hospital-based Violence Intervention Programs (HVIPs) offer opportunities for trauma care to address the upstream root causes of violence. These programs engage patients during critical "teachable moments," connecting them to staff with lived experience, along with social workers and community partners, providi
