99mTc-HSA millimicrospheres were used to detect a site of extra medullary hematopoiesis in the chest of a patient with severe osteopetrosis, anemia, thrombocytopenia and splenomegaly causing renal complications. These problems led to the consideration of splenectomy which necessitated a search for other extra medullary hematopoietic sites.

Extramedullary haematopoiesis is a compensatory response of bone marrow to abnormal haematopoietic tissue formation: the commonest etiology is thalassemia (generally intermedia). The thorax is a rare site and the posterior mediastinum is usually involved. CT scan completes standard chest radiology and allows fine needle aspiration to provide adequate cytologic material for the diagnosis of extramedullary haematopoiesis, especially when an intercurrent pathology is present and could involve paravertebral areas. Thoracotomy may be necessary in rare cases of complications.

Hematopoiesis is a process by which all blood cells are formed. The mechanisms controlling it have been studied for decades. Extramedullary hematopoiesis (EMH) associated with myelofibrosis uncommonly occurs within the thorax. We describe the first reported case of acute and rapidly fatal respiratory failure due to pulmonary interstitial EMH associated with myelofibrosis. Interstitial EMH should be considered in the differential diagnosis of patients with interstitial pulmonary infiltrates and respiratory failure accompanying a disease process known to predispose to the development of EMH.

A 54-year-old man was diagnosed with hereditary spherocytosis. An X-ray of the chest disclosed bilateral round masses in the upper and lower posterior mediastinum. CT showed homogeneous round masses of soft tissue density in the posterior mediastinum. A needle biopsy of the mass showed hyperplastic erythroid bone marrow. Fatty metamorphosis of the masses was observed in CT 2 years after splenectomy but showed no change in size.

The radiographic manifestations of thoracic extramedullary hematopoiesis are unilateral or bilateral, smooth, sharply delineated, often lobulated paraspinal masses without erosion of the vertebral bodies or ribs, sometimes associated with subpleural, paracostal masses. Extramedullary hematopoiesis of the bronchus is rare. The case of a 72-year-old man in whom the right lower lobe bronchus was obstructed by extramedullary hematopoiesis is presented. Ten months after the initial presentation, Philadelphia chromosome-negative chronic myelogenous leukemia was diagnosed. Such findings have not been reported previously. The various anatomic locations of extramedullary hematopoiesis are reviewed, with an emphasis on intrathoracic and pulmonary presentations. The clinical and pathologic features and the differential diagnosis in the present case are discussed.

BACKGROUND: Extramedullary hematopoiesis (EMH) in the middle ear is exceedingly rare, with fewer than five cases reported. The authors report the first Thai case of middle ear EMH
CASE REPORT: A 32-year-old Thai thalassemic man presented with complaint of right-sided hearing loss from a middle ear mass. The CT/MRI was done and the diagnosis of EMH was confirmed by a pathological examination after a surgical removal.
CONCLUSION: A differential diagnosis of EMH should also be done in thalassemic patients presented with a middle ear mass.

Renal extramedullary hematopoiesis is rarely recognized in the antemortem setting. We identified 14 patients with renal extramedullary hematopoiesis on antemortem specimens from 1994 to 2015. The mean age was 68 years (range 47-87 years); males predominated (M:F=9:5). All presented with renal insufficiency, including five (36%) with acute kidney injury. Allergic inflammation is associated with increased generation and trafficking of inflammatory cells, especially eosinophils, to sites of inflammation. The effect of acute versus chronic airway allergen challenge on hematopoietic activity in the bone marrow (BM) and lungs was investigated using murine models of allergic airway inflammation. Acute allergen challenge induced proliferation of BM cells and significantly increased generation of eosinophil, but not multipotent, granulocyte-macrophage (GM), or B-lymphocyte progenitor cells. However, no hematopoietic activity was observed in the lungs.

Intrathoracic extramedullary hematopoiesis is a rare tumor occurring predominantly in the posterior mediastinum, most commonly in patients with congenital hemolytic anemias. The first reported case of this entity's occurring in the anterior mediastinum is described in a 68-year-old man with an incompletely characterized anemia. Definitive diagnosis was established only after median sternotomy and removal of the tumor.
