Spinal subdural hematoma (SDH) is rarely reported, and their simultaneous occurrence with intracranial SDH is even more rare. Intracranial subdural hematoma after spinal anesthesia is a rare and life-threatening complication of spinal anesthesia. The most common complication of spinal anesthesia is the postdural puncture headache. When severe and persistent headache after spinal anesthesia occur, differential diagnosis can be explored. In this report, we aimed to evaluate a patient with persistent headache following spinal anesthesia for cesarean section in a 31-year-old woman ,and emphasize a rare complication of spinal anesthesia which is subdural hematoma.

Subdural hematoma (SDH) is a rare, but life-threatening complication of spinal anesthesia. Subdural hematoma resulting from this procedure could present with vague symptoms such as chronic headache and could easily be missed. BACKGROUND: The spinal subdural hematoma (SSH) is an extremely rare entity which represents only 4.1% of all spinal hematomas. It needs accurate diagnosis and rapid intervention because of the major neurological risk induced by spinal compression. Several etiologies have been reported: anticoagulant treatments, haematological disorders, arterio-venous malformation, repeated attempts at lumbar punctures and tumors. A 63-year-old woman developed acute back pain and sciatica after playing hula-hoop. Urinary incontinence was also noted. Magnetic resonance imaging revealed acute thoracolumbar spinal subdural hematoma. After emergency surgical decompression and evacuation of the spinal subdural hematoma, the patient had good recovery without any postoperative neurologic deficit. In this article, we describe and review the clinical presentations, characteristic findings from imaging studies, and treatment of spinal subdural hematoma.

BACKGROUND: Spinal subdural hematoma (SSDH) is a rare but potentially serious condition characterized by a compressive accumulation of blood within the thecal sac. It may result from trauma, surgery, or underlying vascular abnormalities, often associated with anticoagulant use. In this case, SSDH was diagnosed post-implantation of a spinal cord stimulation (SCS). Current estimates suggest that a neuraxial (including epidural) hematoma following SCS occurs in about 0.32% of cases, with no reported sub/intradural pathology from uncomplicated SCS procedures.
