Are Helicobacter pylori differences important in the development of Helicobacter pylori-related diseases?

Helicobacter pylori colonises the stomach of man and induces a strong mucosal inflammation and a local and systemic immune response. Differences in virulence characteristics of Helicobacter pylori isolates can account for different clinical outcomes of infection. Most determinants of Helicobacter pylori pathogenicity factors are present in all isolates examined; some are present only in or expressed more intensively by certain strains. Many enzymes, i.e., urease, mucinase, phospholipases, alcohol dehydrogenase, neuraminidase, etc. could promote tissue erosion and ulceration by destroying the i

[Perspective of Helicobacter pylori research: molecular pathogenesis of Helicobacter pylori virulence factors].

Helicobacter pylori (H. pylori) causes chronic gastritis in human stomach, a minority of which progress to peptic ulcer disease, atrophic gastritis, or gastric malignancies. Clinical outcomes of H. pylori infection has been shown to depend on the variability of H. pylori virulence factors, host susceptibility, environmental factors and their interactions. This review provides an update on the molecular pathogenesis of H. pylori infection, focused on H. pylori virulence factors, H. pylori-gastric epithelium interactions, and modulation of host cell signaling. Understanding of H. pylori molecula

Bacteriophages of Helicobacter pylori.

The bacterium Helicobacter pylori colonize the stomach in approximately half of the world's population. Infection with this bacterium is associated with gastritis, peptic ulcer, adenocarcinoma, and gastric mucosa-associated lymphoid tissue lymphoma. Besides being a pathogen with worldwide prevalence, H. pylori show increasingly high antibiotic resistance rates, making the development of new therapeutic strategies against this bacterium challenging. Furthermore, H. pylori is a genetically diverse bacterium, which may be influenced by the presence of mobile genomic elements, including prophages.

[Helicobacter pylori and Arteriosclerosis].

Helicobacter pylori (H. pylori) infection-related diseases are known to include gastritis, gastric and duodenal ulcer, gastric cancer, gastric MALT lymphoma, idiopathic thrombocytopenic purpura, iron-deficient anemia, urticaria, reflux esophagitis, and some lifestyle-related diseases. It is indicated that homocysteine involved with arteriosclerosis induces lifestyle-related diseases. Homocysteine is decomposed to methionine and cysteine (useful substances) in the liver, through the involvement of vitamin B₁₂ (VB₁₂) and folic acid. However, deficiency of VB₁₂ and folic acid induces an increase

Helicobacter pylori and inflammation.

Helicobacter pylori is a spiral-shaped, flagellated, microaerophilic Gram-negative bacillus which colonizes the gastric mucosa of more than 50% of human population. The chronic infection generates a state of inflammation, which however is asymptomatic in the majority of the subjects. Nevertheless, in a subset of the H. pylori-infected population, the gastric inflammation may evolve toward chronic gastritis, peptic ulcer, gastric mucosa-associated lymphoid tissue (MALT) lymphoma, and gastric cancer. In brief, the stages of the H. pylori colonization are the following: crossing of the gastric mu

Dyspepsia and Helicobacter pylori.

Dyspepsia is defined as a chronic or recurrent pain centered in the upper abdomen. Dyspeptic symptoms have a high prevalence in the population and represent a common reason for patients to see a primary care physician. Therefore, dyspepsia displays a major health care burden and produces high socioeconomic costs. The etiology of dyspeptic symptoms is various and complex and has opened a wide spectrum of putative mechanisms. In a subset of patients dyspeptic symptoms are likely to originate from Helicobacter pylori infection. Population-based studies have demonstrated that H. pylori is detected

Helicobacter pylori and probiotics.

Helicobacter pylori infection, a highly prevalent pathogen, is a major cause of chronic gastritis and peptic ulcer and a risk factor for gastric malignancies. Antibiotics-based H. pylori eradication treatment is 90% effective. However, it is expensive and causes side effects and antibiotic resistance. Probiotics could present a low-cost, large-scale alternative solution to prevent or decrease H. pylori colonization. A literature search of the MEDLINE database (1966-2006) has been performed selecting all in vitro, animal, and human fully published English-language studies dealing with H. pylori

Treatment of Helicobacter pylori.

The efficacy of established Helicobacter pylori regimes needs to be reviewed. In view of drug resistance, side effects, and compliance and expense of therapy, treatment failure is increasing and second-line treatment strategies need to be developed. A simulation model suggested by the Cochrane review group showed that H. pylori eradication is cost-effective for duodenal and gastric ulcer long-term. The duration of eradication therapy continues to be controversial. In Europe and other parts of the world, 7-day triple regimes are used, whereas guidelines from the United States recommend 10-14 da

Helicobacter pylori and rosacea.

Recent reports have suggested an increased prevalence of Helicobacter pylori infection in patients with rosacea, with some evidence of dermatological improvement in patients treated with antibiotics for this infection. Our study investigates the prevalence of H. pylori infection in rosacea patients in Kerman. Serological examination was done for 29 patients with classical identification of rosacea using the enzyme-linked immunosorbent assay IgG antibody method. Comparison of antibody titres with those of a control group revealed that the prevalence of positive serological tests for H. pylori w

Helicobacter pylori and dyspepsia.

It is clear that non-ulcer (or functional) dyspepsia is a heterogeneous syndrome that includes a subset of patients with unrecognized gastroesophageal reflux. Patient heterogeneity combined with inadequate study methodology has led to enormous confusion in interpreting the relationship between Helicobacter pylori and non-ulcer dyspepsia. The possibility that H. pylori is associated with gastroesophageal reflux disease may explain, in part, the difficulty in establishing a link between non-ulcer dyspepsia and H. pylori infection. It is unclear whether the prevalence of H. pylori is increased in

[Dyspepsia and Helicobacter pylori].

Since Helicobacter pylori (Hp) was first isolated in 1983, much work has been carried out on the pathogenic effects of this organism. Hp infection is common in humans and currently is the most important etiologic agent in the development of chronic active gastritis, gastric and duodenal ulcers, carcinoma and Malt-lymphoma of the stomach. Moreover Hp infection has also been associated with various extradigestive diseases. At present, a role of Hp infection in dyspepsia is discussed. Dyspepsia is defined by persistence of pain, burning or discomfort localised to the upper abdomen; some authors i

Helicobacter pylori: a review.

Helicobacter pylori is a gram-negative bacterium that was first isolated in 1982. In the years following its discovery, H. pylori infection in humans has been shown to be associated with gastritis, peptic ulcer disease, and gastric carcinoma, as well as other, nongastrointestinal disorders. The epidemiology, transmission, and virulence factors of this bacteria have been an area of intense study. Successful treatment improves cure rates of gastritis and ulceration of the stomach and duodenum. Treatment with antimicrobials also decreases the recurrence rates of these diseases. Clinicians have nu

The nature of Helicobacter pylori.

Helicobacter pylori is an important pathogen in humans, causing chronic gastritis and playing a major role in the development of peptic ulcers and gastric cancer. The organism is highly adapted to the human stomach, largely due to its motility and ability to produce large amounts of urease. It binds specifically to the gastric mucosa via adhesion pedestals; colonization of the duodenum only occurs in the presence of gastric metaplasia. Infection with H. pylori leads to gastritis, but the majority of infected patients are asymptomatic, and it is thought that the ability of H. pylori to cause mo

Eradication of Helicobacter pylori.

It has become clear over the past 10 years that Helicobacter pylori is responsible for two of the most common gastrointestinal diseases worldwide, gastric and duodenal ulcer, and that it plays an important role in the development of the second most common lethal malignancy affecting humankind, gastric cancer. At present eradication therapy is considered to be the most appropriate way of treating gastric and duodenal ulcer associated with infection. Treatment, however, remains a difficult area. Although the organism is sensitive to a wide variety of antibiotics in vitro, monotherapy in vivo is

Helicobacter pylori and ulcerogenesis.

The dictum "no acid-no ulcer" had, in the past, summarized the thinking concerning the pathogenesis of peptic ulcer disease. It is now recognized that infection with Helicobacter pylori is the major causal factor leading to both duodenal and gastric ulceration. Infection is associated with many of the acid secretory abnormalities that have traditionally characterized peptic ulcer disease; indeed, acid secretory physiology returns to normal following bacterial eradication. Since not all individuals infected with H. pylori develop ulcers, host susceptibility, bacterial virulence, and/or specific

Helicobacter pylori and hormones.

Helicobacter pylori affects gastric acid secretion via several mechanisms. One of these is by changing gastric regulatory physiology. The infection elevates plasma gastrin levels and decreases gastric mucosal expression of the inhibitory peptide somatostatin. These changes may be due to products of H. pylori itself or inflammatory cytokines released in H. pylori infection: acid secretion is inhibited less by a low intra-gastric pH, infusions of cholecystokinin and gastric distention in infected persons. Eradication of H. pylori rapidly decreases basal acid secretion and gastrin-releasing, pept

Patients with peptic ulcer disease have elevated gastric acid secretion, hyperfunction of G cell, impaired bicarbonate secretion, increased levels of pepsinogen I and the presence of inflammatory mediators as well as Helicobacter pylori-induced gastroduodenitis and gastric metaplasia in the duodenum (in duodenal ulcer patients). Non-H. pylori-associated ulcers include those due to non-steroidal anti-inflammatory drugs, Zollinger-Ellison syndrome, and those related to other forms of gastritis.
