Home blood pressure monitoring. Effect on use of medical services and medical care costs.

The objective of this study was to determine whether a hypertension management program in which patients monitor their own blood pressure (BP) at home can reduce costs without compromising BP control. The prospective, randomized, controlled 1-year clinical trial was conducted at four medical centers of the Kaiser Permanente Medical Care Program in the San Francisco Bay Area. Of 467 patients with uncomplicated hypertension who were referred by their physicians, 37 declined to participate in the study; 215 were randomly assigned to a Usual Care (UC) group and 215 to a Home BP group. Twenty-five

[Frequency of falls among clients of home care services : The importance of care-related and social risk factors].

BACKGROUND: Among the elderly falls frequently result in injuries, increase the need for long-term care and pose a challenge for the quality assurance in nursing care. We describe the frequency and risk factors of falls among care-dependent persons using home care services in Germany. METHODS: The participants of the study "Nursing-related health problems in home care" (n = 880) were recruited based on a list of the officially accredited nursing services. Data collection followed a standardized study protocol. Proportions were calculated and adjusted odds ratios and predicted probabilities we

Factors affecting the use of home-based services and out-of-home respite care services: A survey of family caregivers for older persons with dementia in Northern Norway.

BACKGROUND: Family caregivers contribute substantially to the care for older home-dwelling people with dementia, although community healthcare services tend to be underutilized. In this study, we aimed to explore the use and predictors of use of home-based and out-of-home respite care services available to older home-dwelling persons with dementia, as reported by the family caregivers. METHOD: A cross-sectional survey was administered to family caregivers (n = 430) in Northern Norway during April to November 2016. The use of healthcare services was categorized into two types according to prin

Factors associated to Clostridium difficile infection in a hospital-based home care service.

BACKGROUND AND OBJECTIVE: There are no data related to the risk factors associated with CDI in a Hospital-Based Home Care Service (HBHCS) of the Spanish health system. PATIENTS AND METHODS: Case-control study. The cases were patients admitted to the HBHCS between 01/01/2011 and 31/12/2016 who developed CDI. The controls came from the same population, with suspected CDI and CD(-) toxin. We analysed 82 variables. RESULTS: We analysed 17 cases and 95 controls, without differences in sex, age or comorbidity. Diarrhoea was noted in 94% and 92%, and a percentage of deaths of 18% and 1%, respective

Experience of care home residents with Parkinson's disease: Reason for admission and service use.

AIM: The care needs of people with Parkinson's disease (PD) are poorly understood. We aimed to investigate the factors that precipitate entry to institutional care, and on-going care needs once in care, within a representative cohort of community-dwelling people with PD. METHOD: All people with idiopathic PD, Parkinson's plus syndromes and vascular parkinsonism under the care of the Northumbria Healthcare NHS Foundation Trust PD service who were living in care homes on 1 January 2013 were included. Disease severity (Hoehn and Yahr stage) and demographic data were collected. Admissions to hosp

Preventive service gains from first contact access in the primary care home.

BACKGROUND: The patient-centered medical home (PCMH) concept recently has garnered national attention as a means of improving the quality of primary care. Preventive services are one area in which the use of a PCMH is hoped to achieve gains, though there has been limited exploration of PCMH characteristics that can assist with practice redesign. The purpose of this study was to examine whether first-contact access characteristics of a medical home (eg, availability of appointments or advice by telephone) confer additional benefit in the receipt of preventive services for individuals who alread

The explosion in paid home health care in the 1990s: who received the additional services?

OBJECTIVE: Public expenditures for home health care grew rapidly in the 1990s, but it remains unclear to whom the additional services were targeted. This study tests whether the rapidly increasing expenditures were targeted to the elderly with high levels of disability and low levels of social support, 2 groups that have historically been higher users of paid home health and nursing home services. METHODS: The Asset and Health Dynamics Study, a nationally representative, longitudinal survey of people > or = 70 years of age (n = 7,443), was used to determine the association of level of disabil

[Prevalence and influencing factors of chronic wounds among clients of home care services in Germany].

INTRODUCTION: Chronic or non-healing wounds are a serious problem for both the parties involved and the healthcare system. Currently, there are hardly any reliable data on the prevalence of chronic wounds in outpatient care, although this setting is becoming increasingly important. Therefore, the aim of this study was to determine the prevalence of and the factors associated with chronic wounds in clients cared for by home care services. METHOD: As part of a cross-sectional study throughout Germany in 2012, a survey was conducted among care recipients provided by home care services. The sampl

[Abuse and neglect of older care recipients in domestic settings - a survey among nurses of in-home care services].

Although some anecdotal evidence for the phenomenon of abuse and neglect of community-dwelling older care recipients by in-home care services exists, there is an almost complete lack of data on this topic. In order to determine extent and risk factors of abuse and neglect of older care recipients by nurses, we conducted a self-report study among in-home care workers in the German city of Hanover. A total of 503 nurses took part in the study; the response rate was 43.3%. Nearly 40% of all respondents reported having abused or neglected at least one patient within the last 12 months. The Uses and Experiences of Synchronous Communication Technology for Home-Dwelling Older Adults in a Home Care Services Context: Qualitative Systematic Review.

BACKGROUND: European health care systems regard information and communication technology as a necessity in supporting future health care provision by community home care services to home-dwelling older adults. Communication technology enabling synchronous communication between 2 or more human actors at a distance constitutes a significant component of this ambition, but few reviews have synthesized research relating to this particular type of technology. As evaluations of information and communication technology in health care services favor measurements of effectiveness over the experiences a

How are HIV services in the UK and Ireland managing care home residents living with HIV?

PURPOSE: The average age of people living with HIV in Europe is rising and with it the likelihood of entering long-term care. Little is known about the characteristics of people living with HIV in care homes and how their HIV care is delivered. METHODS: We conducted a multi-site, cross-sectional descriptive evaluation of care home residents receiving HIV care at seven centres in the UK and Ireland. Data included demographics, HIV history, comorbidities, medication use and HIV service contacts over the preceding 12 months. RESULTS: The sample included 62 residents with a median age of 61 year

Antipsychotic use among older patients with dementia receiving home health care services: Prevalence, predictors, and outcomes.

BACKGROUND: Antipsychotic use is a safety concern among older patients in home health care (HHC), particularly for those with Alzheimer's disease and related dementias (ADRD). The objective of this study was to examine the prevalence and predictors of antipsychotic use among older adults with and without ADRD who received HHC, and the association of antipsychotic use with outcomes among patients living with ADRD. METHODS: In this secondary analysis of adults ≥65 years receiving care from an HHC agency in New York in 2019 (N = 6684), we used data from the Outcome and Assessment Information Set

Time-limited home-care reablement services for maintaining and improving the functional independence of older adults.

BACKGROUND: Reablement, also known as restorative care, is one possible approach to home-care services for older adults at risk of functional decline. Unlike traditional home-care services, reablement is frequently time-limited (usually six to 12 weeks) and aims to maximise independence by offering an intensive multidisciplinary, person-centred and goal-directed intervention. OBJECTIVES: To assess the effects of time-limited home-care reablement services (up to 12 weeks) for maintaining and improving the functional independence of older adults (aged 65 years or more) when compared to usual ho

Composition of the home care service package: predictors of type, volume, and mix of services provided to poor and frail older people.

This study of 270 poor and frail elders in a Medicaid waiver program examined the service package, that is, the type of service (health or social), volume, and the mix or combinations of services provided. Predictors of use and volume of service differed depending on type of service. The most frequently prescribed service combinations were: (a) nursing, home health, and homemaker; (b) homemaker only; (c) nursing and home health; (d) nursing and homemaker; and (e) nurse only. Across the service combinations, living alone increased the odds of receiving social services such as a homemaker, where

Understanding research priorities in home health care: a topic modeling analysis of the health services literature.

BACKGROUND: Home health care (HHC) has become vital amid global aging, rising chronic disease burdens, and shifts toward community-based care. Yet, research priorities remain fragmented. This study aimed to map the evolution of HHC scholarship and identify emerging and underexplored themes. METHODS: We analyzed 39,548 PubMed-indexed HHC publications (1952-April 2025) using Latent Dirichlet Allocation topic modeling. A hybrid Konstanz Information Miner-Python workflow supported preprocessing, parameter optimization, and topic extraction. Model performance was assessed with perplexity, coherenc
