Declining functional status in older women with a history of breast cancer: implications for women's health nurses.

Older breast cancer survivors are at risk for a decline in functional status, which is associated with increased risk for disability, increased health care costs and increased risk of nursing home placement. The comprehensive geriatric assessment is a useful tool for identifying women with a history of breast cancer who are at increased risk for decline in functional status. Use of the comprehensive geriatric assessment will allow nurses to identify functional care needs and develop care plans and interventions that are aimed at maintenance of functional status.

Family history, age, and risk of breast cancer. Prospective data from the Nurses' Health Study.

OBJECTIVE: To examine prospectively the risk of breast cancer as influenced by a maternal history of breast cancer, the mother's age at diagnosis, or a sister's history of breast cancer. DESIGN: Prospective cohort study with biennial follow-up. SETTING/PARTICIPANTS: 117,988 women in the Nurses' Health Study aged 30 to 55 years in 1976, followed up through 1988 (1.3 million person-years of follow-up). RESULTS: We identified 2389 incident cases of invasive breast cancer. Compared with women without a maternal history of breast cancer, the age-adjusted relative risk (RR) of breast cancer was h

A Patient with Dementia Presents from a Nursing Home with a History of Decreased Oral Intake, Malodor, and Weight Loss.

Individuals with dementia can present with varying challenges depending on their state of disease. The individuals caring for them may be faced with resistant behaviors when trying to provide adequate oral care. A poor oral condition can lead to decreased caloric and fluid intake causing multiple comorbidities to be exasperated due to the declining oral condition. Creating a management plan and an oral disease prevention plan are key to improving the overall health outcomes of these patients.

Pilot study of the breast cancer experiences of African American women with a family history of breast cancer: implications for nursing practice.

Experts in the area of breast cancer detection and control recommend that women at increased risk discuss their risk status and risk management with their health care providers. In spite of the excessive breast cancer burden borne by African American women, little attention has been given to studying breast cancer risk communication and/or breast cancer risk management in this at-risk population group. This report summarizes the outcomes of a study undertaken to explore the degree to which breast cancer, breast cancer risk, and breast cancer risk management were discussed by African American w

History of Infertility and Plasma Markers of Inflammation, Cholesterol, and Adipokines Among Participants in the Nurses' Health Study II.

INTRODUCTION: There is increasing evidence that women who have experienced infertility are at greater risk for several chronic conditions. However, the mechanisms underlying these associations are not clear. Pathophysiology may be illuminated through observation of aberrant systemic biomarkers. However, there are limited data on infertility history and midlife biomarkers of inflammation, lipids, and adipokines. MATERIALS AND METHODS: Among participants with biomarker measurements in the Nurses' Health Study II, we used generalized linear models to assess history of infertility and plasma C-re

Mental Health Nurses' Spiritual Well-Being, Personal Trauma History, Compassion Fatigue, and Compassion Satisfaction.

INTRODUCTION: Mental health nursing can be highly rewarding but at the same time overwhelmingly stressful or even traumatizing. Spirituality constitutes a central element of mental health nurses' resilience while personal trauma may be activated during exposure to beneficiaries' trauma. AIM: The aim of this study is to examine the impact of sociodemographic and work-related characteristics, spirituality, and history of trauma on compassion fatigue (CF) and compassion satisfaction among psychiatric nurses in Greece. METHODS: A cross-sectional survey study with a total of 91 mental health nurs

The Integrated Foregut Clinical History: enabling nurse-led, joined-up initiation of suspected-cancer pathways.

BACKGROUND: Prompt diagnosis of aerodigestive cancers and other time-sensitive conditions improves survival and reduces morbidity and anxiety. However, early symptoms do not reliably differentiate between different pathologies and, in isolation, seldom localise the disease anatomically. OBJECTIVES: To disseminate a multidisciplinary foregut (head and neck, oesophagogastric, and respiratory) diagnostic dataset, using patients' integrated clinical histories to initiate diagnostic pathways, reassure patients, and communicate information between stakeholders. METHODS: 260 patients had their clin

Royal Naval nursing in Scotland and Ceylon in the Second World War: Official histories, memoirs and a representative microhistory.

The Second World War was an important, but under-researched, transitional period for naval nursing. This article describes one sister's experience and sets it against the narrative in official histories and wartime memoirs and art. Margaret Wallace, a Scot from the skilled working class, was within the demographic that the service's Victorian founders hoped would be attracted. She worked in representative wartime facilities: the largest British auxiliary naval hospital, a secret multi-national naval base, the Headquarters of South East Asia Command in Ceylon (Sri Lanka), and Haslar, the iconic

Mental health nurses' perceived preparedness to work with adults who have child sexual abuse histories.

WHAT IS KNOWN ON THE SUBJECT?: The high prevalence of adults presenting to the mental health services places mental health nurses (MHNs) in a unique position to help to identify and support the person with the associated challenges of child sexual abuse (CSA). Feelings of discomfort have been identified by mental health nurses (MHNs) when working with survivors of CSA due to a lack of knowledge, poor confidence and feeling unprepared to inquire and respond to such a sensitive topic. WHAT DOES THE STUDY ADD TO EXISTING KNOWLEDGE?: MHNs are willing to engage in CSA dialogue; however, the level o

Association of T-EAT-10 and NUFFE-TR scores with a history of aspiration pneumonia in nursing home residents: a cross-sectional study.

BACKGROUND: Aging is associated with physiological changes that increase vulnerability to malnutrition and oropharyngeal dysphagia, both of which are prevalent in nursing homes and major risk factors for aspiration pneumonia. This study aimed to evaluate the association of the Turkish Eating Assessment Tool (T-EAT-10) and the Nutritional Form for the Elderly (NUFFE-TR) with a history of aspiration pneumonia in nursing home residents. METHODS: This cross-sectional study included 415 residents aged 65 and older in Ankara, Turkey. Data were collected via face-to-face interviews using a sociodemo

Longitudinal Analysis of Nursing Home Residents' T-Cell Responses After SARS-CoV-2 mRNA Vaccinations Shows Influence of Biological Sex and Infection History.

BACKGROUND: Vaccines and vaccine boosting have blunted excess morbidity and mortality from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in older nursing home residents (NHR). However, the impact of repeated vaccination on the T-cell response based on biological sex and prior infection of NHR remain understudied. METHODS: We examined T-cell responses to SARS-CoV-2 mRNA vaccines in a cohort of NHR and healthcare workers (HCW) over 2 years. We used interferon-γ ELIspot and flow cytometry to assess T-cell response before, 2 weeks, and 6 months after the initial series an
