Hepatitis B Vaccine: Four Decades on.

Hepatitis B virus is a substantial contributor to cirrhosis and hepatocellular carcinoma (HCC) globally. Vaccination is the most effective method for prevention of hepatitis B and its associated morbidity and mortality, and the only method to prevent infection with hepatitis D virus. The hepatitis B vaccine has been used worldwide for more than four decades; it is available in a single- or triple-antigen form and in combination with vaccines against other infections. Introduction of the vaccine and administration at birth led to sustained decline in mother-to-child transmission, chronic hepati

[Hepatitis B vaccine and liver cancer].

Hepatitis B Virus (HBV) chronic infection contributes to a high risk of hepatocellular cancer (HCC) development. HBV is a strong cancer inducer, due to natural history of infection, virological characteristics and viral DNA integrations events in host genome. Prolonged infection and high viral loads, particularly frequent in patients infected in childhood, are risk factors of HCC development for patients with HBV chronic infection. A HBV vaccine, based on immunization against the surface protein HBs, showed a strong efficacy to prevent chronic HBV infection. The development of universal neonat

Optimizing hepatitis B vaccination in prison.

OBJECTIVE: We aimed to establish the current status of hepatitis B virus (HBV) vaccination in prison. METHODS: We carried out two evaluations within a 1-year interval with inmates incarcerated for 6 to 12 months. A monitoring process was introduced in-between the two evaluations. RESULTS: We included 231 inmates. Overall, 42.9% were immunized because of a previous vaccination and 14.3% because of a previous exposure. Inmates born in an area of medium or high endemicity for HBV were significantly more exposed to HBV. The proportion of non-immunized inmates was 42.8% at the time of incarcerati

Hepatitis B vaccination: myths and controversies.

Hepatitis B has attracted a lot of attention in recent times. It has been suggested that it is a widely prevalent disease and an important cause of morbidity and mortality in our country. A dispassionate analysis of available data suggests that the prevalence of disease may be much less than believed hitherto, and the morbidity and mortality due to the disease may have been overestimated. Vertical transmission from mother to child at the time of delivery may be an important contributor to overall carrier rates in the community which warrants that the first dose of vaccine be administered withi

Hepatitis B vaccination in preterm infants.

UNLABELLED: Preterm infants, especially those with very low birth weight, are at risk of hepatitis B virus infection. They often require invasive diagnostic methods in their first weeks of life, intensive treatment and long-term hospitalisation. Therefore, hepatitis B vaccination is particularly justified in these patients. Our aim was to determine the reaction of preterm children to hepatitis B vaccination. The study comprised 64 preterm children whose birth weight ranged from 700 g to 2460 g (mean 1776.6 g +/- 480.4 g) and whose gestational age was between 25 and 36 weeks. A 10 microg dose o

Lichenoid reaction to hepatitis B vaccination.

Liver disease is now a well recognized association with lichen planus. Recent reports suggest that the hepatitis viruses may play a central role in this association. The present report describes a case of a lichenoid reaction to hepatitis B vaccination, which has only been rarely documented previously. A 16-year-old male developed a widespread pruritic erythematous eruption 5 weeks after receiving his second dose of hepatitis B vaccine. The eruption evolved to produce strikingly blue macules before spontaneously disappearing. A skin biopsy showed an intense lichenoid reaction. The hepatitis B

[Vaccination against hepatitis B virus].

More than 200 million people infected with hepatitis B virus worldwide, with high risk of developing hepatic disease ranging from chronic hepatitis to hepatocellular carcinoma, underline an imperative need for an efficacious vaccine. Many studies have provided repeated assurance on the safety and efficacy of hepatitis B vaccines derived from plasma pooled from carriers. However, alternative methods for vaccine production are necessary and DNA recombinant vaccines are currently available. Recombinant hepatitis B vaccines are safe, antigenic, and free from side effects. The immunogenicity of yea

[Hepatitis B vaccination - before a breakthrough?].

Within the next few months hepatitis B vaccine will be introduced in Switzerland, a country where hepatitis B is one of the most frequent infectious disorders. The vaccines so far developed are produced from blood of healthy HBsAg carriers. HBsAg is then purified and possible infectious material is inactivated by formalin or heat. Two vaccines will soon be available, one produced by Merck Sharp & Dohme, which is given at day 0 and after 1 and 6 months in doses of 20 microgram or 40 microgram, and the other from the Pasteur Institute, which is given at day 0 and after 1 and 2 months in doses of

Hepatitis B vaccine administered to chronic carriers of hepatitis b surface antigen.

We administered up to 6 monthly doses of hepatitis B vaccine to 16 chronic carriers of hepatitis B surface antigen (HBsAg) in an attempt to eliminate the antigen. The HBsAg in this vaccine differs from native antigen. No patients had elimination of HBsAg, but one of 10 no longer carried hepatitis B e antigen (HBeAg), Of 13 patients without preexisting antibody to HBsAg (anti-HBs), none acquired the antibody; two of three patients with preexisting heterotypic anti-HBs had transient, low-level increases in anti-HBs titers. Serum alanine aminotransaminase (ALT) levels fell in eight patients, rema

Simultaneous injection of plasma-derived or recombinant hepatitis B vaccines with yellow fever and killed polio vaccines.

Previous studies have shown an absence of interaction between hepatitis B (HB) vaccine and other vaccines used in EPI programmes except for an apparent decrease of yellow fever antibody levels when hepatitis B and yellow fever vaccines are given simultaneously. We have therefore reinvestigated the interaction of these two vaccines and assessed the absence of interaction between inactivated polio vaccine and recombinant or plasma-derived HB vaccine. The immune responses to polio vaccine injected simultaneously with plasma-derived or recombinant HB vaccine were observed to be equivalent and simi

Evaluation of the immunogenicity of hepatitis B vaccination in hematopoietic stem cell transplant recipients vaccinated with a hexavalent vaccine compared with patients vaccinated with a combined hepatitis A and B vaccine.

BACKGROUND: In 2020, the Catalan Department of Health recommended a pediatric hexavalent vaccine for adult hematopoietic stem cell transplant (HSCT) recipients. Previously, patients at our center received a combined hepatitis A + B vaccine with double the hepatitis B antigen content. The immunogenicity of the hexavalent formulation in this population remains unclear. OBJECTIVE: To compare immunogenicity of two hepatitis B vaccination strategies in adult HSCT recipients and identify predictors of response. METHODS: We conducted a retrospective cohort study including adult HSCT recipients vacc

Do the therapeutic vaccines hold hope for the treatment of hepatitis B?

BACKGROUND: Chronic hepatitis B (CHB) is a global disease that can induce severe conditions such as cirrhosis and liver cancer, posing a serious threat to human health. The low functional cure rate of existing antiviral treatment drugs is primarily due to the dysfunctional immune response in CHB patients and the difficulty in clearing intrahepatic cccDNA. METHODS: This article reviews the latest progress in the research on therapeutic vaccines for CHB, to analyze and discuss the potential impact of current achievements and to outline the prospects for future research, exploring therapeutic st

Advancing therapeutic vaccines for chronic hepatitis B: Integrating reverse vaccinology and immunoinformatics.

Current treatments for chronic hepatitis B (CHB) are lifelong, often accompanied by side effects and the risk of drug resistance, highlighting the urgent need for alternative therapies such as therapeutic vaccines. However, challenges such as selecting appropriate antigens and addressing multiple hepatitis B virus (HBV) genotypes hinder the development of these vaccines. One approach to overcoming these challenges is reverse vaccinology (RV) combined with immunoinformatics. RV uses computational methods to identify antigens from pathogen genetic information, including genomic and proteomic dat

Chronic hepatitis B: Immunological profile and current therapeutic vaccines in clinical trials.

More than 250million people worldwide are chronically infected with hepatitis B virus (CHB), and over half a million die each year due to CHB-associated liver complications such as cirrhosis and hepatocellular carcinoma. The translation of immunological knowledge about CHB into therapeutic strategies aiming to a sustainable hepatitis B virus (HBV) clearance has been challenging. In recent years, however, the understanding on the immune effectors required to overcome chronicity has notably increased thanks to preclinical and clinical research. Therapeutic vaccination may prove to be useful for

Is hemodialysis a reason for unresponsiveness to hepatitis B vaccine? Hepatitis B virus and dialysis therapy.

Impaired renal function is associated with a high risk of chronicity of hepatitis B virus (HBV) infection. Patients on hemodialysis (HD) or peritoneal dialysis are at an increased risk of viral transmission due to frequent necessity of blood product transfer as well as use of contaminated dialysate or dialysis materials. Additionally, health professionals may cause viral spread via contaminated hands and carelessness against hygiene rules. The frequency of chronic HBV infection may be as high as 80% in patients on renal replacement therapies. This is because HBV vaccination is essential to eli

An assessment of the effect of hepatitis B vaccine in decreasing the amount of hepatitis B disease in Italy.

BACKGROUND: Hepatitis B (HBV) infection is an important cause of morbidity and mortality and it is associated to a higher risk of chronic evolution in infected children. In Italy the anti-HBV vaccination was introduced in 1991 for newborn and twelve years old children. Our study aims to evaluate time trends of HBV incidence rates in order to provide an assessment of compulsory vaccination health impact. METHOD: Data concerning HBV incidence rates coming from Acute Viral Hepatitis Integrated Epidemiological System (SEIEVA) were collected from 1985 to 2006. SEIEVA is the Italian surveillance na

[Role of mutations on the "hepatitis B virus 'a' determinant hotpoint" to the efficacy of hepatitis B vaccine].

OBJECTIVE: To study how hepatitis B virus(HBV) 'a' determinant hotpoint mutations were influecing the hepatitis B vaccine efficacy. METHODS: Primers were designed in HBV conservative region, and the degenerate probes for detecting 16 'a' determinant hotpoint mutations were developed for gene chips. Sensitivity and specificity of the gene chips were evaluated by clone sequencing. Sera of 47 pairs of mothers and infants with immune failure and 323 mothers of children with immune protection of HB vaccine were detected by the gene chips. RESULTS: Result from clone sequencing demonstrated that th

Perspectives on hepatitis B infections and the efficacy of vaccination (hepatitis B and pneumococci) in dialysis patients.

Hepatitis B is a well known problem in dialysis units. We therefore examined the historical frequency of hepatitis B carriers in our unit, our vaccination program to hepatitis B virus (HBV), the response to hepatitis B vaccine, the IgG subclass response of anti-HBs and the response and IgG subclass response to pneumococcal vaccination (another vaccine) in dialysis patients. From 1970 and onwards 23 HBV carriers were found, but no new cases of hepatitis B occurred during the study period, i.e. from 1980 and onwards. Only one of the carriers was alive by the end of 2001. In four patients liver d
