[The importance of hearing-aid testing by the otorhinolaryngologist].

In a period of 16 months, 312 patients suffering from impaired hearing were supplied with hearing aids after undergoing conventional hearing tests conducted by acousticians. During the hearing follow-up tests, it was found that 111 patients (35.5%) had been optimally supplied with hearing aids and that for 98 hard-of-hearing persons (31.4%) merely fine adjustments had to be made on the hearing aids. In some cases a second bore had to be drilled into the ear mould. We had to completely refit 103 patients (33.0%) with hearing aids, since the previously fitted devices no longer complied with the

Impact of Genetic Testing on Hearing Interventions.

OBJECTIVE: Clinical guidelines recommend genetic testing when evaluating congenital and late-onset sensorineural hearing loss (SNHL). Genetic diagnoses can provide parents additional information regarding anticipated hearing loss progression, comorbid conditions, and family planning. Additionally, obtaining a genetic diagnosis may increase parental acceptance of hearing loss and subsequent pursuit of intervention. This study evaluates the association between genetic diagnoses and hearing loss intervention. METHODS: We included children ages 0-18 years with SNHL who were hearing aid or cochlea

What is the role of the distraction test of hearing?

The use of the distraction test as a screen for hearing loss in infancy was reviewed in the Bristol and District Health Authority. Inaccuracies in data recording were found which challenged the screening and referral rates produced by the National Child Health Computer. Representative samples of children failing the distraction test and children needing hearing aids were followed up through the screening process. Of 130 children failing the test, only 46 (35%) had moderate conductive hearing loss and none had a severe loss. Of 34 children with sensorineural hearing loss (SNHL) requiring hearin

[Hearing tests in practice : Audiometric masking in pure-tone and speech audiometry].

Correct execution of hearing tests is essential for audiologic diagnostics and selection of treatment by the ENT physician. Especially in the case of pure-tone and speech audiometry, incorrectly performed audiometric masking can lead to false measurement results. Often, hearing that is too good is feigned by overhearing in the contralateral ear. Herein, a masking strategy is described by which errors in the execution of masking can be recognized and corrected. The aim of this paper is to identify and prevent the most common errors made during the masking process in pure-tone and speech audiome

Diagnostic Accuracy of Tuning Fork Tests for Hearing Loss: A Systematic Review.

Objective (1) To determine the diagnostic accuracy of tuning fork tests (TFTs; Weber and Rinne) for assessment of hearing loss as compared with standard audiometry. (2) To identify the audiometric threshold at which TFTs transition from normal to abnormal, thus indicating the presence of hearing loss. Data Sources PubMed, Ovid Medline, EMBASE, Web of Science, Cochrane, and Scopus and manual bibliographic searches. Review Methods A systematic review of studies reporting TFT accuracy was performed according to a standardized protocol. Two independent evaluators corroborated the extracted data an

Smartphone-Facilitated In-Situ Hearing Aid Audiometry for Community-Based Hearing Testing.

OBJECTIVES: Hearing loss prevalence is increasing, with an estimated 2.5 billion people affected globally by 2050. Scalable service delivery models using innovative technologies and task-shifting are World Health Organization priorities to improve access to hearing care, particularly in low- and middle-income countries. Smartphone-facilitated audiometry in the community using hearing aids covered by noise-attenuating ear cups ("in-situ") could support more accessible hearing care when provided by less trained individuals such as community health workers (CHWs). This study aimed to determine th

Validation of the Thai five-minute hearing test to screen hearing loss in the community.

BACKGROUND: Hearing impairment affects communication, social interactions, and quality of life. Audiometry is the gold standard method for hearing assessment. However, it is impractical in a country with limited resources. This study aimed to validate the Thai version of the Five-Minute Hearing Test (Thai-FMHT) to screen hearing loss in the community. METHODS: This prospective cohort study was conducted in Phu Wieng district, Khon Kaen Province, Thailand during July to September 2011. Subjects who were older than 18 years, could read or understand the Thai language, and wanted to participate

Application of the TEN test to hearing-impaired teenagers with severe-to-profound hearing loss.

Hearing impairment is often associated with damage to the hair cells of the cochlea. An area of the cochlea with complete loss of function of inner hair cells is known as a 'dead region'. Dead regions can be identified by measuring detection thresholds for pure tones in quiet and in threshold-equalizing noise (TEN). So far, the TEN test has only been used to identify dead regions in adults with moderate-to-severe hearing impairment. The aim of this study was to assess problems in applying the TEN test to teenagers with longstanding severe-to-profound sensorineural hearing impairment, and to as

[New hearing test for functional deafness--the procedure to estimate the genuine hearing threshold].

This report describes a new procedure for examining functional deafness with binaural sound stimulation. This new hearing test can estimate the genuine pure tone hearing threshold quantitatively at every frequency by using the ordinary audiometer. In the case of the nonorganic deafness, even if the hearing disorders are bilateral or hemilateral, we believe that measuring auditory threshold separately, causes the deterioration of the threshold. Therefore, this procedure is designed so that the subject may not be aware of testing the auditory acuity of each ear, and utilizes the response of the

Automated hearing tests: applying the otogram to patients who are difficult to test.

OBJECTIVE: The Otogram is an automated audiometer capable of determining air and bone conduction thresholds with masking when appropriate. The manufacturer claims that testing can be done in a quiet physician's consultation room without a sound-treated booth. We aimed to test the validity of the Otogram on "difficult-to-test" patients, all of whom require masking. METHODS: Twenty-eight difficult-to-test patients underwent three audiograms: two by an audiologist and one by the Otogram. First, audiograms performed by the audiologists were compared, establishing test-retest reliability. Second,

Toward a Diagnostic Test for Hidden Hearing Loss.

Cochlear synaptopathy (or hidden hearing loss), due to noise exposure or aging, has been demonstrated in animal models using histological techniques. However, diagnosis of the condition in individual humans is problematic because of (a) test reliability and (b) lack of a gold standard validation measure. Wave I of the transient-evoked auditory brainstem response is a noninvasive electrophysiological measure of auditory nerve function and has been validated in the animal models. However, in humans, Wave I amplitude shows high variability both between and within individuals. The frequency-follow

Hearing in the elderly: a test protocol and preliminary results.

Hearing deteriorates with age. ISO 7029 (2000) presents data on hearing threshold as a function of sex and age up to 70 years. There is a need for an extension of these data beyond 70 years. There is also a need for testing the central component of hearing in the elderly. A test protocol was established, and groups of subjects aged 60 to > or = 90 were selected. After a questionnaire and an ear examination, the following tests were performed: air-conduction pure-tone audiometry in the frequency range of 125-16 kHz, tympanometry, speech recognition score in quiet conditions, transient evoked ot

[Otoacoustic emissions. A futuristic objective hearing test].

In a third of all persons with normal hearing, the inner ear emits sounds that are caused by movements of the hair cells. By applying an acoustic stimulus, otoacoustic emissions can be provoked in all healthy ears. These transitory evoked otoacoustic emissions (TEOAE) and the distortion products of otoacoustic emissions (DPOAE) can be utilized by two different procedures for testing hearing. The latter parameter reflects possible damage to the inner ear. Such hearing tests are quick, harmless, and can be performed irrespective of patient age. For this reason, they are particularly suitable for

Temporary loss of hearing after a glycerin test.

The glycerin test has been proposed as an adjunct in the diagnosis of Meniere's disease. Complications of the test are rare and usually minor. We describe a case in which a 20- to 40-dB hearing loss developed in the uninvolved ear of a patient during a standard glycerin test. He made a complete recovery within three days after the test. We discuss the possible mechanisms of the loss.

OBJECTIVE: To survey public attitudes about self-administered home-based hearing tests and fit-at-home hearing aids. DESIGN: Online survey enquiring about willingness to try the above. Explanatory variables included sex, age, and education. STUDY SAMPLE: A representative sample of 2135 UK adults. RESULTS: Only 13% of respondents were unwilling to undertake a home-based hearing test. Perceived benefits of home-based testing were associated with convenience and ease, and its use as a precursor to a formal hearing test. Main concerns were associated with accuracy and reliability. Regarding hea
