Suicidal thoughts and behaviors in adults with hoarding disorder.

BACKGROUND: Hoarding disorder (HD) is characterized by difficulty discarding possessions and clutter that impairs daily functioning. Previous research has demonstrated a high correlation between hoarding behaviors and suicidal thoughts and behaviors; however, there is limited research on suicidal thoughts and behaviors in individuals meeting Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria for HD. Our aim in this study was to report our findings about suicidal thoughts and behaviors in a HD sample. METHODS: We used existing data from participants meeting D

Community Partnership in Response to Hoarding Disorder in Montreal.

Clinical management of hoarding disorder is challenging because of the weak insight of people with hoarding disorder, the lack of available tools for disease management in the health care system, and the absence of communication between health care and primary responders. To tackle this communication gap and, hence, improve clinical management of hoarding disorder, a community partnership initiated by people with hoarding disorder took place in Montreal. This initiative could profitably offer guidelines for other communities facing hoarding disorder challenges.

Visually mediated functioning improves following treatment of hoarding disorder.

BACKGROUND: Hoarding disorder (HD) is a highly debilitating psychiatric disorder that affects 2-6% of adults. Neuropsychological deficits in visual memory, detection, and categorization have been reported in HD. To date, no study has examined the relationship between neurocognitive functioning and treatment for HD. We aim to determine the association between neurocognitive functioning and treatment outcomes, as well as the impact of HD-specific treatment on cognitive functioning. METHODS: 323 individuals with HD were randomized to 20 weeks of peer- or clinician-led group behavioral treatment.

Identifying hoarding disorder in the elderly using the interRAI.

OBJECTIVES: Hoarding disorder (HD) is prevalent in older adults and contributes to morbidity and mortality. We attempted to estimate rates of hoarding amongst the elderly using the international Residential Assessment Instrument - Home Care (interRAI-HC). SETTING: The interRAI is a mandatory prerequisite in New Zealand (NZ) for government-funded community support or for entry into aged residential care. PARTICIPANTS: All 50 years and older people who were assessed using the interRAI in the southern district health board of NZ. MEASUREMENTS: The age, gender, socially inappropriate or disrupt

Hoarding in Children and Adolescents with Obsessive-Compulsive Disorder.

Compared to studies in adults, there have been few studies of hoarding in children and adolescents with obsessive-compulsive disorder (OCD). In the current study, we evaluated OCD clinical features, Axis I disorders, and social reciprocity scores in 641 children and adolescents with OCD, of whom 163 (25%) had hoarding compulsions and 478 did not. We found that, as a group, youth with hoarding had an earlier age at onset and more severe lifetime OCD symptoms, poorer insight, more difficulty making decisions and completing tasks, and more overall impairment. The hoarding group also had a greater

Hoarding disorder: a new diagnostic category in ICD-11?

Despite the long-held view that hoarding is a symptom of both obsessive-compulsive disorder and obsessive-compulsive personality disorder, increased evidence has emerged during the last 20 years suggesting that hoarding represents a distinct form of psychopathology. This study reflects the discussions on the nosological status of hoarding carried out by the WHO ICD-11 Working Group on the Classification of Obsessive-Compulsive and Related Disorders. The distinctiveness of hoarding is based on its having core symptoms that differ from those of other disorders, as well as distinctive neurobiolog

Written Imaginal Exposure for Hoarding Disorder: A Preliminary Pilot Study.

Hoarding disorder (HD) is marked by difficulty discarding possessions. Many refuse treatment or drop out, which may be due to treatment's incorporation of in-home decluttering, which is feared and avoided. Thus, strategies to prepare patients for decluttering/discarding are needed. Imaginal exposure (IE), or imagining one's worst fears about discarding, could be one such strategy. This pilot preliminarily tested a short-duration IE intervention compared with a control intervention. Over 3 days, adults diagnosed with HD (n = 32) were randomly assigned to either write about and imagine their wor

Subjective cognitive complaints and objective cognitive impairment in hoarding disorder.

Individuals with Hoarding Disorder (HD) frequently complain of problems with attention and memory. These self-identified difficulties are often used as justification for saving and acquiring behaviors. Research using neuropsychological measures to examine verbal and visual memory performance and sustained attention have reported contradictory findings. Here we aim to determine the relationship between self-reported problems with memory and attention, objective memory and attention performance, and self-reported depression and anxiety symptoms in HD. Data was available for 319 individuals who p

The Association between Age and Experienced Emotions in Hoarding Disorder.

Objectives: Hoarding disorder (HD) is conceptualized as a fear-based disorder and exposure to sorting/discarding possessions is a core part of treatment. However, there has been no investigation of age-related differences in emotional reaction to sorting. The objective of this study was to explore the association between age and affective response during a sorting task. Methods: Forty-nine adults with HD completed a standardized sorting task. Participants reported their current emotion before and after the sorting task and reported their subjective distress throughout the task. Results: Older pa

Attachment theory and hoarding disorder: A review and theoretical integration.

Hoarding disorder (HD) is a severe and persistent mental illness characterized by extreme difficulty parting with possessions and considerable clutter that can result in dangerous living conditions. HD poses a considerable public health burden; however, treatment for HD remains relatively limited, as many individuals do not respond to treatment and/or do not maintain treatment gains, suggesting there are important factors not being adequately addressed. In particular, one area that is not well-understood nor well-integrated into cognitive behavioral models is the pathological attachment indivi

Obsessive compulsive disorder with and without hoarding symptoms: Characterizing differences.

OBJECTIVE: In recent years there has been some ambiguity about the way hoarding and OCD are related to each other. The present study examines the differences between persons with OCD/hoarding and OCD/non-hoarding and examines which characteristics are associated with the OCD/hoarding group. Information is established about prevalences, socio-demographical characteristics, OCD and related characteristics, OCD subtypes, comorbidity (depression, anxiety disorders and PTSD) and personality traits. METHODS: Data from baseline assessment of The Netherlands Obsessive Compulsive Disorder Association

Is the DSM-5 hoarding disorder diagnosis valid in China?

Hoarding disorder, newly included as a separate diagnostic entity in the Obsessive-Compulsive and Related Disorders section of DSM-5, has been reported to have significantly different symptoms and etiology than obsessive-compulsive disorder (OCD). However, the validity of this new diagnosis in China - where the storing of possessions is sanctioned and normalized - remains to be proven. We considered available data about pathological hoarding in East Asia and found the condition to be relatively common and symptomatically similar to that reported in western countries. We conclude that the 'Hoar

Distinct resting state functional connectivity abnormalities in hoarding disorder and major depressive disorder.

Emerging research suggests that hoarding disorder (HD) is associated with abnormal hemodynamic activity in frontal brain regions. Prior studies have not examined intrinsic network connectivity in HD during unstructured "resting state" fMRI. Furthermore, it remains unclear whether previously observed HD abnormalities might be better explained by the presence of other disorders frequently comorbid with HD, such as major depressive disorder (MDD). The current study compared resting state functional connectivity in HD-only patients (n = 17), MDD-only patients (n = 8), patients with co-occurring HD

An investigation of delay and probability discounting in hoarding disorder.

Behavioral impulsivity may be a mechanism of hoarding disorder (HD). A commonly used and well-validated measure of impulsivity is the delay and probability discounting task, which consists of making decisions about receiving monetary rewards after varying delay intervals and delivery probabilities. We compared delay and probability discounting and self-reported behavioral impulsivity in 81 patients with a primary diagnosis of HD and 45 nonclinical controls. HD participants completed the impulsivity measures before and after 16 weekly sessions of cognitive-behavioral therapy (CBT), whereas cont

Co-occurring Obsessive-Compulsive Disorder and Hoarding Disorder: A Review of the Current Literature.

Current research suggests obsessive-compulsive disorder (OCD) co-occurs in around 20% of people with hoarding disorder (HD). The article discusses the theoretical conceptualization of co-occurring HD and OCD (HD+OCD), highlighting similarities between the disorders that may contribute to comorbidity, such as potentially overlapping etiological factors, comorbidity profiles, and phenomenological aspects; and differences that are important to consider in differential diagnosis and conceptualization, such as belief patterns, ego-syntonicty/dystonicity, and trajectory. The combination of HD+OCD ve

Who really hoards? Hoarding symptoms in adults with attention-deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD) and healthy controls.

UNLABELLED: Although hoarding disorder (HD) is included in the DSM-5 in the obsessive-compulsive and related disorders chapter, in the last few years, HD has been consistently associated with attention-deficit hyperactivity disorder (ADHD). Some studies on HD patients show higher comorbidity with ADHD than with OCD and some studies on ADHD patients found significant higher rates of HD symptoms compared to the general population. However, the aim of the present study was to be the first direct comparison of the prevalence of HD and HD symptoms across adults with a primary diagnosis of ADHD, OCD

Loneliness in hoarding disorder: a systematic review and meta-analysis.

INTRODUCTION: According to attachment theory, loneliness may be a cause of hoarding symptoms. However, due to mixed findings, it is unclear whether people with hoarding disorder (HD) are lonelier than healthy and clinical controls, whether there is a significant positive association between loneliness and hoarding, and whether loneliness causes more severe hoarding. METHOD: We conducted a systematic review and included peer-reviewed quantitative cross-sectional, longitudinal, experimental and intervention research that measured hoarding and loneliness. We excluded hoarding due to other condit

Daily Functioning in Low-Income Older Adults With Hoarding Disorder.

OBJECTIVES: Hoarding disproportionately affects older adults and has been linked with functional and cognitive impairment. However, existing work investigating is limited by methodological issues (e.g., absence of diagnostic interviewing) and little attention to low-income individuals or older adults. The present study sought to address these limitations in a large sample of low-income older adults diagnosed with hoarding disorder. METHODS: We administered measures of functional disability, hoarding severity, and cognitive functioning to 169 low-income older adults with hoarding disorder. RE
