Predicting neurological recovery following cardiac arrest based on dynamic brain-heart coupling.

BACKGROUND: Cardiac arrest (CA) often results in severe neurological injury, with both the central (CNS) and autonomic nervous systems (ANS) playing critical roles in recovery. Brain–heart coupling (BHC) and heartbeat-evoked potentials (HEP) reflect CNS and ANS interactions, yet their temporal evolution after return of spontaneous circulation (ROSC) and prognostic relevance remain unclear. OBJECTIVE: This study aims to examine frequency-specific variations in BHC and HEP within the first 96 h after ROSC in cardiac arrest patients and evaluate their prognostic value for neurological recovery. Urinary Cell Cycle Arrest Biomarkers and Diuretic Efficiency in Acute Heart Failure.

INTRODUCTION: This study aimed to evaluate the association between the NephroCheck® test AKIRisk® score, diuretic efficiency (DE), and the odds of worsening kidney function (WKF) within the first 72 h of admission in patients hospitalized for acute heart failure (AHF). METHODS: The study prospectively enrolled 125 patients admitted with AHF. NephroCheck® test was obtained within the first 24 h of admission. DE was defined as net fluid urine output per 40 mg of furosemide equivalents. RESULTS: The median AKIRisk® score was 0.11 (IQR 0.06-0.34), and 38 (30.4%) patients had an AKIRisk® score &g

Congestive heart failure and respiratory arrest secondary to methyldopa-induced hemolytic anemia.

We report a case of methyldopa-induced hemolytic anemia in a 64-year-old woman who presented to the emergency department in congestive heart and respiratory failure. She subsequently suffered a respiratory arrest and required endotracheal intubation and brief CPR. After the initial evaluation revealed anemia and a 3+ positive Coombs' test, a diagnosis of methyldopa-induced hemolytic anemia was entertained. Withdrawal of the drug and supportive therapy resulted in the complete recovery of the patient. Emergency physicians should be aware of this syndrome and be able to diagnose it early in its

Cardiac imaging for the prediction of sudden cardiac arrest in patients with heart failure.

The identification of heart failure (HF) patients at risk for arrhythmic sudden cardiac arrest (SCA) is a major challenge in the cardiovascular field. In addition to optimal medical treatment for HF, implantable cardioverter defibrillator (ICD) is currently recommended to prevent SCA in patients with reduced left ventricular ejection fraction (LVEF). The indication for an ICD implantation, in addition to HF etiology, New York Health Association (NYHA) class and life expectancy, mainly depends on LVEF value at echocardiography. However, the actual role of LVEF in the prediction of SCA has recen

Surviving a Broken Heart: A Case of Intraoperative Cardiac Arrest and Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy manifests as global myocardial hypokinesis, a rare challenge for anesthesia practitioners. This report discusses a case in which a seemingly healthy patient presented for open abdominal hysterectomy and experienced intraoperative cardiac arrest requiring cardiopulmonary resuscitation. Takotsubo cardiomyopathy was diagnosed following resuscitation. This case examines risk factors and the intraoperative and postoperative management of a patient with Takotsubo cardiomyopathy.

Resuscitation for out-of-hospital cardiac arrest in adults with congenital heart disease.

AIMS: Adult congenital heart disease (ACHD) patients are at increased risk of sudden cardiac death and out-of-hospital cardiac arrest (OHCA). Currently, insufficient data exist on outcome, causes and circumstances of OHCA of ACHD patients resuscitated for OHCA. We investigate these parameters in ACHD patients in comparison to OHCA in the general population. METHODS AND RESULTS: We identified ACHD patients with OHCA by linking data from a Dutch nationwide registry of ACHD patients (CONCOR, n = 15,727), and ARREST, a cohort of OHCA cases (n = 17,868). 62 ACHD patients with OHCA were identified.

The heart of the matter: utility of ultrasound of cardiac activity during traumatic arrest.

BACKGROUND: The clinical utility of determining cardiac motion on ultrasound has been reported for patients presenting in pulseless medical cardiac arrest. However, the relationship between ultrasound-documented cardiac activity and the probability of surviving pulseless electrical activity has not been examined in populations with trauma. We hypothesized that cardiac activity on ultrasound predicts survival for patients presenting in pulseless traumatic arrest. METHODS: We conducted a retrospective analysis at our university-based urban trauma center of adult patients with trauma, who were p

New paradigms in the prevention of sudden cardiac arrest and heart failure treatment.

Congestive heart failure is a very real public health issue not only in the United States, but worldwide. Mortality in patients with congestive heart failure is typically either sudden cardiac death or pump failure. Paradoxically, patients with less severe heart failure are at higher relative risk of sudden cardiac death. Defining which patients are best treated with implantable defibrillators and resynchronization is the purpose of this review.

Comparison of heart rate variability in survivors and nonsurvivors of sudden cardiac arrest.

Imbalances in autonomic nervous system function have been posed as a possible mechanism that produces ventricular fibrillation and sudden cardiac arrest in patients with cardiovascular disease. Heart rate (HR) variability is described in survivors and nonsurvivors of sudden cardiac arrest within 48 hours after resuscitation using time and frequency domain analytic approaches. HR data were collected using 24-hour ambulatory electro-cardiograms in 16 survivors and 5 nonsurvivors of sudden cardiac arrest, and 5 control subjects. Survivors of sudden cardiac arrest were followed for 1 year, with re

Risk factors and outcomes of sudden cardiac arrest in pediatric heart transplant recipients.

BACKGROUND: Sudden cardiac arrest (SCA) is a prevailing cause of mortality after pediatric heart transplant (HT) but remains understudied. We analyzed the incidence, outcomes, and risk factors for SCA at our center. METHODS: Retrospective review of all pediatric HT patients at our center from January 1, 2009 to January 1, 2021. SCA was defined as an abrupt loss of cardiac function requiring cardiopulmonary resuscitation and/or mechanical circulatory support (MCS). Events that occurred in the setting of limited resuscitative wishes, or while on MCS were excluded. Patient characteristics and ri

Pediatric Post-Cardiac Arrest Care: A Scientific Statement From the American Heart Association.

Successful resuscitation from cardiac arrest results in a post-cardiac arrest syndrome, which can evolve in the days to weeks after return of sustained circulation. The components of post-cardiac arrest syndrome are brain injury, myocardial dysfunction, systemic ischemia/reperfusion response, and persistent precipitating pathophysiology. Pediatric post-cardiac arrest care focuses on anticipating, identifying, and treating this complex physiology to improve survival and neurological outcomes. This scientific statement on post-cardiac arrest care is the result of a consensus process that include

Hypoglycemia detected during cardiac arrest of a non-diabetic patient with heart failure.

Hypoglycemia in non-diabetic patients with heart failure is a rare finding. It is thought to be caused by hepatic dysfunction secondary to chronic passive congestion, and reduced gluconeogenesis. In this report we present a 23-year-old man with a history of Duchenne muscular dystrophy hospitalized for decompensated heart failure and implantable cardioverter defibrillator shock. Laboratory examination on admission showed slightly elevated levels of hepatic enzymes. Despite the therapy for heart failure the patient's clinical status deteriorated, and sinus bradycardia and then cardiac arrest occ

[Sudden cardiac arrest as the first symptom of coronary heart disease--case report].

We describe the patient at whom sudden cardiac arrest was the first symptom of coronary heart disease. In the paper, we describe factors which influence the effectiveness of resuscitation in out of hospital cardiac arrest. Applying the cardiac massage by a bystander, the presence of defibrillation rhythm and short period of time to the first defibrillation, determined the survival of the patient.

From ECPR for refractory cardiac arrest to heart transplant and neurologically intact survival.

A 58-year-old male wearing a wearable cardioverter defibrillator due to severe dilated cardiomyopathy with multi-vessel coronary disease presented a cardiac arrest (CA) with ventricular fibrillation refractory to conventional cardiopulmonary resuscitation (CPR). The decision was made to expand CPR to extracorporeal cardiopulmonary resuscitation (ECPR) using extracorporeal life support in the prehospital setting. ROSC was obtained after pump activation, and the patient was admitted to the intensive care unit, sedated and mechanically ventilated while under ECPR support. Despite an initial hemor
