Incarcerated obturator hernia: early diagnostic using helical computed tomography.

Obturator hernia is a rare event with poor clinical signs. Delayed diagnosis is a cause of increased mortality due to ruptured gangrenous bowel. We report a case of incarcerated obturator hernia which highlights the usefulness of computed tomography (CT) scanning in diagnosing this condition.

Incarcerated obturator hernia: case diagnosed at barium enema fluoroscopy.

Incarcerated obturator hernia is an uncommon entity that on occasion may produce specific radiographic findings and clinical signs (Howship-Romberg sign) that permit diagnosis prior to celiotomy. A case is reported in which the diagnosis was suggested when pain in the thigh was elicited during the course of a barium enema examination.

Iatrogenic Obturator Hernia with Ureter as Content: Case Report.

A male in his early 70s presented with complaints of bilateral flank pain for seven months, hematuria for 2 days along with the passage of clots associated with an increased frequency of urination and nocturia. The patient had a history of deep vein thrombosis and was on anticoagulation. On ultrasound, multiple calculi with posterior acoustic shadowing were seen in bilateral kidneys, corroborated by Computed Tomography (CT) urography. Laser lithotripsy with DJ stent placement was conducted. The follow-up CT urography showed an inferiorly displaced DJ stent within the right distal ureter and th

An extraordinary cause of intestinal obstruction, incarcerated obturator hernia.

AIM: Presenting an extraordinary cause of intestinal obtuction, leading to incarcerated obturator hernia. CASE REPORT: We present a rare case of obturator hernia in a 90-year-old female, who presented with a three-day history of inability to defecate and abdominal pain, distension, and vomiting on the day of presentation. Hours later, a computed tomography scan revealed a bowel obstruction secondary to a right-sided obturator hernia. She underwent an emergency exploratory laparotomy and the incarcerated bowel was reduced with a repairment of the hernial defect. The patient demonstrated an une

Obturator hernia (the little old lady's hernia) diagnosed via computed tomography: a case report.

UNLABELLED: Obturator hernia is an infrequent clinical entity of abdominal wall hernia, accounting for an incidence rate ranging from 0.073 to 2.2% of all hernias and being responsible for 0.2-1.6% of all cases of mechanical intestinal obstruction. The computed tomography (CT) scan, as an imaging modality, is critical in improving the diagnostic rate of obturator hernia. CASE PRESENTATION: The authors herein report an 87-year-old thin male patient with a known history of chronic obstructive pulmonary disease who presenting with complaints of abdominal pain for 3 days and constipation for 2 da

A Case of Laparoscopic Transabdominal Pre-peritoneal Hernia Repair Using a 3D Mesh Inversion Technique for Bilateral Obturator and Femoral Hernias After Incarcerated Obturator Hernia Reduction.

An obturator hernia (OH) is a rare type of hernia that accounts for a very small proportion of all hernias and cases of small bowel obstruction. This condition predominantly affects older, underweight individuals, with the vast majority of patients being women. Laparotomy with simple suture closure of the defect is commonly used as surgical treatment for OH. However, the closeness of the obturator nerve to the obturator defect causes difficulty in observing this nerve due to the deep operative field in laparotomy. Thus, transabdominal pre-peritoneal hernia repair (TAPP) has advantages over an

Unilateral to Contralateral: A Rare Case of Sequential Obturator Hernias.

Obturator hernias are the rarest of all abdominal wall hernias. Multiparous women with a low body mass index (BMI) are at increased risk, and therefore, these hernias are sometimes referred to as "little old lady hernias." They occur more frequently on the right side than on the left, due to the sigmoid colon protecting the left obturator canal. These hernias are repaired through surgical intervention to prevent complications. In this case report, a thin, elderly, multiparous female patient presented with signs and symptoms of bowel obstruction. Imaging revealed a left-sided obturator hernia,

Two different surgical approaches for strangulated obturator hernias.

Obturator hernia is a rare condition that may present in an acute or subacute setting in correlation with the degree of small-bowel obstruction. Pre-operative diagnosis is difficult, as symptoms are often non-specific. A high index of suspicion should be maintained for emaciated elderly women with small-bowel obstruction without a previous abdominal operation and a positive Howship-Romberg sign. When diagnosis is in doubt, computed tomography scan of the abdomen and the pelvis (if available) or laparotomy should be performed immediately, as high mortality rate is related to the perforation of

Pararectal and obturator hernias as incidental findings on gynecologic laparoscopy.

Pararectal and obturator hernias are relatively rare and typically affect elderly emaciated women. They are difficult to diagnose preoperatively. A gynecologist may come across such hernias as a secondary finding during laparoscopy. Such findings are expected to increase with the aging population. We present two cases and the technique to treat them.

Obturator hernia: a persisting clinical diagnostic challenge - a case report.

INTRODUCTION: The presentation of a strangulated obturator hernia is rare, with it accounting for less than 0.04% of all hernias. Delay in presentation and diagnosis results in complications like bowel ischemia, necrosis, perforation, and peritonitis, thereby increasing morbidity and mortality. CASE PRESENTATION: The authors report the case of an 85-year-old multiparous woman who presented with a 3-day history of abdominal pain and vomiting. Upon examination, she exhibited hypotension, altered sensorium, and a distended abdomen with visible peristalsis. An abdominal pelvic computed tomography

Acute Intestinal Obstruction Due to Obturator Hernia: A Case Series.

Most obturator hernias are diagnosed intraoperatively due to their vague signs and symptoms. However, they are associated with a high mortality rate mainly because of the patient's age, comorbidities, and late diagnosis. We present three cases of obturator hernia in patients admitted under our care with signs of acute intestinal obstruction. All the patients were elderly with comorbidities, and they underwent open surgery with anatomical repair of the hernial defect with or without resection of any gangrenous bowel. They were discharged in good health, and during the limited follow-up period,

Rare case of bilateral incarcerated obturator hernia: a case report.

Here, we report the case of an 84-year-old woman with acute mechanical intestinal obstruction (AMIO) who was admitted to our Emergency Department. Computed tomography (CT) scan revealed an incarcerated bilateral obturator hernia, and the defect was resolved using transabdominal preperitoneal (TAPP) technique with polypropylene mesh. The patient was administered an oral regimen two days after the operation. The patient stayed in the intensive care unit for 4 days and was uneventfully discharged on the 9th postoperative day. Follow-up was scheduled at the 6th month, during which no adverse event

Rare case of obturator hernia in a patient with Marfan's syndrome.

Obturator hernia is a very rare type of abdominal hernia which constitutes <1 % of all the hernias. It is an important cause of small bowel obstruction which is associated with a high mortality rate if left untreated. Obturator hernia typically occurs in an elderly women or patients with chronically raised intraabdominal pressure or previous multiple pregnancies. We report a case of obstructed obturator hernia in a young female patient with Marfan's syndrome complicated with dissecting aortic aneurysm and chronic kidney disease. Though recurrent and incisional hernia constitutes one of the min

Obturator hernia mimicking recurrent femoral hernia: A diagnostic challenge in an elderly patient (case report).

INTRODUCTION: Obturator hernia is a rare cause of bowel obstruction, predominantly affecting elderly women. Its non-specific symptoms often delay diagnosis, increasing the risk of complications. CASE PRESENTATION: We report the case of an 86-year-old woman presenting with signs of intestinal obstruction. CT imaging identified a strangulated right obturator hernia without ischemia, managed successfully via laparotomy. This case report has been reported in line with the SCARE 2025 checklist (Kerwan et al., 2025 [1]). DISCUSSION: While laparoscopy is increasingly used in selected cases, open su

Obturator hernia associated with inclusion body myositis: a case report.

BACKGROUND: Inclusion body myositis is a progressive muscle disease characterized by weakness, specifically in the flexor digitorum profundus and quadriceps muscles, and commonly affects men over 50 years of age. No previous reports have examined the impact of inclusion body myositis on the internal and external obturator muscles. This case study suggests that inclusion body myositis-related fat infiltration of internal and external obturator muscles may contribute to obturator hernia. CASE PRESENTATION: We present the case of a 75-year-old Japanese woman admitted for management of an obturat

Case report of ureter obturator hernia and literature review analysis.

BACKGROUND: Ureteral obturator hernia is a rare condition, usually found accidentally during imaging examinations, or found during surgery. Ureteral hernia can easily lead to ureteral obstruction and hydronephrosis. Long-term hydronephrosis may lead to kidney damage and infection, and eventually cause kidney failure. As of December 31, 2020, there are only 2 literature reports. CASE PRESENTATION: This article reports a 67-year-old female patient with no symptoms. The computed tomography (CT) scan of the urinary system to show the left kidney and ureter had hydrops. The CTU imaging of the urin

Preoperative diagnosis and successful laparoscopic treatment of incarcerated obturator hernia.

Obturator hernia is rare and frequently delayed in surgical treatment and, thus, is associated with high morbidity and mortality. It is often found in thin, elderly, multiparous women. We reported a case of early diagnosis due to clinical alert and in-time computed tomography (CT). The patient received laparoscopic surgery and had good postoperative recovery. The regular use of CT for diagnosing nonspecific abdominal complaints associated with pain over the inner thigh, especially in thin, elderly, multiparous women, will result in increased preoperative detection of the obturator hernia.
