ESICM guidelines on circulatory shock and hemodynamic monitoring 2025.

OBJECTIVE: These European Society of Intensive Care Medicine (ESICM) guidelines provide recommendations for the diagnosis of shock and hemodynamic monitoring for adult critically ill patients. METHODS: An international panel of experts formulated PICO-formatted questions, and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach was applied to assess evidence and formulate recommendations. In the absence of strong evidence, panelists issued ungraded good practice statements (UGPS). RESULTS: Panelists issued 50 statements. Among others, skin perfusion should

Persistent left superior vena cava complicating hemodynamic monitoring catheterization.

Two cases of flow-directed, balloon-tipped catheterizations of the right heart via a persistent left superior vena cava are presented. In both cases, the monitoring catheters assumed similar, atypical configurations on plain chest roentgenograms. Recognition of the characteristic configuration of the course of transvenous catheters through the persistent left superior vena cava allows for plain film diagnosis of this anomaly.

Non-Invasive Hemodynamic Monitoring in Transcatheter Aortic Valve Implantation.

Background/Objectives: Aortic valve stenosis (AS) is a prevalent cardiovascular condition among elderly patients frequently treated with Transcatheter Aortic Valve Implantation (TAVI). Traditional hemodynamic monitoring during TAVI relies on invasive methods. The ClearSight® Finger Cuff system offers a non-invasive alternative for continuous hemodynamic monitoring. To compare the reliability and feasibility of non-invasive hemodynamic monitoring with traditional invasive hemodynamic monitoring during TAVI procedures. Methods: In this prospective observational study, patients undergoing electiv

Cerebral hemodynamic monitoring combined with infusion test in hydrocephalus.

INTRODUCTION: Disturbance in cerebrospinal fluid (CSF) circulation may overlap with abnormality of cerebral blood flow (CBF) in hydrocephalus. Transcranial Doppler (TCD) ultrasonography is a non-invasive technique able to assess CBF velocity (CBFv) dynamics in response to a controlled rise in ICP during CSF infusion tests. RESEARCH QUESTION: Which TCD-derived cerebral hemodynamic parameters change during controlled rise of ICP, and in which direction? MATERIAL AND METHODS: Infusion tests combined with TCD monitoring and non-invasive monitoring of arterial blood pressure (ABP) were conducted

Less or more hemodynamic monitoring in critically ill patients.

PURPOSE OF REVIEW: Hemodynamic investigations are required in patients with shock to identify the type of shock, to select the most appropriate treatments and to assess the patient's response to the selected therapy. We discuss how to select the most appropriate hemodynamic monitoring techniques in patients with shock as well as the future of hemodynamic monitoring. RECENT FINDINGS: Over the last decades, the hemodynamic monitoring techniques have evolved from intermittent toward continuous and real-time measurements and from invasive toward less-invasive approaches. In patients with shock, c

Transesophageal echocardiographic hemodynamic monitoring during preoperative acute normovolemic hemodilution.

BACKGROUND: Preoperative acute normovolemic hemodilution may compromise oxygen transport. The aims of our study were to describe the hemodynamic effects of normovolemic hemodilution and to determine its effect on systolic and diastolic cardiac function by multiplane transesophageal echocardiography. METHODS: In eight anesthetized patients (aged 13-51 yr) without heart disease, hemoglobin was reduced in steps from 123 +/- 8 (mean +/- SD) to 98 +/- 3 and to 79 +/- 5 g/l. Hemodynamic measurements (intravascular pressures, thermodilution cardiac output, and echocardiographic recordings) were obta

Peripheral postcapillary venous pressure--a new hemodynamic monitoring parameter.

Peripheral postcapillary venous pressure (PCVP) appears to be an indicator of peripheral perfusion (cutaneous blood flow) in the experimental animal. Eighteen male mongrel dogs under balanced anesthesia were subjected to either slow, continuous hemorrhage 0.65 ml/kg/min) or plasma expansion with dextran 40 (0.80 ml/kg/min) over a period of 1 hour. Peripheral PCVP, cental venous pressure arterial pressure, cardiac output (CO), mixed venous oxygen saturation, pulmonary arterial pressure, and pulmonary wedge pressure were monitored on a continuous basis. The only parameter that consistently gave

Monitoring of cerebral hemodynamics with jugular bulb catheters.

Jugular venous oxygen saturation (SjvO2) monitoring is useful for detecting episodes of cerebral hypoxia/ischemia in patients with head injury, patients undergoing neurosurgical procedures, and patients undergoing cardiopulmonary bypass. The use of SjvO2 monitoring can direct the treatment of ischemic episodes and identify the optimal level of cerebral perfusion pressure and PCO2 for the individual patient.

Three decades of hemodynamic monitoring (1995-2025): from invasive pressure based hemodynamic profiling to functional physiology.

PURPOSE OF REVIEW: Hemodynamic monitoring has undergone a profound transformation over the last 30 years. The field has transitioned from the "standard of care" invasive pulmonary artery catheterization (PAC) toward minimally invasive and noninvasive technologies. This evolution is characterized by a shift in clinical philosophy. RECENT FINDINGS: A very strong shift away from the measurement of static pressures and flow defining specific hemodynamic profiles to the assessment of functional physiology and fluid responsiveness characterize the main changes in hemodynamic monitoring over this ti

Aligning cardiac monitoring with American Heart Association Guidelines: Impact on utilization, hemodynamic monitoring, and outcomes.

BACKGROUND: Overuse of continuous cardiac monitoring can lead to poor patient experience, increased costs, and decreased efficiency. Because significant variation in continuous cardiac monitoring ordering exists, implementation strategies that promote care in alignment with practice standards and an examination of use cases that fall outside of standards are needed. The purpose of this study, therefore, was to evaluate if implementation of American Heart Association (AHA) practice standards on continuous cardiac monitoring could reduce utilization without jeopardizing patient safety. METHODS:

Accuracy assessment of noninvasive cardiac output monitoring in the hemodynamic monitoring in critically ill patients.

BACKGROUND: The consistency of cardiac output (CO) measured by noninvasive cardiac output monitoring (NICOM), pulse index continuous cardiac output (PiCCO), and ultrasound in the hemodynamic monitoring of critically ill patients was studied. Using the NICOM built-in passive leg raising (PLR) test, stroke volume index variation (∆SVI) was calculated and was used to predict volume responsiveness in patients with circulatory shock (excluding cardiogenic shock). METHODS: Critically ill patients requiring hemodynamic monitoring were admitted during the study period. The CO of each included patient

Real Time Hemodynamic Monitoring During M-TEER Using Electrical Cardiometry.

BACKGROUND: Mitral regurgitation is a common valvular dysfunction causing patient morbidity and mortality. Mitral transcatheter edge-to-edge repair (M-TEER) allows grasping of valvular leaflets and approximation via a small implant, thus reducing mitral regurgitation (MR). The implant allows staged leaflet capture and leaflet optimization before it is finally released. Real time hemodynamic monitoring could facilitate procedural success and improve patient outcomes. METHODS: Fourteen patients scheduled for elective M-TEER were included in this study. Right heart catheterization (RHC) and dete

The Intensivist's Perspective of Shock, Volume Management, and Hemodynamic Monitoring.

One of the primary reasons for intensive care admission is shock. Identifying the underlying cause of shock (hypovolemic, distributive, cardiogenic, and obstructive) may lead to entirely different clinical pathways for management. Among patients with hypovolemic and distributive shock, fluid therapy is one of the leading management strategies. Although an appropriate amount of fluid administration might save a patient's life, inadequate (or excessive) fluid use could lead to more complications, including organ failure and mortality due to either hypovolemia or volume overload. Currently, inten

Contemporary Review of Hemodynamic Monitoring in the Critical Care Setting.

Hemodynamic assessment remains the most valuable adjunct to physical examination and laboratory assessment in the diagnosis and management of shock. Through the years, multiple modalities to measure and trend hemodynamic indices have evolved with varying degrees of invasiveness. Pulmonary artery catheter (PAC) has long been considered the gold standard of hemodynamic assessment in critically ill patients and in recent years has been shown to improve clinical outcomes among patients in cardiogenic shock. The invasive nature of PAC is often cited as its major limitation and has encouraged develo

Hemodynamic Consideration in Intraoperative Neurophysiological Monitoring in Neuromuscular Scoliosis Surgery.

OBJECTIVE: To prove the hypothesis that the parameters of intraoperative neurophysiological monitoring (IONM) during will be more deteriorated in neuromuscular scoliosis (NMS) than in adolescent idiopathic scoliosis (AIS). METHODS: This retrospective study reviewed the data of 69 patients (NMS=32, AIS=37) who underwent scoliosis surgery under IONM. The amplitude of motor evoked potentials (MEPs), and the amplitude and the latency of somatosensory evoked potentials (SEPs) were examined. Demographic, preoperative, perioperative and postoperative data were analyzed to determine whether they affe

Variation in perioperative cerebral and hemodynamic monitoring during carotid endarterectomy.

BACKGROUND: Hemodynamic disturbances cause half of the perioperative strokes following carotid endarterectomy (CEA). Guidelines strongly recommend strict pre- and postoperative blood pressure (BP) monitoring in CEA patients, but do not provide firm practical recommendations. Although in the Netherlands 50 centres perform CEA, no national protocol on perioperative hemodynamic, and cerebral monitoring exists. To assess current monitoring policies of all Dutch CEA-centres, a national survey was conducted. METHODS: Between May and July 2017 all 50 Dutch CEA-centres were invited to complete a 42-q

Continuous Noninvasive Hemodynamic Monitoring in an Infant With Tetra-Amelia.

Tetra-amelia syndrome is a congenital disorder associated with near or complete absence of all 4 limbs. Noninvasive hemodynamic monitoring may be difficult or impossible in such patients. We describe the use of a finger cuff blood pressure system for continuous noninvasive blood pressure monitoring in an infant with near-complete tetra-amelia undergoing laparoscopic gastrostomy tube placement. This case suggests the potential use of such a blood pressure monitoring system for other patients with comparable deformities.

Intraoperative esophageal Doppler hemodynamic monitoring in free perforator flap surgery.

Goal-directed fluid therapy optimizes cardiac output and flap perfusion during anesthesia. Intraoperative esophageal Doppler (ED) monitoring has been reported as more accurate and reliable, demonstrating improved surgical outcomes compared with central venous pressure and arterial catheter monitoring. A prospective study of patients undergoing free perforator (deep inferior epigastric artery perforator/anterolateral thigh) flap surgery with intraoperative ED monitoring (51 patients) or central venous pressure monitoring (53 patients) was undertaken. Fluid input included crystalloids, colloids,
