Extramedullary hematopoiesis: A case report.

Extramedullary hematopoiesis occurs primarily in patients with hematologic disorders and is a physiological response to chronic anemia. Its pathophysiology remains unknown. It commonly affects the liver, spleen, and lymph nodes. Vertebral involvement is the least affected site. Clinically, it is asymptomatic unless complications arise, such as spinal cord compression. MRI remains the key diagnostic tool. We herein report the case of a 47-year-old female patient with a history of severe anemia since the age of 16 and pancytopenia, admitted to the emergency department for sudden-onset febrile ja…

Extramedullary Hematopoiesis in Gastric Mucosa.

In this paper, pathohistological images of extramedullary hematopoiesis (EMH) in stomach mucosa in a 68-year-old female patient with treated osteomyelofibrosis are presented. The digestive system is a potential but uncommon site of EMH, with the gastric mucosa being particularly rare. According to the available literature, only 12 cases have been described.

Clonal Hematopoiesis and Premalignant Diseases.

Clonal hematopoiesis (CH) arises when mutations in the hematopoietic system confer a fitness advantage to specific clones, thereby favoring their disproportionate growth. The presence of CH increases with age and environmental exposures such as cytotoxic chemotherapy or radiotherapy. The most frequent mutations occur in epigenetic regulators, such as DNMT3A, TET2, and ASXL1, leading to dysregulation of tumor suppressor function, pathogen response, and inflammation. These dysregulated processes elevate risk of overall mortality, cardiovascular disease, and eventual hematologic malignancy (HM). …

Clonal hematopoiesis: Pre-cancer PLUS.

Clonal hematopoiesis is a common, age-related process in which a somatically mutated hematopoietic precursor gives rise to a genetically distinct subpopulation in the blood. This phenomenon has been observed in populations across the globe and, while virtually non-existent in children is estimated to affect >10% of the 70-and-older age group. The mutations are thought to occur in stem cells, which makes them pre-cancerous, and precursors to cancer stem cells. Many of the genes most commonly mutated in clonal hematopoiesis are also recurrently mutated in leukemia, genes such as DNMT3A, TET2, AS…

[Renal extramedullary hematopoiesis. Case report].

UNLABELLED: Extramedullary hematopoiesis (EMH) is defined as the formation and development of erythrocytic, granulocytic and megakaryocytic cell lines outside the bone marrow; it is a rare occurrence, which is limited to case reports. OBJECTIVE: To describe a case of renal EMH secondary to chronic hypoxia. CASE REPORT: The patient is a 16 year old teenager who developed chronic hypoxia, central cyanosis, clubbing and lives more than 2,500 m above sea level. Chronic hypoxia is probably secondary to pulmonary hypertension, neither detected nor previously treated. The Consultation was due to a …

Hypercholesterolemia links hematopoiesis with atherosclerosis.

Atherosclerosis is characterized by the progressive accumulation of lipids and leukocytes in the arterial wall. Leukocytes such as macrophages accumulate oxidized lipoproteins in the growing atheromata and give rise to foam cells, which can then contribute to the necrotic core of lesions. Lipids and leukocytes also interact in other important ways. In experimental models, systemic hypercholesterolemia is associated with severe neutrophilia and monocytosis. Recent evidence indicates that cholesterol-sensing pathways control the proliferation of hematopoietic stem-cell progenitors. Here we revie

Extramedullary hematopoiesis in leiomyoma uteri.

Extramedullary hematopoiesis (EMH) that often occurs as a compensatory reaction to an underlying hematologic abnormality is a non-neoplastic proliferation of hematopoietic tissue outside the bone marrow and peripheral blood. Rarely, EMH may be seen in hematologically normal individuals. EMH is most commonly (95%) seen in reticuloendothelial organs such as the spleen, liver, and lymph nodes but has rarely been reported in other locations. EMH is extremely rare in the uterus. In this case report, we present EMH in leiomyoma uteri in patients without any underlying hematologic abnormalities. Very…

Impact of Exercise on Clonal Hematopoiesis.

BACKGROUND: Clonal hematopoiesis (CH) is more prevalent in cancer patients and is associated with excess all-cause mortality (ACM) risk compared to patients without CH. Interventions to mitigate risk are not available. OBJECTIVES: The objective of the study was to investigate whether exercise reduces CH prevalence and excess ACM risk of CH in cancer patients. METHODS: We integrated genomic sequencing data with detailed exercise and clinical annotation from 2 retrospective studies: 3,539 patients with solid tumors from the MSK-IMPACT (Memorial Sloan Kettering-Integrated Mutation Profiling of …

MLLT10 in benign and malignant hematopoiesis.

Non-random chromosomal translocations involving the putative transcription factor Mixed Lineage Leukemia Translocated to 10 (MLLT10, also known as AF10) are commonly observed in both acute myeloid and lymphoid leukemias and are indicative of a poor prognosis. Despite the well-described actions of oncogenic MLLT10 fusion proteins, the role of wild-type MLLT10 in hematopoiesis is not well characterized. The protein structure and several interacting partners have been described and provide indications as to the potential functions of MLLT10. This review examines these aspects of MLLT10, contextua…

Myc roles in hematopoiesis and leukemia.

Hematopoiesis is a process capable of generating millions of cells every second, as distributed in many cell types. The process is regulated by a number of transcription factors that regulate the differentiation along the distinct lineages and dictate the genetic program that defines each mature phenotype. Myc was first discovered as the oncogene of avian leukemogenic retroviruses; it was later found translocated in human lymphoma. From then on, evidence accumulated showing that c-Myc is one of the transcription factors playing a major role in hematopoiesis. The study of genetically modified m…

Mitochondria in hematopoiesis and hematological diseases.

Mitochondria are involved in hematopoietic cell homeostasis through multiple ways such as oxidative phosphorylation, various metabolic processes and the release of cytochrome c in the cytosol to trigger caspase activation and cell death. In erythroid cells, the mitochondrial steps in heme synthesis, iron (Fe) metabolism and Fe-sulfur (Fe-S) cluster biogenesis are of particular importance. Mutations in the specific delta-aminolevulinic acid synthase (ALAS) 2 isoform that catalyses the first and rate-limiting step in heme synthesis pathway in the mitochondrial matrix, lead to ineffective erythro

The role of MLL in hematopoiesis and leukemia.

The MLL gene, also called HRX or ALL-1, was originally identified as a recurrent chromosomal translocation in particular subtypes of acute lymphocytic leukemia (ALL) and acute myelogenous leukemia (AML). Reciprocal rearrangements of the MLL gene are most common in infant ALL and secondary AML. Because of the unique association with infant leukemia and the intriguingly immature and mixed lineage phenotype of leukemic cells, the authors speculate that the wild-type MLL gene plays an important role early in the development of the hematopoietic system. This article reviews recent progress in under

Extramedullary hematopoiesis in a pyogenic granuloma.

Extramedullary hematopoiesis is rare outside the setting of significant primary hematologic disease. We describe this phenomenon in an exuberant pyogenic granuloma in an otherwise healthy man. We postulate that this vascular lesion provided a suitable milieu for homing and proliferation of stem cells.

Homeobox genes in hematopoiesis and leukemogenesis.

Homeobox genes have a strikingly conserved 61-amino acid sequence, encoding DNA-binding domain. Some homeobox genes, located in four clusters are designated HOX A through D, while others are known as divergent homeobox genes. Many researchers have demonstrated that hematopoietic cells express many homeobox genes. Induced over- and under-expression of these genes has been used to demonstrate their affect on some aspects of hematopoiesis and leukemogenesis. Recently, knock-out mice of homeobox genes by targeted disruption have also been used to examine their hematological effect. There are examp…
