Heart, heart-lung, and lung transplantation in the first year of life.

Seventeen infants less than 1 year of age have undergone heart (12), heart-lung (3), and lung (2) transplantation for end-stage cardiopulmonary disease. The infants undergoing heart transplantation had a mean age of 4.5 months (range, 19 days to 12 months) with the diagnosis of cardiomyopathy in 4 and congenital heart disease in 8. Four of the 8 patients (50%) had hypoplastic left heart syndrome. Actuarial survival at 1 and 2 years was 74% and compared favorably with the survival of older children at 1 and 2 years of 82% and 69%. The linearized rejection rate was less in infants as compared wi

Technique of combined heart-lung transplantation.

Heart-lung transplantation was, for many years, conceptualized as a possible treatment for patients with combined end-stage cardiac and pulmonary disease. As experience grew with heart transplantation, particularly in the 1970s, the difficulties of performing the orthotopic operation in patients with fixed pulmonary hypertension became apparent. This further impetus for combined heart-lung transplantation led to successful animal experiments in the late 1970s, and the first successful heart-lung transplant operation was performed in 1981. There has been significant evolution in the operative t…

Shifting pneumothorax after heart-lung transplantation.

Heart-lung transplantation involves the total replacement of two of the most complex organs of the thoracic cavity. This procedure is usually reserved for patients with failure of both systems, such as in primary pulmonary hypertension or chronic Eisenmenger physiology. The en bloc replacement of the heart and lungs leaves an open communication between the two sides of the thorax that may allow air or fluid to shift from one side to the other. To evaluate this possibility, the authors reviewed the chest radiographs of 25 heart-lung transplant recipients for signs of rapidly changing pneumothor…

[Heart-lung transplantation. Initial clinical experience].

PURPOSE: To assess this initial clinical experience with heart-lung transplantation in Brazil. METHODS: Four patients underwent heart-lung transplantation from December 1988 to March 1990, one patient with cardiomyopathy and high pulmonary resistance, two patients with primary pulmonary hypertension and another with pulmonary silicosis. The heart lung blocks were harvested from the donor using cardiopulmonary bypass and deep hypothermia. They were transplanted to the recipient by anastomosing the tracheae, right atrium or venae cavae, and the ascending aorta. RESULTS: The second patient died…

Heart-lung transplantation for cystic fibrosis.

13 patients with severe lung disease and cor pulmonale from cystic fibrosis were accepted for heart-lung transplantation (HLT). 6 have had the operation, of whom 5 are well, with normal lung function, 3-29 months after operation. 1 patient died from adult respiratory distress syndrome after reoperation to control persistent chest-wall bleeding: at necropsy, this patient proved to have cirrhosis. Respiratory tract infections and acute lung rejection after HLT for cystic fibrosis were no more common than in other HLT patients. Of the 7 patients for whom suitable donor organs were not found, 3 di…

Recipient selection for heart-lung transplantation.

Cardiac transplantation has emerged as a standard mode of clinical therapy for end-stage heart failure, while heart-lung transplantation is still an experimental rather than a standard mode of clinical therapy for end-stage heart and lung disease. Since clinical application of heart-lung transplantation began in March 1981, there has been continual improvement in donor care, surgery, post-operative management, and the diagnosis and treatment of lung and heart rejection. According to our 6-year clinical experience, both patients with Eisenmenger's physiology and patients with primary pulmonary …

Bronchoalveolar lavage in heart-lung transplantation.

Distinguishing lung rejection from infection in patients who have undergone heart-lung transplantation is difficult. Since bronchoalveolar lavage has been safely used to investigate other pulmonary conditions, its safety and the value of the cellular data obtained were evaluated on 44 occasions in ten heart-lung transplant recipients. This study established that bronchoalveolar lavage is safe after heart-lung transplantation, that it reliably diagnoses infection, and lastly that serial lavages are valuable in finding subclinical Pneumocystis carinii infections. The study failed to distinguish …

Pregnancy after heart and lung transplantation.

Patients awaiting transplantation should be counseled regarding posttransplant contraception and the potential adverse outcomes associated with posttransplant conception. Pregnancy should be avoided for at least 1-2 years post transplant to minimize the risks to allograft function and fetal well-being. Transplant patients, particularly lung transplant recipients, have an increased risk of maternal and neonatal pregnancy-related complications, including prematurity and low birth weight, postpartum graft loss, and long-term morbidity and mortality compared to other solid-organ recipients. [Immunosuppression after heart and lung transplantation].

In the last two decades heart and lung transplantation have evolved to an accepted treatment of endstage heart and lung failure. A differentiated immuno-suppressive regimen is the long term basis of successful transplantation and must be adopted to the individual patient. Rejection and immunosuppression with potential side effects must be balanced to achieve optimal outcome in each patient. In the long-term follow up an efficient diagnosis and treatment of rejection probably also results in reduction of chronic organ failure and improvement of general postoperative results. In cases of vasculo

Heart-lung transplantation: Cardiac clinicopathological correlations.

Reports on heart-lung transplantation emphasize the pathology of the transplanted lungs. This study is a clinicopathological assessment of cardiac pathology in the hearts transplanted as part of the combined heart-lung block. Seventy-five consecutive heart-lung transplants (H-LTx) performed between 1981 and 1989 were studied. Endomyocardial biopsy, autopsy and clinical data were analyzed for information on cardiac rejection, graft coronary disease, transplant survival and the presence of obliterative bronchiolitis and compared with controls. The controls consisted of 391 heart transplants (HTx…

Heart-lung transplantation in situs inversus.

Reconstruction was accomplished in a 2 heart-lung recipients with situs inversus resulting in a left-sided systemic venous atrium. We created a large common atrium that was closed on the left side, leaving an atrial cuff on the inferior right quadrant. To this we anastomosed the donor right atrium, which had been opened laterally between the cavae. This resulted in some clockwise rotation of the ventricles and anterior positioning of the apex. The right pulmonary veins passed superior to the atrial anastomosis and posterior to the donor right atrium. Cardiopulmonary function was excellent in b

Simplified technique of heart-lung transplantation.

Well-known complications of heart-lung transplantation include mediastinal bleeding and phrenic nerve injury. Conventional technique places the hila behind the phrenic nerves. We have placed the hila in front of the phrenic nerve in our last 10 patients, using direct caval anastomoses when feasible. This minimizes traction on and dissection around the phrenic nerves, and allows anterior rotation of the heart-lung block for easier hemostasis of the posterior mediastinum after implantation.

Improved survival after heart-lung transplantation.

Sixty-two patients underwent 65 heart-lung transplant operations before April 1989. Thirty of these patients (operated on March 1981 to February 1986) were given cyclosporine and prednisone for immunosuppression. These 30 patients (group 1) are compared with the 32 most recent patients (group 2) treated with cyclosporine, prednisone, and azathioprine. Patient characteristics (group 1 versus group 2) were similar including age (mean 32 versus 26 years) and indication for operation (51% versus 38%, primary pulmonary hypertension; 46% versus 45%, Eisenmenger complex; 3% versus 17%, other). Surgical management of heart-lung transplantation.

Using cyclosporin A, long-term survival after heart-lung transplantation became possible. The drug blocks the immune system more selectively and leaves the tracheal wound healing unimpaired. Since 1981, 501 clinical cases have been collected by the registry of the International Society for Heart Transplantation. Candidates for heart-lung transplantation reveal signs of irreversible heart and lung diseases that may have been caused by cardiac lesions (valvular diseases, Eisenmenger reaction due to congenital malformations) or by pulmonic disorders (primary pulmonary hypertension, emphysema, fib…

Early experience of heart-lung transplantation.

We report our experience of heart-lung transplantation for the treatment of children with terminal respiratory disease. Between May 1987 and October 1988 we performed heart-lung transplantation in five children under the age of 16 (age range 11-15). All the patients were severely disabled by dyspnoea and hypoxia. Two had primary pulmonary hypertension, two cystic fibrosis, and one had Eisenmenger's syndrome. All five children are alive and well five to 17 months after operation and have returned to activities normal for their age. Three of the five patients had episodes of infection after oper…

Psychiatric morbidity in patients undergoing heart, heart and lung, or lung transplantation.

OBJECTIVES: To determine the rate of psychiatric disorder in people undergoing heart and/or lung transplantation; to identify the associations of psychiatric disorder in this group. METHOD: Preoperative assessments were carried out on an 18-month sample of consecutive admissions to a regional unit for heart and lung transplantation in the UK. Assessment included psychiatric morbidity, sexual dysfunction, quality of life, and demographic and clinical characteristics. RESULTS: Seventy-six of 79 eligible subjects took part in the assessment. Thirty (39%) were suffering from a psychiatric disord

Transplantation of the heart and lungs in children.

Heart transplantation in children with complex congenital heart disease or cardiomyopathies has become an effective form of therapy for patients with no other surgical options. Because pediatric pulmonary transplantation is a relatively new procedure, long-term follow-up data are not yet available. However, the intermediate-term results of pediatric lung transplantation appear similar to those for heart and lung transplantation. Children represent a particularly difficult patient group for transplant of thoracic organs, and recipients must be selected carefully. Additionally, physicians must p

Pathology of the transplanted heart and lung.

Combined heart-lung transplantation is now being carried out in many centers around the world. Single-lung and double-lung transplantation are being done in only a few centers and are still considered to be experimental procedures. Combined heart-lung transplantation has been performed successfully in adults and in small children and offers a definite therapeutic alternative to such diseases as severe congenital heart disease with pulmonary hypertension, cystic fibrosis, and primary pulmonary hypertension. At present, the survival statistics for this procedure are similar to those for heart tr…

Lung Retransplantation after Heart-lung Transplantation: A Case Report.

Lung retransplantation after prior heart-lung transplantation is uncommon and presents significant surgical and immunologic challenges. We report the case of a 24-year-old male with a history of congenital heart disease and Eisenmenger syndrome who underwent heart-lung transplantation in January 2018. While his cardiac function remained stable postoperatively, his pulmonary function declined 2 years later due to chronic lung allograft dysfunction (CLAD), confirmed by a 17% drop in forced expiratory volume in 1 second (FEV1)/forced vital capacity (FVC) ratio, recurrent infections, and positive …

[Lung and heart-lung transplantation: indications, complications and prognosis].

Lung- and heart-lung transplantation has become a serious therapeutic option for terminally ill patients with chronic pulmonary failure. Due to the still rather high risk of acute rejection, severe pulmonary or systemic infection, and the relatively demanding postoperative care, this treatment can only be considered for patients where inspite exhausted conservative options for therapy quality of life and prognosis are not acceptable. Not only due to lack of donors single-lung transplantation has developed to be the favorite treatment for patients with interstitial lung disease (idiopathic, sar…
