Anatomic hemispherectomy for intractable epilepsy in a patient with unilateral schizencephaly.

Schizencephaly is a rare congenital cortical brain malformation defined by unilateral or bilateral clefts of the cerebral hemispheres. These malformations are often associated with medically intractable epilepsy. Surgical solutions include lesionectomy, lobectomy, or hemispherectomy. The authors describe the case of an anatomic hemispherectomy for medically intractable epilepsy in an 8-year-old boy with a large schizencephalic cleft. Seven years prior to his epilepsy surgery, the patient underwent placement of a ventriculoperitoneal shunt for communicating hydrocephalus that resulted in severe

Hemispherectomy for intractable epilepsy in adults: the first reported series.

Hemispherectomy for intractable unihemispheric epilepsy (IUE) has long been established in pediatric patients. This study reports the first series examining hemispherectomy exclusively in adult patients (>18 years old). Nine adults with IUE underwent hemispherectomy at the University of Minnesota. All patients had unilateral hemiplegia and visual field loss. Seven patients (77.8%) were Engel class I/II at last follow-up. Five (83.3%) of the six patients with >30 years of follow-up were seizure free. No surgery-related mortality, hydrocephalus, or superficial cerebral hemosiderosis occurred. He

[Modified hemispherectomy for intractable epilepsy in patients with infantile hemiplegia].

OBJECTIVE: To explore the effectiveness of modified hemispherectomy for intractable epilepsy in patients with infantile hemiplegia. METHODS: Eighteen cases of patients were treated with modified hemispherectomy and the effectiveness was studied and followed up. RESULTS: The seizures in all 18 cases of patients were controlled effectively and stopped completely in 16 cases of them, without nervous disfunction worsened. The patients' cerebral peduncles on healthy side were much thicker than those on sick side (t = 58.32, P < 0.001) and healthy peoples' (t = 14.63, P < 0.001) and the patients'

Intensive mobility training postcerebral hemispherectomy: early surgery shows best functional improvements.

BACKGROUND: Limited research exists on rehabilitative techniques focused on reducing disabilities after cerebral hemispherectomy despite persistent hemiparesis. OBJECTIVES: The efficacy of Intensive Mobility Training (IMT) for improving gait, balance and mobility was evaluated in patients after cerebral hemispherectomy and compared with clinical variables for signs of developmental neuroplasticity. METHODS: Participants (N.=19; 13.8±5.7 years) postcerebral hemispherectomy received IMT, three hours/day for 10 days. Outcomes measures were assessed pre- and post-intervention using the GAITRite

Seizure outcome after functional hemispherectomy for malformations of cortical development.

MRI features were correlated with postsurgical seizure outcome in patients with hemispheric malformations of cortical development (MCD). After functional hemispherectomy, 5 of 6 patients (83%) with hemimegalencephaly had persistent, although markedly improved, seizures; 5 of 6 patients (83%) with relative preservation of part of one lobe or atrophy were seizure free. Hemimegalencephaly and other types of hemispheric MCD appear to differ in prognosis for freedom from seizures after functional hemispherectomy.

Delayed complications thirty-six years after hemispherectomy: a case report.

PURPOSE: To describe a late complication of hemispherectomy in a patient in whom symptoms of hydrocephalus developed 36 years after her left-sided hemispherectomy, the longest delay on record. METHODS: Hemispherectomy has been successfully used in the treatment of intractable epilepsy associated with infantile-type hemiplegia for a half century. Of the patients, however, up to 33% have late increased cerebrospinal fluid pressure complications attributed to superficial cerebral hemosiderosis. Through a retrospective case analysis, we describe such complications in a 52-year-old woman with cogn

Phonological dyslexia and phonological dysgraphia following left and right hemispherectomy.

Four adults who had hemispherectomies because of severe epilepsy following infantile of childhood damage to one hemisphere of the brain, are assessed on their reading and spelling abilities in an attempt to see if the two hemispheres are equipotential for these abilities in infancy. The psycholinguistic assessments of language processing in aphasia (PALPA) are used, and the results are interpreted from the viewpoint of hypotheses of "normal" right and left hemisphere reading abilities. Overall, the results suggest that the two hemispheres are equipotential at infancy for developing the skills

Residual sensorimotor functions in a patient after right-sided hemispherectomy.

Sensorimotor functions were examined in a patient with left-sided infantile hemiparesis who underwent hemispherectomy (HS) on the right side at age 18 for intractable epilepsy. Pathological examination of the removed hemisphere showed a porencephalic cyst of the temporal lobe and of the frontoparietal operculum. On examination, the patient had hemianopia to the left and sensorimotor deficits only of the distal limbs contralateral to the HS. She walked with a barely perceptible limp. Axial and proximal movements were quasi normal, so that the patient could fully elevate both arms, flex and exte

Transarterial embolization versus hemispherectomy in infants with hemimegalencephaly and drug-resistant epilepsy.

BACKGROUND: Hemimegalencephaly is a severe congenital brain malformation characterized by early-onset, medically refractory seizures. The standard treatment, surgical hemipherectomy (SH), is constrained by age-related limitations. To address this gap, we developed a staged transarterial embolization (TAE) approach as a minimally invasive alternative for infants under 10-12 weeks. The comparative efficacy of TAE versus SH has not been systematically evaluated. METHODS: This single-center observational comparative effectiveness study evaluated seizure burden, neurodevelopmental outcomes, and pe

Craniosynostosis following hemispherectomy in a 2.5-month-old boy with intractable epilepsy.

The authors report on the case of a 6-week-old boy who presented with infantile spasms. At 2.5 months of age, the patient underwent a right hemispherectomy. Approximately 3 months postoperatively, the patient presented with left coronal craniosynostosis. Subsequent cranial vault remodeling resulted in satisfactory cosmesis. Four years after surgery, the patient remains seizure free without the need for anticonvulsant medications. The authors believe this to be the first reported case of iatrogenic craniosynostosis due to hemispherectomy, and they describe 2 potential mechanisms for its develop

Onset of speech after left hemispherectomy in a nine-year-old boy.

Case Alex, with Sturge-Weber Syndrome affecting the left hemisphere, failed to develop speech throughout early boyhood, and his comprehension of single words and simple commands remained stagnant at an age equivalent of 3-4 years. But then, following left hemidecortication at age 8.5 years and withdrawal of anticonvulsants when he was more than 9 years old, Alex suddenly began to acquire speech and language. He also showed an unusual degree of residual motor capacity on his right side. Alex's remarkable progress in learning speech and language, and the development of his other cognitive abilit

Residual vision with awareness in the field contralateral to a partial or complete functional hemispherectomy.

Two patients with unilateral disconnection or removal of the entire occipital lobe were tested for residual vision in their blind field. Using image stabilization to eliminate eye motion artifacts, the central portion of each subject's visual field was tested, beginning 1 degree from fixation and extending outward to 13.5 degrees. A narrow zone of residual vision was identified along the retinal vertical meridian of each patient. The lateral edge of this zone was generally within 3.5 degrees of the vertical meridian, though extended farther outward (but not beyond 6 degrees) at one field locat

Hemispherectomy in the newborn hamster: effect on cell death in the extraocular motor nuclei.

In order to test the effect of removing the corticonuclear pathway on the development of the extraocular motor nuclei, a hemispherectomy was performed in newborn hamsters. After a survival period of 20 days, the number of neurons in the extraocular motor nuclei was compared to that of intact animals. The number of neurons in the stated nuclei showed a significant reduction in the experimental animals in comparison to controls. The decrease in the number of neurons affected all types of cells. Both sides of the brain showed the reduction in the number of neurons, although more intensely in the

Aphasia after hemispherectomy in an adult with early onset epilepsy and hemiplegia.

A 55 year old left handed man with left hemisphere subcortical encephalomalacia, seizures, language impairment, and right hemiparesis from a motor vehicle accident at age five was evaluated for epilepsy surgery. The patient continued to speak and followed commands during a left intracarotid amobarbital test (IAT). Left functional hemispherectomy resulted in expressive aphasia. Based on postoperative outcome, language was bilateral. The injury after primary development of language function, the predominantly subcortical lesion, and the late timing of surgical intervention well past development

Linguistic and non-linguistic abilities in a patient with early left hemispherectomy.

A patient who underwent early removal of the left hemisphere because of Sturge-Weber syndrome was submitted to detailed linguistic and visuospatial batteries. The performances were compared to performances of subjects matched for age, education and IQ, but without focal cerebral lesions. Language was mildly impaired but to the same extent as in IQ controls. On the contrary, visuospatial abilities were clearly worse than in IQ controls, and the most preserved visuospatial abilities seemed to be the less sophisticated ones. Non literal comprehension of language, a function generally attributed t
